The influence of jejuno-ileal bypass surgery on mineral metabolism was studied in patients with morbid obesity before operation, and 2 and 5 years after-wards. When calcium and potassium were orally substituted post-operatively, in serum calcium fractions, parathyroid hormone, phosphate, potassium and the bone mineral content remained unchanged, while serum magnesium decreased. Serum 25-hydroxyvitamin D was already low before bypass operation, and did not change thereafter. Post-operatively, the urinary excretion of oxalate rose into the upper normal range, while that of calcium, magnesium and citrate was markedly reduced. The urinary activity product of calcium oxalate rose slightly, but that of brushite decreased. Since these changes were manifest in the urine of all patients, the reasons for the post-operative formation of renal stones in 4 of 19 patients remain unclear at the moment. We conclude that the recommendation for precise oral calcium substitution after jejuno-ileal bypass operation seems justified in order to avoid serious disturbances of calcium metabolism in the long term, and to reduce intestinal oxalate absorption.

The prognosis of gastric carcinoma is particularly unfavorable. 58% of 966 patients could be resected at the time of diagnosis, only 25% radically. The absolute 5-year survival rate is 18%, the 10-year survival rate 12%. After a curative resection 43% survive 5 years and 28%, 10 years. "A priori" gastrectomy showed no advantage in our patients. Therefore we prefer resection adapted to stage and grade for gastric cancer.

Incidence and distribution of lymph node metastases were examined in 100 gastrectomy specimens with 104 primary gastric carcinomas. Special emphasis was placed on the correlation with the location of the tumor and its size as well as the depth of infiltration and the tumor histology. Sixty-one percent of carcinomas with a diameter of up to 6 cm had lymph node metastases while this was the case in 83% of larger tumors. Infiltration of the muscularis propria only was associated with a 23% incidence of lymphogeneous metastases. This figure rose to 75% if the tumor reached the subserosa. Carcinomas of the diffuse (Lauren) or infiltrating (Ming) type show metastases more frequently (83% and 81% respectively) than those of the intestinal or expansive type (58%). In addition, several regions of lymphatic drainage are more frequently involved with the diffuse type. In only 2 patients (2% of all patients or 3% of the patients with lymphogeneous metastases) skipping of lymph nodes could be observed. In all other patients distant lymph nodes showed metastases only if the perigastric nodes next to the tumor were involved also. These findings support a differentiated, stage- and histology-dependent surgery.
The aim of this study was to determine whether there is a relationship between sedentary occupation and the occurrence of rectal and sigmoid cancer. 105 patients with histologically proven rectal cancer were questioned. 55 patients with gastric carcinoma and 99 patients with cholelithiasis served as control groups. The percentage of patients with a sedentary occupation is significantly higher in the group with rectal cancer. In addition, the mean duration of the sedentary occupation is also longer. A direct relationship between the sedentary occupation and the tumor stage is obvious. It is concluded that a sedentary occupation-which leads to changes in the motility of the large bowel-had to be considered a risk factor in the occurence of rectal and sigmoid cancer.
Of 137 patients operated on for adenocarcinoma at the esophageal-gastric junction by left-sided thoracotomy, only 25% were resectable radically, 25% relatively radically. Because of local or distant metastases the tumor was resectable palliatively in 36% of all patients. The 3-year-survival rate after curative resection is 38%. The left-sided approach to a cardia tumor allows good survey and enables radical resection and lymphonodulectomy. The complication rate is lower than after abdominothoracic incision.
154 patients out of 317, who had undergone the Converse type of otoplasty in the ENT Department at Göttingen University, were assessed. Both subjective results and objective findings showed a successful operative outcome. Retractions of the concha were seldom seen and revision surgery was only necessary in 2 patients.
In animal experiments (20 mongrel dogs) malabsorption was induced by resection of 100 cm ileum. Subsequently the influence of the ileocoecal valve (Valvula Bauhini) in absorption was studied. In one group of animals intestinal continuity was reestablished by ileoileostomy, in a second by ileocolostomy. The results of the absorption studies were compared with a healthy control group. After ileoileostomy animals do not lose weight, serum cholesterol and protein remain normal. Oral tolerance tests with Triolein-iodine-131 and Cyanocobalamine show results identical with those of the control group. After ileocolostomy all these parameters are lowered--partially to a pathological level. It is concluded that in all operations in the area of the terminal ileum and colon the Valvula Bauhini should be retained if possible.

Jejunoileostomy is an effective procedure in the treatment of morbid obesity. The end-to-end and end-to-side anastomoses are competitive techniques. The end-to-end anastomosis prevents a reflux of food into the shunted intestine loops, but stool reflux to the colon very often occurs over the drainage anastomosis. The patients develop an immunological disease with fever and arthritis. Resection of the infected intestinal loop is the standard therapy. The immunologic manifestation after jejunoileostomy does not occur after end-to-side anastomosis; end-to-end anastomosis should therefore no longer be used.
The gastric bypass is an effective surgical procedure for the treatment of extreme obesity. A small gastric reservoir and a narrow anastomosis are combined (to limit the possibility of food intake). In patients with a mean weight of 94% above the Broca value the normal weight is reached 18 months after the operation. Failure to lose weight can be caused by technical errors during the procedure or by a continuous food intake by the patients. Surgical complications are rare. The operative lethality is less than 1%, and anastomotic ulcers occur in 2% of cases. There is no reason to expect metabolic derangements as after jejunoileal bypass.
A follow-up examination was possible in 63 patients from a total of 323 with a total gastrectomy. All patients gained weight. The laboratory findings are in the normal range except for the low level of serum carotene and vitamin A. Carotene is greatly reduced by duodenal bypass or a short interposition of a small intestine loop. Radiological, endoscopic and histological findings correlate closely. Also, a reflux into the lower third of the esophagus causes clinical discomfort in 2/3 of all patients after esophagojejunostomy with end-to-side anastomosis. The interpretation of the negative points in conjunction with follow-up findings demonstrates the advantage of Longmire's interposition of a long small intestine loop for post-gastrectomy reconstruction.
