[Cardiac rupture: histological study of ruptured and non-ruptured myocardial infarcts (author's transl)].

178 myocardial infarcts with wall rupture and 116 without were studied histologically. Mean morphological age of the infarct was 3.7 in the ruptured and 2.9 days in the non-ruptured cases. 57.7% of patients had died within the first 24 hours from a non-ruptured infarct. Death rate within the first 24 hours after infarction was 57.7% among the non-ruptured cases, 25.8% among those with rupture. 23.1% of infarcts had ruptured between the third and fourth day. Only 5.1% of patients with non-ruptured infarcts had died on the third or fourth day. Transmural infarction was present in 71.7% of those that had ruptured. The ruptured channel in general ran through the middle of the necrotic-infarcted, leucocyte-infiltrated myocardium. Infarct recurrence was demonstrated in 20% of ruptured and 73% of non-ruptured infarcts. Morphological comparison indicated that transmural infarction and marked leucocytic reaction in a not previously infarct-damaged myocardium are the main causes for rupture in myocardial infarction.

Aged

Megakaryocytopoiesis in the spleen of growing rats.

Age-dependent changes in splenic megakaryocytopoiesis of Wistar rats, 1 hour to 60 days old, were investigated using autoradiograms of semi-thin sections. The highest values for 3H-thymidine labeling and mitotic indices were observed immediately after birth, whereas the maximal concentration of megakaryocytes was seen on the 14th day after birth. The transit time of megakaryocytes in newly born rats was 34.5 hours and, thus, much shorter than in adult animals.

The biological significance of atypical hyperplasia of the prostate.

In tissue biopsies and resection material (TUR) of the prostate a high coincidence (49.4%) was found between atypical primary hyperplasia with atypical-dysplastic microglandular, adenomatous and cribriform structures on the one hand and carcinomas on the other. The frequency of atypical hyperplasia in prostatic tissue without carcinoma was 2.8%. Microglandular pattern predominates in atypical hyperplasia combined with differentiated adenocarcinomas. A high coincidence between cribriformly structured glands of atypical primary hyperplasia and solid anaplastic-cribriform carcinomas can be observed. Autoradiographically the labeling index of atypical hyperplasia was three times as high as that of simple hyperplasia. The mean labeling index of atypical hyperplasia, however, was similar to that of poorly differentiated adenocarcinomas and cribriform carcinomas. The similar proliferation pattern of atypical hyperplasia and carcinomas as well as the high coincidence between both indicate that severe atypical primary hyperplasia is a precancerous lesion. Therefore, those patients with primary atypical hyperplasia with distinct cellular and structural atypia but without manifest carcinomas in prostatic biopsy or resection material should be followed up at short intervals.

Adenocarcinoma

[Route of infection of the biliary tract: experimental evidence for an enterohepaticobiliary bacterial cycle (author's transl)].

Suspensions of Escherichia coli bacteria were injected into the colon of ten mongrel dogs. Primarily, the cystic duct and the distal choledochal duct had been ligated. Choledochal bile was continuously collected with a ductal catheter. The bacteria could be cultured from the portal vein blood, the liver, and the gallbladder wall 5 h after injection, but not from choledochal or gallbladder bile. Suspensions of tritiated Escherichia coli bacteria were injected into the portal vein of another 20 mongrel dogs. Now the bacteria could be cultured from the liver, the gallbladder wall, the choledochal and the gallbladder bile 10 min after injection. Elevated radioactivity counts could be found in the tissue and bile samples. In the liver and gallbladder wall, the tritiated bacteria could be seen in phagocytes and capillary ducts by autoradiography. We conclude that there is an enterohepaticobiliary bacterial cycle which could explain the pathogenesis of infections in biliary tract diseases.

The granulocytopoiesis in the spleen of newborn rats.

The granulocytopoiesis in the spleen of newborn, 1h-old rats was analysed. It was found that the transit times of mycleocytes and metamyelo cytes were remarkably shorter than in adult animals, and that the mitotic frequency of myelocytes was only nearly half as high. The average generation time of immature myelopoietic cells was estimated to be 8.4h, the S-phase duration 4.0h. The findings may be attributed to an elevated input from stem cells in contrast to the undisturbed granulocytopoiesis of adult rats, which is obviously maintained by a higher divisional rate.

[Dysgerminoma in pure gonadal dysgenesis (author's transl)].

By modern cytogenetic methods gonadal dysgenese are more often and exactly clarified than in former times. The cytogenetic analyses have shown that pure XY gonadal dystenesis is often combined with germ cell tumors. A hypergonadotropic ovarian insufficiency as well as a rapidly growing tumor of the left adnexa were diagnosed in a 26-year old woman. At abdominal hysterectomy and bilateral adnexectomy a dysgerminoma on the left side was found as well as on the right side gonadal stroma and rudiments of the Wolffian duct. Blood lymphocytes showed a 46, XY karyotype. The H-Y antigen determination was a positive intermediary titer as compared with normal female and male controls. The cytogenetic demonstration of a pure gonadal dysgenesis in patients with dysgerminoma is important because bilateral development of tumors may occur. Therefore, hysterectomy with bilateral adnexectomy should be carried out.

Adult

[Mucous adenocarcinoma of the urinary bladder (urachal carcinoma) (author's transl)].

A report is presented of 5 cases of urachal carcinomas (3 women and 2 men). The mucous adenocarcinomas were located on the apex of the urinary bladder. In 3 cases partial cystectomy and in 2 cases a subtotal cystectomy were performed. Two patients were treated with cytostatics postoperatively. The prognosis of urachal carcinomas is poor. 3 patients had metastases to the bones, cerebrum and peritoneum. Diagnostic and therapeutic procedures are discussed.

Adenocarcinoma, Mucinous

[Actinomycosis of the liver (author's transl)].

A case report is given of a 61 year old female patient, who came to the hospital with chronic pain in the right upper abdominal quadrant, fever, chills, dyspnea, and general weakness. 4 days later she died in hemorrhagic shock after having had melena. At postmortem an actinomycotic abscess of the size of a childs head was found in the left lobe of the liver; in addition there was a highly differentiated leiomyosarcoma of the duodenum. Isolated visceral actinomycosis is rare; it should however be thought of when the causes of liver abscesses are being scrutinized. Diagnosis can be established only bacterioiogically or histologically.

[Morphology of late hepatic lesions after thorotrast (author's transl)].

Late lesions of the liver after longstanding exposure to thorotrast are the following: adaptive and degenerative alterations of hepatocytes, proliferation and atypical morphology of sinusoidal cells, ectasia of sinusoids, portal fibrosis due to inflammatory as well as non-inflammatory changes with transition to complete cirrhosis. Pronounced changes of sinusoidal cells and sinusoidal shape occur most frequently in the immediate vicinity of angiosarcomas. Just as in chronic vinyl chlorid intoxication these changes may represent an early stage of angiosarcoma, or may be due to the fact, that an angiosarcoma is close to the hepatic area, where the biopsy specimen was taken from. However, liver tissue surrounding carcinoma induced by thorotrast usually shows only noncharacteristic alterations, except in cases, where cirrhosis of the liver is present.

[Thorotrastosis: survey and CT pattern (author's transl)].

Following a historical survey of the discovery of late damages caused by the contrast medium Thorotrast which was commercially available from 1930 to 1951, the article reports on the possibilities of identifying Thorotrast in the computer tomogram. In a male patient aged 60 years, the size and anatomic dimensions of a hepatocholangiolar carcinoma induced by Thorotrast was determined by computer tomography 35 years after carotid angiography. Over and above this, a 20 times increased pathologic tissue density was measured in parapancreatic lymph nodes, whereas the density of tissue was only slightly increased in the liver and twice the normal value in the spleen. The sclerosing and malignoma-inducing properties of Thorotrast as well as the pathogenesis of late damages by foreign body irritation and chronic internal radiation, are discussed.
