Tissue culture, electron microscopic and enzyme histochemical investigations of extraadrenal paragangliomas.

Light and electromicroscopical as well as histochemical investigations were performed on three cases of extraadrenal paragangliomas. They were localized in the carotid body, tympanicum and cauda equina region. Tissue of two cases was cultivated in vitro in nutrient medium TCM 199. The tumours were classified as paragangliomas of the paraganglionic type with typical cell clusters, of the adenomatous and angiomatous type. The enzyme histochemistry showed a very high dehydrogenase activity. Ultrastructurally numerous typical osmiophilic granules could be observed in the cytoplasm of the tumour cells. In tissue culture only a minimal cellular proliferative activity could be detected. The few proliferating cell colonies showed mostly characteristics of epithelial tissue and sometimes a similar behaviour to cells of a ganglioneuroblastoma. The minimal proliferative activity in vitro is in good agreement with the proliferative behaviour of the extraadrenal paragangliomas in vivo.

[Report on 2 cases of malignant hyperthermia with different clinical courses].

Two cases of malignant hyperthermia with different clinical courses are reported. The patients showed the classical signs of malignant hyperthermia consisting of tachycardia, tachypnoea, ocasional peripheral cyanosis, high body temperature as well as characteristic rise in serum enzymes. In one of the patients the symptoms were recognized early during the operation. The immediate commencement of therapy with ice-cooled. Ringer-Lactate-Solution, Procainmedication, Corticoids as well as physical body cooling favourably influenced the clinical course and the patient survived. In both cases the patients underwent succinylcholine and halothane anaesthesia, but the symptoms of the second patient appeared after the reduction of anaesthesia. In spite of vigorous therapy the hyperpyrexia resulted in heart arrest and death. Morphologically, both patients showed signs of preexistent myopathy with volumetric alterations of the muscle fibres, centralisation of the nuclei and acute muscle fibre necrosis. On the basis of the observed variable course, the various symptom complexes reported in the literature to data are reviewed. A detailed discussion of the "carrier problem" and the available treatment possibilities is also made. Realising that malignant hyperthermia is an inheritable disease, prophylactic measures such as, f.i. the issue of medical certificates to the patient and his relatives are suggested.

[Comparative studies of simultaneous intraoperative frozen sections and imprint cytology of the mamma (author's transl)].

250 tissue biopsies of the mamma were examined simultaneously by intraoperative histology (frozen sections) and imprint cytology. In 95% of the cases there was a good agreement between the histologic and cytologic diagnoses. The reliability of the imprint cytology was tested in some complicated cases such as proliferating fibroadenomas, sclerosing adenosis and carcinomas and the significance of the imprint cytology for the diagnosis of difficult cases was pointed out in detail. The results have demonstrated, that such a cytologic examination can not be a substitute for intraoperative frozen sections - but the simultaneous cytologic examination of unfixed mamma biopsies can be a good searching method and can be helpful in the intraoperative diagnostic.

Histological and autoradiographical findings in the immunologically stimulated spleen.

In order to estimate the role of a possible immunological coeffect on the splenic cellular proliferation during wound healing after mechanical or thermal lesions of internal organs, light microscopical and autoradiographical investigations with tritiated thymidine were performed on spleens of non-germfree rats, immunized by a single injection of sheep erythrocytes. Two days after an initial dissociation of preexisting germinal centers in the spleen a steep rise of the percentages of labeled cells in newly formed germinal centers occurs with a maximum on the fifth day. In the marginal zone an increased cell proliferation (mostly large lymphoid blasts) starts at 12 hours. In the lymphatic mantle zone a marked increase of labeled basophilic blasts (immunoblasts) can be observed with a maximum on the second day. The values are higher in the perifollicular B cell region than in the periarteriolar T cell region. In the red pulp of the spleen the highest percentages of labeled cells occur five days after the antigen injection. These findings in the spleen after a strong antigenic stimulation are characteristic for an immunological reaction of anamnestic type. In comparison with investigations of the spleen after cryolesions in internal organs such studies may be particularly helpful in judging the proportion of an unknown immunological reaction in the spleen after such an operation, which by tissue necroses and cellular destruction may have caused denaturation of self components.

The cell proliferation of epithelial metaplasia in the prostate gland. An autoradiographic in vitro study.

In 147 autoradiographically examined prostate biopsy cylinders 5.4% squamous metaplasia and 6.0% transitional metaplasia were diagnosed. The average labeling index of squamous metaplasia was 4.3% and corresponded with values from non-keratinizing squamous epithelium in other locations. The labeling index of the transitional cell metaplasia was, in contrary, almost 10 times lower with an average value of 0.29% and corresponded with the values well known for the urothelium of the urinary bladder. Higher labeling indices were observed in 2 cases but these had chronic prostatitis. The autoradiographic results are in good agreement with the ultrastructural findings in which the metaplastic squamous epithelium corresponds with normal squamous epithelium by which the cellular proliferation takes place only in the stratum basale. On the other hand, transitional metaplasia exhibits DNA-synthesizing cells in the superficial cell layers just like in the urothelium.

[Histological, histochemical, and ultrastructural findings in malignant hyperthermia (author's transl)].

Malignant hyperthermia is a rare but severe complication of modern anesthesia, induced by halothane and succinylcholine. The syndrome is characterized by a rapid sustained and extreme rise in body temperature associated with muscular rigidity, tachycardia, tachypnoea and cyanosis. The lethality is about 60%. The present paper describes the histological, histochemical and electron microscopical findings performed on muscle biopsies of 3 patients with malignant hyperthermia (1 patient died) and a so called risk patient. In all patients morphological findings consistent with a pre-existent myopathy were found. Histologically there were acute necrotic muscular fibers as well as in types I and II, variations in the fiber diameter and centralization of the nuclei. In two cases even fibers that had a normal aspect in HE slides, showed a pathologic pattern after phosphorylase reaction. In addition to acute rhabdomyolysis, electron-microscopic investigations revealed cystic expansion of the cisterns of the sarcoplasmic reticulum with a peculiar proliferation of the sarcolemma. In a degenerating mitochondrium, a crystalline inclusion was identified. These findings support the pathogenetic concept of Britt and coworkers of a functional defect in the calcium release or binding mechanism of sarcoplasmic reticulum. Since it is known that malignant hyperthermia has a familial predilection, it seems very important that clinical, biochemical, and morphological investigations be performed such as CPK estimations and muscular biopsies not only of the patients but also of the relatives in order to rule out this type of latent myopathy.

The proliferation of prostatic epithelium in chronic prostatitis.

The labelling index in chronic nonspecific prostatitis ranges from 0.7-2.1%. This value is markedly higher than in nodular hyperplasia (0.4%) as well as in differentiated adenocarcinomas (0.6%). Increased labelling index could only be determined in glands adjacent to inflammatory foci. In recognition of these findings we believe that a correct evaluation of the cell proliferation in chronic prostatitis is only possible if each single cell can be evaluated, whether it is altered by inflammation or not. Therefore, solid tissue biopsies appear to be more suitable in autroadiographical studies than cytological material alone.

[Metastasising carcinoma of the renal pelvis after Thorotrast pyelography].

A carcinoma of the renal pelvis was diagnosed in a female patient 42 years after a retrograde Thorotrast pyelogram. The patient died as a result of the carcinoma at the age of 60. The literature is reviewed. As a result of this case, the need is stressed for early recognition of Thorotrast depositis in the kidney. In this case it was demonstrated by autoradiography. Appropriate treatment is discussed.

[Neurogenic gastric tumours. Schwannomas and neurofibromas (author's transl)].

The literature concerning the definition, frequency, development and morphology of neurogenic gastric tumours is reviewed. The clinical, radiological and histological findings in eleven cases seen by the authors are described. The radiological differential diagnosis and errors in diagnosis are discussed. The significance of tumour perforation in making a type diagnosis is pointed out. There are few angiographic reports available at present.
