We have studied liver biopsies obtained in 12 hyperlipoproteinemic (HLP) patients (type II, 6; type IV, 6) treated with diet and fenofibrate, and in 15 patients (type II, 11; type IV, 4) receiving diet only. Electron microscopy of liver biopsies and the morphometric analysis according to the method of Weibel and Rohr showed mitrochondrial changes in patients treated with fenofibrate, these changes depending on the type of hyperlipoproteinemia. In type II HLP, we found a decreased volume of normal mitochondria (fenofibrate, 125.72 +/- 17.04 X 10(-3) cm3/cm3; diet only, 185.84 +/- 8.96 10(-3), P less than .05). In type IV HLP we found a decreased number of giant mitochondria (fenofibrate, 0.08 +/- 0.03 X 10(10) cm-3; diet only, 0.32 +/- 0.08 X 10(10) cm-3, P less than .05) and a decreased volume of altered mitochondria (fenofibrate, 6.00 +/- 1.44 X 10(-3) cm3/cm3; diet only, 13.61 +/- 1.17 X 10(-3), P less than .05). In contrast with the rodent studies, the present study shows no change in the number of volume of peroxisomes.
Several morphological alterations of the basement membrane of the seminiferous tubules have been reported in the patients presenting with a testicular sterility. The proteoglycans are one of the major components of the basal membrane and may be ultrastructurally defined by the use of the cationic marker polyethyleneimine (PEI). In comparaison to the normal distribution of the anionic sites labelled by PEI which is characterized by a regular pattern of distribution along both the epithelial and the interstitial faces of the lamina densa, the pattern of distribution of the sites is largely altered in 6 patients, 27 to 40 years of age, with oligo- or azoospermia. Several types of alterations are reported: parallel thickening of the lamina densa with an abnormal distribution of the anionic sites inside the lamina densa, stratifications of the basal membrane depressing the seminiferous epithelium with anionic sites labelled on both faces of the lamina densa-like layers, massive expansions of the lamina densa with either a regularly circumferential labelling of the sites or a random type of distribution. The digestion of the sites by the enzyme heparitinase is highly suggestive of the presence of proteo-heparan-sulfate. The analysis of the distribution of the anionic sites may represent a usefull tool for studying the pathogenesis of testicular sterility.
The authors have studied 50 samples of patellar cartilage, normal and pathological, obtained by biopsy, using a scanning electron-microscope. The normal cartilage shows characteristic regular wave-like undulations. The size of these waves varied between 5,000 and 7,000 nm and the undulations were linked by regularly-spaced fibrillary bridges. In a case with an osteochondral fracture, the fracture site was covered by peripheral cartilage, as had previously been shown by Ghadially. Eighteen cases of patellar chondropathy were divisible into three groups: group I (four cases) showed fibrillary chondropathy, of cerebriform appearance and without fibres. Group II (12 cases) showed a fibrillary chondropathy with loss of geometrical regularity and a staghorn appearance. Group III (two cases) showed an oedematous chondropathy with destruction of fibres. In group II, the presence of bundles of fibres with microfractures indicates a potential for progress to frank arthrosis.
A morphometric study on liver biopsies from patients with primary hyperlipoproteinemia (IIa n = 4, IIb n = 7, IV n = 7) and in controls (n = 7) was performed by light and electron microscopy. We found hypertrophy of peroxisomes in all subjects with hyperlipoproteinemia. As none of our patients had been given lipid-lowering drugs, this increase in volume seems to be due to hyperlipoproteinemia. Hypertrophy of peroxisomes in hyperlipoproteinemia may constitute a response of the hepatocyte to a disturbed metabolic state. Alterations of peroxisomes in hyperlipoproteinemia may thus occur prior to any lipid-lowering therapy.
We obtained liver biopsies in 4 patients with type IIA and 7 patients with type IIB hyperlipoproteinemia, and also in 7 healthy controls. A morphometric analysis of liver tissue was carried out by light and electron microscopy. Structural alterations of the hepatocyte and its organelles in type IIA and IIB hyperlipoproteinemia seem to fall into different categories: smaller nuclei, enlarged peroxisomes and altered mitochondria are found in both types of hyperlipoproteinemia, while an increase in the volume of hepatocytes is found only in type IIA. The enlargement of liver peroxisomes in untreated hyperlipoproteinemia is noteworthy, since it has been considered as a marker of the liver toxicity of lipid-lowering drugs.

Mosaic tetrasomy 12p is a dysmorphic syndrome which has been described under the name of Pallister mosaic syndrome and Teschler-Nicola/Killian syndrome and has sometimes been incorrectly interpreted as tetrasomy 21. Here we report the first case to be diagnosed prenatally and confirmed by enzyme assays, and we summarize the clinical and biological characteristics of all the cases reported so far under various names.
The authors report 8 new cases of granular cell tumour of Abrikossoff's tumour located in the trachea or bronchus. After briefly recalling the generally accepted features of this disease, they report several new aspects: the obviously under-estimated frequency, the absence of progression, requiring therapeutic abstention and regular follow-up, the epidemiological problems posed by the association with chronic bronchitis or the coexistence with a malignant bronchial tumour, the uncertainties which still surround the histogenesis, which is partly mesenchymal and partly nervous tissue, more particularly Schwann cells, which can be supported by ultrastructural arguments.
Three cases of triploidy have been diagnosed prenatally on ultrasonography and clinical features. A morphological and histological study of both foetus and placenta is made. The cases and a review of live-born triploid infants published in the literature allow a description of some characteristic malformations. Possibilities of triploïd pregnancy recognition are discussed.
Early lesions of the exocrine pancreas of the Syrian Golden Hamster induced by lifetime weekly oral or subcutaneous administration of N-nitroso-bis (2 hydroxy propyl) amine (250 mg/kg) were studied by light and electron microscopy and autoradiography. There were no pathological differences according to the way of administration. The histogenesis of induced pancreatic neoplasms may be summed up as follows: hyperplasia of ductular cells with new islet cell formation that lead to the development of peri or intra insular cystic ductules which become cystedenomas and carcinomas.
Associated with nasal secretion, the ciliary activity establish the first line of defense of respiratory mucosa. The primary pathology of the ciliary structures is not well known. Correlated with testing of the mucociliary transport, only the ultrastructural study could open this very interesting field. With a short review of the present status of the ineffective mucociliary clearance mechanisms, 9 new cases of immotile cilia syndrome, with or without situs inversus, are studied. Actually, the following anomalies seems to be significant : lack of one or two dyneïn arms, confusion of the central doublets' axis, ciliary roots anomalies. Despite that more information is needed, a good knowledge of this genetic errors is henceforth necessary for clinical and therapeutical applications especially to avoid undue surgical treatments.
The pulmonary parenchyma is characterized by a marked development of blood-tissue and gaz-tissue interfaces. Thus the study of the lung is easy by scanning electron microscopy. Numerous and various technics of sample preparation are described and discussed. They permit to carry out morphological and morphometrical studies of fine structures in the adult and foetal lung surface. The systematisation of the different parts of the pulmonary air tract (bronchi, bronchioli, alveolar ducts and alveoli) is analysed. The different epithelial cells are characterized by their apical differentiations and by their often specific localization. Bronchial (mucus), bronchiolar and alveolar (surfactant) secretions are studied after different fixations. The observation of the vascular network (arteries, veins and pulmonary capillaries) is performed after vascular injection of fixator liquid. The scanning electron microscope is also used to study the alveolar macrophage (in situ or in cell culture), the visceral surface of pleura, the pre and post-natal development of the lung parenchyma. The SEM is a complementary and modern method of lung observation. It gives a dynamic view of the fine structure of lung surfaces, which appears needful for the study of the local histophysiological processes.

Micro-aggregate transfusion filters have been suggested in order to prevent pulmonary complications in cases of shock after massive transfusions and cerebral complications after ECC. It is important to have prior knowledge of performance, in particular with regard to perfusion flow rate and the efficacy of filtration. Eight filters have been thoroughly studied: Bentley PF 127, Bentley PFF 100, Biotest, Fenwal 4C2417, Fenwal 4C 2423, Intersept, X Pall Ultipor SQ 40 S, Swank IL 40 S. For each of them, study included the following: perfusion flow rate, filtration pressure (Swank technique) of whole blood distal or proximal to the filter, viscosity and various haematological parameters (CBC, free haemoglobin, red cell ATP). Finally, the deposit retained by the filter must be studied morphologically by sweep electron microscopy. It would seem that good aggregate elimination power (decrease in filtration pressure of 75 to 95 per 100) often coincides with a poor flow rate and the need to rapidly change the filter. Efficacy is virtually the same for all filters with the exception of Pall filter where it seems to be less. The Bentley PF 100 and Fenwal 4C 2423 filters combine this efficacy with an adequate flow rate, resulting in easy use in practice. By contrast, the Pall filter with a very satisfactory flow rate is less effective than the other with regard to the retention of aggregates. Other parameters are only very slightly modified by filtration. The deposit collected on the filters consists of fibrinogen, deformed red cells (echniocytes), leucocytes and platelets.
The type III pneumocyte or alveolar brush-border cell was studied in the normal adult rat by transmission electron microscopy. The observation of thick sections and the technique with alcian blue-lanthanum nitrate gave precisions on the fine structure of this cell by revealing the existence of pinocytotic micro-vesicles, of basal ergastoplasm and the proximity of unmyelinated nervous fibres. The different morphological data were confronted with the hypothesis of this cell playing an alveolar chemoreceptive part. Different appearances of the cell body of the type III pneumocyte suggest on the other hand contractile properties of a not well known determinism. Present data concerning the origin of this cell are discussed.
Special filters for the elimination of cellular aggregates were conceived especially to prevent pulmonary complications in patients shocked particularly after massive transfusions as well as to prevent cerebral complications after extra-corporeal circulation. The performances and the conditions of use of these filters (perfusion output, change of the filter) were considered. Four filters were carefully studied: the Fenwal filter, the Swank filter, the Biotest filter and the intersep Johnson and Johnson filter. The following studies were carried out for each filter: the perfusion output (of three bottles filtered one after another with or without pressure); the filtration pressure (Swank technique) of the whole blood up-and-down filter; the viscosity and various hematologic parameters (NF, hemoglobin and gobular ATP); finally, the deposit was studied morphologically with scanning electron microscopy. It appeared that a good capacity of aggregates elimination (decrease of the filtration pressure of 95%) was often linked to a feeble output and the necessity of replacing the filter. The efficacy of the Fenwal filter, the Swank filter, the Biotest filter and the intersept filter was practically the same, but the Intersept filter provided the best output with an equal filtration capacity. The other parameters remained about the same. The deposits included fibrinogen, deformed red blood corpuscules (echinocytes), leucocytes and platelets.
