Ovarian autoimmunity and corticotherapy in an in-vitro fertilization attempt.

This case report describes a successful pregnancy and delivery in a woman presenting with ovarian autoimmunity who had previously been involved in two unsuccessful in-vitro fertilization (IVF) attempts. Ten days before a third attempt, she began a regimen of 25 mg per day of prednisolone which was continued throughout the whole IVF protocol. Ovulation was induced by human menopausal gonadotrophin. After administration of 5000 IU of human chorionic gonadotrophin, 18 oocytes were collected. At 48 h after insemination with the patient's husband's spermatozoa, four 4-cell pre-embryos were transferred. A singleton pregnancy developed and led to the birth of a girl who is doing well 10 months later.

[A Buschke and Lowenstein tumor in a female patient].

Although it presents clinically as a malignant formation, the Buschke-Lowenstein tumour is known to be a histologically benign tumour caused by a papillomavirus infection from an condyloma acuminatum. The Buschke-Lowenstein tumour is generally observed in male subjects, usually on the penis, and rarely occurs in women. A female case is reported. In the literature, human papillomavirus 6 is the most common type although types 11, 16 and 18 are also reported. Surgery is the only treatment for this tumour which recurs readily.

Leukocyte compartments in the nasal secretion medium.

Nasal secretions are known to play a role in respiratory tract and host defense. Besides the mucociliary transport and biochemical properties of the mucus, we hypothesize the role of a secretory leukocyte compartment. We designed a time-series study to count leukocytes in baseline, physically- and pharmacologically-induced secretions. Twenty-three healthy volunteers and 29 patients participated in the study. In healthy subjects, secretion weights significantly increased from 24 +/- 5 mg (mean +/- SEM) at baseline to 35 +/- 6 mg and 115 +/- 12 mg, respectively, in physically and methacholine-induced secretions (p < 0.05). The leukocyte count did not change between baseline (14,445 +/- 5,010) and physically induced secretions (13,396 +/- 6,8401), but significantly increased after methacholine (28,140 +/- 11,411; p = 0.02). Leukocyte differential counts showed, moreover, an increase of lymphocytes in the methacholine-induced compartment. In patients, secretion weights significantly increased from 70 +/- 12 mg at baseline to 117 +/- 22 mg and 223 +/- 28 mg, respectively. The leukocyte count significantly increased between baseline (172,109 +/- 95,890) and physically induced secretions (410,503 +/- 318,224; p = 0.02), but decreased after methacholine (112,774 +/- 54,860). These data argue for the existence of a secretory leukocyte compartment, with a subcompartment of surface and an intraglandular subcompartment of reserve, the kinetics of which are different in patients and control subjects.

Experimental and clinical study of the effect of naftidrofuryl on the recovery from peripheral nerve lesions.

After primary repair of lesions of nerves and vessels, patients who were administered a vasoactive drug, naftidrofuryl, postoperatively, had a better functional recovery than untreated patients. Evidence for the favourable effect of the drug on nerve regeneration was first obtained from experiments in the rat. The sciatic nerve was transected and repaired with standard microsurgical techniques. Results were evaluated by electromyography and histology. Data indicated that treated animals had better motor responses and distal latency than untreated rats. Fibre counts showed a greater number of nerve fibres in the distal stump of treated animals. The clinical study was undertaken in cases of carpal tunnel syndrome with muscle atrophy. Assessment was performed by electrophysiology: motor and sensory conduction was studied. Data showed that all treated patients recovered totally or partially from the thenar atrophy they presented before operation, whereas thenar atrophy persisted in 37% of the patients receiving placebo. In conclusion, naftidrofuryl has an experimentally and clinically favourable effect on nerve regeneration and on muscle trophicity, but the mechanism of action of this drug is not known.

Giant-cell arteritis of the female genital tract.

A 56-year-old woman with long-lasting fever of unknown origin was diagnosed as having a giant-cell arteritis of the genital tract with no evidence of temporal arteritis. Diagnosis relied on pathological examination, which showed a segmental panarteritis of ovaries, myometrium, endometrium and uterus cervix. Corticosteroid therapy led to clinical cure within a few weeks. Twenty-five cases of giant-cell arteritis of the female genital tract have been published of which only four were associated with temporal arteritis. We recommend that such a diagnosis should be considered in women presenting with long-lasting fever of unknown origin, even in the absence of temporal arteritis and the clinical evidence of genital abnormalities.

[Qualitative and quantitative study of bone colonisation in a wired surface effect. An experimental study].

PURPOSE: The aim of this study was to evaluate the bony anchorage of a new implant (orderly wired surface effect with alloy Ti Al Va and ordered pores of 488 mu). Bony integration was analyzed on qualitative and quantitative aspects. MATERIALS AND METHODS: The implants were inserted into both femurs of six sheep in three localisations, in all 36 implants, during a period of 6 to 33 weeks. The implants were opposed on cortical bone or impacted into trabecular cancelous bone after corticotomy. Evaluation of rehabitation was realized using conventional radiography, MRI, isotopic study and particularly by computer assisted histomorphometry (BIOCOM program). This new technic evaluated the trabecular bone volume (TBV) of the implant, the trabecular bone density, and the mean trabecular width in real value with precision and objectivity. RESULTS: This study shows that there was good osteointegration of the implant, that this osteointegration differed with the localization of implantation. Quantitatively, the BIOCOM program showed that for metaphyseal implants in cancelous bone, the TBV of the implant was greater than that of the environment of the implant. DISCUSSION: It is questionable to speak of osteoinduction in an osteogenic situation but it is certain that in many implantation conditions, the TBV is more important in the implant than that of the environment surrounding the implant. This shows the excellent osteointegration of the implant. CONCLUSION: This implant brings a new concept of rehabitation space with its particular geometry of structure. This quality is confirmed by clinical experience with uncemented knee prosthesis.

Tumour necrosis factor alpha and interleukin 2 in normal and infected human seminal fluid.

The presence of cytokines such as the tumour necrosis factor alpha (TNF alpha) and interleukin 2 (IL2) in human spermatozoa is still to be defined. The aim of this study was to measure the concentration of both soluble factors in seminal fluid. Data from normal semen samples (n = 24) confirmed the presence of IL2 (258 +/- 84 fmol/ml corresponding to 953 +/- 369 fmol/total volume of ejaculate) and TNF alpha (62.2 +/- 16.4 fmol/ml corresponding to 231.3 +/- 86 fmol/total volume of ejaculate). A significant positive correlation (r = 0.59; P < 0.01) was observed between the TNF alpha and the IL2 concentrations. The concentrations of these cytokines were not related to sperm parameters. In contrast, IL2 concentrations (196.9 +/- 60.4 fmol/ml; 686.2 +/- 236.7 fmol/total volume of ejaculate) evaluated in 16 seminal fluids with identified bacterial agents were lower than in the control group, whereas TNF alpha concentrations (68.6 +/- 12.3 fmol/ml; 241.3 +/- 78.9 fmol/total volume of ejaculate) were not significantly different from the controls. Further studies are needed to determine the potential role of these cytokines in the physiology of semen and their usefulness as indicators of reproductive pathology.

[Interleukin-2 and human seminal fluid].

Involvement of the cytokines in male infertility were considered from in vitro studies, however the nature and the role of the mediator susceptible of arising in vivo is not completely elucidated. The purpose of this study is to measure the concentrations of interleukin-2 (IL-2) in normal and pathologic seminal fluid and to analyse their relationship with semen parameters. Our results demonstrate the presence of IL-2 in seminal fluid. Comparatively to normal seminal fluid, significant lower IL-2 concentrations were observed in infected semen samples or with sperm antibodies. No correlation was found between the cytokines values and the sperm parameters.

[Cytometry of nasal secretory eosinophilia].

Nasal secretory eosinophilia was evaluated in inflammatory chronic rhinitis by a cytometric method. Thirteen control subjects and 97 patients (nasal polyps = 44; chronic sinusitis = 26; chronic rhinitis = 27) took part in this study. Secretory eosinophilia was low in the control group (0.3 +/- 0.2 to 1.3 + 0.8%), compared to: 13.8 +/- 8.2 to 15 +/- 7.6% in idiopathic chronic rhinitis, 16.3 +/- 7.3 to 22.7 +/- 10.2% in perennial allergic rhinitis, 18.5 +/- 6 to 25.3 +/- 6.1% in nasal polyps, 23.1 +/- 1.1 to 25.9 +/- 8.6% in nasal polyps with asthma, 27.3 +/- 7.1 to 30.3 +/- 7.8% in aspirin triad. The NARES syndrome was clearly individualized with a secretory eosinophilia between 48 +/- 16.9 and 58.8 +/- 10.7%. Nasal cytometry is a new method that confirms the role of secretory eosinophilia as an interesting marker in the study of inflammatory chronic rhinitis. Nevertheless reference values would be established only after a better knowledge of cell kinetics in nasal secretion under normal and pathological conditions.

Evaluation of fibrosis in the disse space in noncirrhotic alcoholic liver disease.

In 40 alcoholic noncirrhotic patients, we performed a liver biopsy and determined the wedged hepatic vein pressure, the free hepatic vein pressure, and the intrahepatic vein pressure. In 27 of them, the serum concentration of the N-terminal peptide of type III procollagen (PIIIP) and of the laminin P1 fragment was measured. All the liver biopsies were studied by light and transmission electronic microscopy. A score of collagenization of the Disse space (six classes) was performed using transmission electronic microscopy. 37 of the 40 patients had pathological collagenization of the Disse space which was correlated with intrahepatic pressure (p less than 0.01). The lamin P1 blood level in patients (1.38 +/- 0.51 U/ml) was increased, compared to the values of our controls (0.99 +/- 0.10 U/ml, p less than 0.01) and was correlated with the wedge hepatic vein pressure (p less than 0.01). The PIIIP blood level was not significantly increased except when Mallory bodies were found in hepatocytes (p less than 0.05). The laminin P1 blood level seemed to be a good biological marker for detection of liver fibrosis in long-term alcoholic intake.

[Scanning electron microscopic study of Campylobacter pylori and associated gastroduodenal lesions].

The detection of Campylobacter pylori was carried out with scanning electron microscopy (SEM) on 4,590 biopsies taken from 1,714 adult patients (1,348 duodenal biopsies, 1,690 antral biopsies and 1,552 fundic biopsies). The mucus was removed and the fragments of mucosa were then fixed with glutaraldehyde, dehydrated, critical point-dried with CO2 or hexamethyldisilazane, mounted, coated with gold, and observed with SEM. Under SEM, C. pylori was characterized by its spiral-shape (long form 2.5 to 6 microns) or by its curved-shape (short form 1.5 microns) and by its relatively constant diameter (0.5 micron). C. pylori was present in 671 patients (40 p. 100) and was found mainly in the fundus and antrum, near the neck of the gastric glands and at the level of the intracellular spaces. It was never present on the surface of enterocytes but was found in the 26 duodenal biopsies in the gastric metaplasia areas and on the surface of adjacent muciparous cells. The main epithelial lesions associated with C. pylori were of an erosive or desquamative type. The high definition observed using SEM allowed C. pylori to be distinguished from other microorganisms observed on the surface of the gastroduodenal mucosa: bacilli and cocci contaminants, Candida albicans, Giardia lamblia, spirochetes and other Campylobacters. SEM is a specific and rapid technique (less than 2 hours) which determines the morphology, localisation of bacteria as well as the associated lesions of the mucous cells.

Cardiorespiratory function and pathological findings in heart-lung block reimplanted after hypothermic preservation.

A good result from the heart-lung transplantation depends on the quality of the preservation of cardiopulmonary transplants. To determine the functional and pathological status of the heart-lung block after preservation for several hours, we performed 10 heterologous heart-lung transplantations in Beagle dogs (weight 13.5 kg) under extracorporeal circulation. Weight and length compatibility between donor and receiver was ensured. Measurements of hemodynamics, lung mechanics and blood gases were performed in the donor and in the receiver before the transplantation, and in the receiver after heart-lung reimplantation. Histological studies were carried out by biopsy on the heart and on the lung of the donor before removal, at the beginning of the preservation at low temperature, after 3 h of ischemia in cold, and every hour after recirculation in the heart-lung block. Myocardial preservation was conducted with cold cardioplegia at 4 degrees C (Ringer lactate solution with high potassium). Lung preservation was achieved by injecting a Euro-Collins solution at 4 degrees C, with addition of dog plasma, into the pulmonary artery; during the whole ischemic phase, the lung parenchyma was maintained at 0 degrees C, and inflated at a 10 cm H2O pressure. After transplantation, we observed that cardiac output was low in all cases, with normal or subnormal pulmonary arterial pressure. Dynamic lung compliance was very low immediately after transplantation, and increased when restarting the circulation, but deteriorated again after several hours. At the same time alveolo-arterial O2 pressure difference and arterio-alveolar CO2 pressure difference progressively increased, due to the extensive gas exchange impairment.(ABSTRACT TRUNCATED AT 250 WORDS)

Intrapleural injection of elastase: a model for inducing subpleural emphysema?

Subpleural emphysema induction was attempted by injecting elastase intrapleurally into rats and cats. Microscopic evaluation of emphysema (alveolar dilation and septal rupture) was compared with a quantification of alveolar dilation (Fawell-Newman method). The emphysema found in animals treated with an intrapleural injection of porcine pancreatic elastase did not appear to be specifically located in a subpleural zone, and was identical to the focal emphysema induced by an enzyme instillation into the airways. It should be noted that a placebo instillation into the feline airways sometimes induced alveolar dilation without septal disruption presumably by causing bacterial contamination. Cats were seen to be very sensitive to pleural stress since this enzyme injection produced an alveolar dilation (increased mean intercept) as significant as that in cats treated by an intrabronchial instillation of elastase (P less than 0.001).
