Polymeric and monomeric IgA response in serum and milk after parenteral cholera and oral typhoid vaccination.

The effect of vaccinating lactating Pakistani mothers with a combination of live oral typhoid vaccine and parenteral inactivated cholera vaccine on specific milk and serum IgA antibodies in both monomeric (m) and polymeric (p) forms was analysed. IgA antibody titres peaked for both antigenic specificities 2 weeks after the first dose of vaccine. 82 +/- 7% of anti-Vibrio cholerae and 72 +/- 17% of anti-Salmonella typhi IgA were in the polymeric form. These serum pIgA antibodies were mainly dimeric IgA, not complexed with the secretory component. They disappeared more rapidly from serum than mIgA antibodies. Anti-V. cholerae IgA responses were parallel in serum and milk samples, whereas anti-S. typhi responses were dissociated. In milk, IgA antibodies were secretory IgA for both antigenic specificities, being probably of local origin in the mammary gland. Our results indicate that both oral and parenteral vaccinations can induce pIgA antibodies in serum and secretions, confirming that the presence of pIgA in serum does not necessarily reflect an immune stimulation only at the mucosal level.

Avidity of IgA antibody to Escherichia coli polysaccharide and diphtheria toxin in breast milk from Swedish and Pakistani mothers.

The avidity of breast milk IgA antibody was studied with the aid of thiocyanate elution of antibody from solid-phase bound E.coli polysaccharides and diphtheria toxoid. The relative avidity index for each sample was determined by the molarity of thiocyanate required to elute 50% of the bound IgA antibody under conditions of antigen excess. Milk samples collected from Pakistani mothers during early lactation (2-4 weeks after delivery; n = 12) had a significantly lower median relative avidity index of IgA antibody to E.coli antigens than did early lactation samples from Swedish mothers (n = 11; avidity indices 1.78 M and 2.65 M; P less than 0.02). Samples collected from Pakistani mothers in mid-lactation showed a significant rise in the relative avidity index to a median of 2.50 M (P less than 0.01), with a subsequent fall in late lactation (28-36 weeks after delivery) to 1.75 M (P less than 0.01). Milk samples from Pakistani mothers in mid-lactation (n = 12) also had a lower median relative avidity index of IgA antibody to diphtheria toxoid than did samples from Swedish mothers (n = 14; avidity indices 2.35 M and 4.30 M; P less than 0.002). The lower avidity of breast milk IgA in Pakistani mothers in comparison with Swedish mothers may arise from differences in antigen exposure or nutritional status or could possibly be genetically determined.

The organization of work in Sweden.

The well-being of workers in Sweden is extensively protected through legislation and through the activities of organized labor which represents the great majority of Swedish workers. Despite the comparatively advanced state of Swedish social legislation and the large percentage of women represented by trade unions, women remain at a disadvantage both in terms of job advancement and the quality of working life. The author of this article, a trade union official who works in the area of women's equality, provides case examples and personal insights as to the current and future needs of women workers, particularly with regard to the organization of work.

The tissue-integrated prosthesis in the treatment of edentulous patients. A follow-up study.

This paper reports the results of clinical series of osseointegrated implants (BiotesTM) at the Karolinska Institute, Stockholm. From 1980 to 1985, 350 implants were installed in 68 upper jaws and 440 implants in 83 lower jaws. After observation periods of 1 to 6 years, 307 (88%) maxillary implants and 434 (99%) mandibular implants were fully functional. Of 128 bridges and 23 overdentures 124 (97%) and 21 (91%) respectively were in function at the end of the observation time. The most frequent tissue complications were fistulae (8 cases) and mucosal hyperplasia adjacent to abutments (11 cases). The most frequent prosthetic complications were phonetic problems in the maxilla only (10 cases), acrylic fractures in bridges (3 cases) and fractures or distortion of the metallic framework (2 cases). The results indicated that treatment with osseointegrated implants (BiotesTM) is a safe method with predictable long-term prognosis for rehabilitation of the edentulous patient.

HLA-DR-DQ haplotypes defined by restriction fragment analysis. Correlation to serology.

Through the analysis of RFLP (restriction fragment length polymorphism) of the HLA-DR beta, -DQ alpha, and -DQ beta genes from 70 serologically well-characterized individuals, we have established unique HLA-DR-DQ RFLP haplotypes correlating to all of the DR1-w14 specificities. The RFLP of DR beta, DQ alpha, and DQ beta genes is very high using the restriction enzyme TaqI and 21 DR-DQ RFLP haplotypes were defined with this restriction enzyme. Our analysis confirms the strong linkage disequilibrium between alleles in the DR and DQ loci. DR beta RFLP indicates a common ancestor for the DR alleles within either of the supertypic DRw52 and DRw53 specificities. The DQ beta gene shows a high degree of RFLP, and the RFLP alleles partly reflect the serologic DQw1-w3 specificities. The results presented here also demonstrate the heterogeneity of DRw6 (DRw13 and DRw14) associated haplotypes, and the DRw13 related Dw18 and Dw19 specificities were found to have distinct DR-DQ haplotypes. The DQw1 positive haplotypes DR1, 2, w10, w13, and w14 are related with regard to DQ alpha and DQ beta RFLPs and the DRw52 positive haplotypes DR3, w11, and w12, as well as the DRw53 positive haplotypes DR4, 7, and w9, are related with regard to DR beta and DQ alpha RFLPs. These findings indicate that polymorphic sequences around the DQ alpha gene are associated with DR beta and DQ beta polymorphism, which suggests a location of the DQ alpha gene between DR beta and DQ beta.

Only one DQ-beta restriction fragment pattern of each DR specificity is associated with insulin-dependent diabetes.

Insulin-dependent diabetes is generally associated with the serologic HLA-DR specificities 3 and 4, in particular with DR-3,4 heterozygosity. The disease is negatively associated with DR-2. To investigate these associations further at the genomic level, DNA from 13 families with a proband having insulin-dependent diabetes, from 11 other individuals with the same disease, and from HLA-DR-matched control individuals was subjected to restriction fragment analysis. Three different enzymes (Bam HI, Eco RI, and Pvu II) and cDNA clones for three HLA-D region class II antigen alpha- and beta-chains (DR-beta, DQ-beta, and DQ-alpha) were used. In six families, a total of 11 siblings HLA-DR-identical to the proband were examined. There was no discrepancy between the hybridization patterns of the proband and those of the DR-identical siblings. Two different DQ-B fragment patterns were detected with each one of the serologic specificities DR-2 and DR-4. In both cases, only one of the patterns correlated significantly with diabetes. Thus, DQ-beta genomic hybridization may be used in conjunction with HLA-DR typing to identify individuals with higher relative risk to acquire insulin-dependent diabetes. These results may suggest that insulin-dependent diabetes is associated with the DQ rather than with the DR locus.
