[Psychopathological findings and social psychiatry problems in patients with transplanted kidneys].

Chronic haemodialysis and kidney transplantation reveal a great number of psychological and psychiatrical problems and tasks. As there are special psychological problems and many psychoreactive symptoms and the fact that haemodialysis and kidney transplantation are strain situation in themselves, one should examine the question of capability, and often a psychiatric or psychotherapeutic treatment is necessary. Authors examined twelve patients who had undergone kidney transplantation, regarding their psychopathological and social-psychiatrical status. The great improvement in the quality of life in comparison to the time of dialysis treatment is often reduced, however, because of psychological and severe social problems as well as deficient or missing steps of rehabilitation.

[Endoscopic diagnosis of acute perianal hemorrhage].

Emergency upper endoscopy was performed in 22 consecutive patients with acute peranal hemorrhage. In 8 patients with negative upper endoscopy emergency coloscopy was subsequently performed. In 6 cases the bleeding source was located in the colon. Overall, the correct diagnosis was made by endoscopy in 21 of the 22 patients. Thus, emergency coloscopy is a useful diagnostic procedure in cases with acute peranal hemorrhage and negative upper endoscopy.

[Drug abuse and suicidal tendencies].

66 juvenile drug-consumers--development-crises (32%), neurotic developments (30%), abnormal juvenile personalities (38%) - with a predominantly polytoxicomanic drug-anamnesis of several years are examined as to their suicidal development. The quantitative and qualitative progression of drug-abuse, the duration of drug-anamnesis, frequent broken-home-situations, grave educational deficiencies together with lacking possibilities of identification, a distinct social decline and chronic conflict-situations indicate the serious degree of the suicidal development. Different suicidal syndromes are worked out which in the form of death-phantasies or suicidal thoughts and horror trips directly derive from drug-abuse or are accompanied and reinforced by a continuous and frequently increasing drug-consumption, and which extend from an increase in depressive irritations, further psychological disturbances and personality changes, doubts about one's personal value and a sense of guilt to the "sickness of death".

[The psychopathology of early and chronic psychotic symptoms in young drug-users].

Among the psychotic symptoms in juvenile drug-consumers one can find autonomous, i.e. drug-independent developments, whose connection with the drug-abuse is to be assessed in differing ways. A beginning psychosis can be modified in its actual symptoms by drug-consumption. On the other hand one must consider the manifestation of a latent psychosis or purely symptomatic psychosis, which, in its symptoms, can hardly be distinguished from schizophrenia. Finally drug-induced personality-changes can develop together with secondary psychotic symptoms. Psychotic symptoms are determined and influenced in a varying degree by drugs. Both after short drug-consumption and after a longer drug-anamnesis with polytoxicomanic symptoms psychotic syndromes can be discovered. Even the initial psychotic symptoms can hint at an adverse development and a bad prognosis. Sometimes the drug-experiences conceal the autonomous development of the psychosis, which, as a rule, shows predominantly schizophrenic symptoms. In addition to a quick change of the actual symptoms, acute states of confusion and depressive-suicidal syndromes, flash-back and horror-trip phenomena, closely connected with the psychotic experience, and a schizophrenic colouring of affective psychoses can be found as frequently drug-induced modifications of the psychotic symptoms. Furthermore one finds an increase of symptoms and of the psychotic episodes in the case of psychoses of the schizophrenic variety which have already begun. Grave personality changes with psychotic symptoms after chronic drug-abuse can cause differential-diagnostic difficulties.

[The phenomenon of possession. Conception and experiences of possession in youth].

In the last few years, a trend to the multiplication of experiences of possession has been observed in young people. On the basis of four typical examples, the author examines this phenomenon in the light of the psychiatric, psychoanalytic and theological understanding of possession. It involves mostly young people, who do not have hysterical fits or psychotic episodes during spiritualist practices but who specially tend to take a strong interest in occultism, who very often consume drugs and have contacts with groups in which the interest for demonology plays an important part.

[Delirium and ecstasy. The phenomenon of toxic ecstasy in young men].

In the whole history of mankind, hallucinogenic drugs have played a big part in religious and cult practices and have transmitted special religious experiences during a drug-induced ecstasy. The present extension of drug consumption with a view to solving personal conflicts and transitory experience of happiness, as well as the unsuccessful 'quest for God' of many young drug consumers should be understood in the context of the specific drug effects, of the technological developments of our times, as well as of the change and breakdown of religious and cult traditions. This desperate quest for happiness and religious experience calls for a critical reflection about the present sociocultural developments and raises the question of the necessity of an authentic religious life.

[Identy crisis and drug abuse in young people].

Nowadays young people during the phase of their puberty and adolescence often show symptoms of a grave crisis of identity which particularly idspose them to drug-abuse. On the one hand this search for one's identity is characteristic of the formative years of a juvenile; on the other hand it is a general problem of the generations which must be seen in the full context of the social, technological, industrial and political development of our time. Lacking possibilities of identification in the context of a general relativism of traditional norms and values, radical changes in the structures of the family and of our society, accompained by over-exaggerated demands on one's personal performance and prosperity, and significant failures of communication as well as the unreflected liberalization of a "permissive society" make young people look for new possibilities of identification which they try to find in sub-cultural groups and fascinating experiences which drugs. Many juvenile drugconsumers show increasing crises in their personalites and farreaching psychological distrubances often followed by a serious development. All these phenomena illustrate the frustrated search for genuine personal identity in an illusionary world of drugs. Only the solidaric and constructive cooperation of the generations can overcome the manifold problems and conflicts of young people which are concentrated in their identitycrises, and only this cooperation can open up new perspectives for a future which is humane and worth living, a future which is the real aim of the subcultural movements and psychodelic experiences.

[Depression and suicide in the elderly].

The suicidal behavior in 155 patients older than 60 years is analyzed. Patients were treated for endogenous, neurotic, or reactive depression. Differences between diagnostic categories and sex are discussed in detail. Motivation and recent causes of suicidal behavior are analyzed. Psychopathological symptoms correlating with suicidal behavior in endogenous depression in the elderly are depicted. The social environment of and the therapeutic approach to suicidal elderly patients are discussed. The problem of rational suicide and the demand for voluntary death are mentioned.

[Grief and depression in the aged].

Bereavement after recent or earlier losses of loved ones is analyzed in 155 patients older than 60 years. Patients were treated by psychiatrists for endogenous, neurotic, or reactive depression. Bereavement was characterized by depressive syndromes, depressive and anxiety developments, chronification of somatic complaints, induction of endogenous depression, lack of grief or late onset of grief reaction, substance abuse and suicide. Women are more prone to bereavement. Special stressors and characteristic features of bereavement in old age are discussed. The pivotal meaning of interpersonal dependence on lost ones is discussed. Difficulties in the management of bereavement in old people are mentioned.

[Abuse and dependence on alcohol and drugs in advanced age].

The report is based on records of 117 in-patients over 60 years of age treated because of alcohol or drug dependence. Remarkably, the abuse of alcohol and particularly of drugs had frequently started at higher age. Situations of loss, loneliness, and isolation as well as mental and physical disease played a precipitation part. In the majority of patients the course of the dependence was severe. Prophylactic and diagnostic problems will be discussed and the necessity of improved therapeutic means will be pointed out. The avoidance of iatrogenically induced drug dependence appears to be particularly important.
