[Histomorphological alterations of lung after strangulation. A comparative experimental study (author's transl)].

It was investigated by animal experiments (50 rats, 15 rabbits) if histomorphological vital reactions can be found in the lung after strangulation and how far these findings differ from other violent causes of death with comparatively short survival period. Besides strangulation by ligature the animals were killed by overdose of ether, chloroform, pentobarbitone, and ketaminehydrochloride; gasing with propane and nitrogen; suffocation in a small closed box; thoracic compression; injection of KCN i.p.; decapitation, blunt force; electric current; cardiac injection of KCL. In comparison with the controls the striking findings after strangulation consisted of a qualitatively and quantitatively considerable hemorrhagic syndrome and an edema of all compartments of the lung which was frequently hemorrhagic, too. The authors award specific significance to the alveolar-septal edema which was found in semithin sections (and electron-microscopically)--which never occurred in the controls.

[Death by obstructive asphyxia: the pathophysiology of respiration and hemodynamics (author's transl)].

The pathophysiology of respiration and hemodynamics during the agonal period of death by obstructive asphyxia was investigated. Experiments were carried out on ten dogs that were strangulated by ligature (seven cases) or asphyxiated by occlusion of the intubation tube (three cases). The following parameters were registered: Blood pressure in the arteria femoralis and arteria pulmonalis, electrocardiogram, endotracheal pressure, respiratory frequency, electroencephalogram. These were the most important findings: (a) An excessive and distinctly biphasic hypertonic reaction takes place after strangulation by ligature whereas occlusion of the intubation tube results in a slight increase of arterial blood pressure. (b) After occlusion of the tube more than after strangulation the endotracheal pressure becomes deeply negative during the inspiration trials. (c) After cessation of respiration a distinct pulmonary arterial hypertension develops in both forms of obstructive asphyxia. (d) The paralysis of the respiratory center takes place after about 4 min at the same time when the circulatory breakdown develops. The final cardiac failure occurs after about 9-10 min. The dysregulation of respiration and hemodynamics in the agony of obstructive asphyxia can be differentiated from other shock models. This results in special morphological alterations of the lung tissue which have been described elsewhere.

[Malignant hyperthermia. Present implications in forensic medicine (author's transl)].

Literature about the medicolegal implications of MH incidents has been examined. The authors themselves have observed 11 typical cases of malignant hyperthermia, 4 of which were investigated by the public prosecutor. From the point of view "ex ante" it should be expected that anaesthetists known about the diagnosis, therapy, and preanaesthetic procedures concerning MH.

[Multifactorial disturbances and compensatory phenomena in the materno-fetal unit (author's transl)].

Clinical and laboratory data as well as morphological placenta findings of approximately 7 000 pregnancies recorded in a program of the German Research Association (DFG): "Course of pregnancy and development of the newborn" are examined for statistically significant correlations. It could be shown that there is a correlation between haemoglobin-level during pregnancy and maturation of the placenta villi. Low maternal haemoglobin frequently coexists with complete villous maturation. The same interaction is true for placental size or weight and maternal haemoglobin. Small placentas show a--sometimes precocious--complete villous maturation, whereas placentas with greater volume still possess immature villi at term. This finding is different in combination with placental infarcts, which lead to a complete maturation of persistent villi, especially, if the maternal haemoglobin shows low values at the same time. The combination of the factors: low maternal haemoglobin and small placental insertion area or insufficient placental maturation and low maternal haemoglobin lead with statistical significance to a high frequency of babies with low birth weight in relation to the duration of pregnancy. This is not the case, if one of the two factors involved shows favorable characteristics (morphological findings in the placenta with good functional aspects--maturity, insertion area--and low haemoglobin or high haemoglobin in combination with favorable placenta findings). Summing up the results demonstrate that interactions between single factors of the materno-fetal transfer-system constitute a basic developmental principle.

[Acute death from scalding: vital reactions (author's transl)].

After a boiler explosion on a ship 12 men with cutaneous scalding of 90-100% of body surface were found dead. 15 other fatalities occured after different intervals. This investigation deals with the histomorphologic changes in those 12 cases of instantaneous death. The most important findings consisted of: 1. Thermal injuries of the skin with multiple subcutaneous and corial bubbles, intravascular heat coagulation, and edema in the transitional zone to the undamaged submucosa. 2. Coagulation necrosis of the tracheal and bronchial wall extending to alveolar ducts in central parts of the lung. Marked congestion and edema in the deeper layers of the mucosa. Hemorrhagic alveolar and interstitial edema of the lung. Striking congestion of the pleura parietalis saving up the recessus diaphragmaticus. 3. Acute nephron-nephrosis and periglomerular microangiopathy. Interstitial edema of the myocardium, degeneration of myofibrils. Acute vacuolar degeneration and swelling of hepatocytes. Phagocytosis of detritus by the RHS. 4. Swelling of endothelium and edema of the vascular walls. Intravascular fragments of erythrocytes, platelet aggregation and fibrin thrombi. The fatal cours in these cases is considered to be due to a fulminant shock syndrome.

Population genetic data on properdin factor B (Bf) in northern Germany.

Properdin factor B phenotypes were determined in 1,112 unrelated individuals and in 151 mother/child combinations from Northern Germany. Gene frequencies were : F = 0.1960, S= 0.7905, F1 = 0.0072, S1 = 0.0063. The data of the mother/child combinations are in full accordance with the postulated gene model.
