[Sonographic differentiation of pulmonary opacities (author's transl)].

Sonography offers a supplementary diagnostic method in cases where X-ray examination has established the presence of opacities in the lungs. Simple criteria generally allow differentiation between solid and liquid structures and thus to distinguish between effusions, haematoma, empyema and adhesions and tumors. - Sonographically directed needle biopsies following the sonographic localization of encysted effusions or a tumor ensure the correct site of puncture and provide the means to establish the diagnosis histologically, cytologically and bacteriologically.

[Ultrasound diagnosis of cholelithiasis: comparison with radiological studies (author's transl)].

Results of ultrasonic anc cholecystographic (oral or intravenous) gall-bladder studies were compared retrospectively. 302 radiological studies of the gall-bladder, 156 oral and 146 intravenous cholecystograms, had been performed on 223 patients tested by ultrasound within six months. The gall-bladder was visualised by ultrasound in 217 (97.3%), in the intravenous cholecystogram in 92 of 146 (63%) and the oral cholecystogram in 82 of 156 (53%). The diagnosis of cholelithiasis by ultrasound was made in 97 of 223 cases, confirmed in 96 by further studies or operation. There was only one false-positive result. 143 radiological studies in these 97 patients gave 87 negative cholecystograms, gall-stones being directly demonstrated radiologically in only 46. There was one false-positive intravenous cholecystogram. In a further 97 patients ultrasound studies failed to demonstrate any gall-bladder disease, but in three of them radiology demonstrated stone (2.9% false-negative results). Amond 49 normal intravenous cholecysto-cholangiograms 47 were confirmed; once the diagnosis was made by ultrasound and at surgery (2% false-negative results). Oral cholecystography was the least reliable study. In every fourth negative test ultrasound and intravenous contrast-medium injection demonstrated a normal gall-bladder (28% false-positive).

Experimental study on the free transplantation of mucosa and lyophilised dura to the oral cavity.

The covering of epithelial defects in the oral cavity presents special problems in preprosthetic surgery, since special requisites for function and quality of mucosal grafts must be satisfied. In animal experiments mucosal defects in the oral cavity of 30 rabbits were repaired with free transplants of autologous mucosa and heterologous lyophilised dura. A comparative clinical and histological assessment of the healing processes showed that there was no essential difference between both methods. Considering the necessary second intervention, the infliction of a second wound, as well as the limited source of mucosa, our results show that the use of lyophilised dura in preprosthetic surgery is preferable.

[Ultrasound in the diagnosis of cystic lesions of the liver (author's transl)].

Possibilities and limitations of the use of ultrasound in diagnozing cystic liver diseases are discussed with reference to our own investigations. Solitary or multiocular liver cysts can be detected in the right proportion and in the correct anatomical position from about 1.5 cm in size; they are often coincidental findings after ultrasonography. The ultrasonic method is a simple investigation technique without any complications or contraindication and it now has a firm place as a screening investigation programme within diagnostic procedures especially before an intended liver needle biopsy or puncture during laparoscopy. By carrying out ultrasonic investigations on families of patients with cysts of the liver it has been possible to make an early diagnosis of hereditary cystic disease.

[The use of ultrasound in the diagnosis of gastrointestinal tumors (author's transl)].

In 19 patients with gastrointestinal tumors the diagnosis was made by ultrasound. 7 patients were examined for further investigation of an abdominal mass; 12 patients, presented with symptoms suggesting malignants, e. g. weight loss, vomiting, fever, abdominal pain, and/or anaemia from bloodloss were first investigated by ultrasound. In 5 patients with colon tumors, diagnosed by ultrasound, a resection and anastomosis was possible. 13 cases were found to be inoperable during laparotomy, one patient had an ileocolitis Crohn with stenosis. Ultrasonic examination with real-time scanning technique proved to be a valuable method as a screening procedure for patients with suspected gastrointestinal tumors. During the same procedure local (mesenteric) or liver metastases can be detected. A normal abdominal ultrasonography does not exclude malignant intestinal lesions in any location during the early stage and in the region of cardia and rectum even in the more advanced stage.

[Coronary artery reserve in patients with hyperlipoproteinemias (author's transl)].

The coronary reserve was measured in 119 patients with different types of primary hyperlipoproteinemia. Cardiovascular diseased with clinical manifestation were excluded. 90 patients of same age with normal serum lipids served as controls. The groups did not differ in other risk factors as blood pressure or overweight (the latter excluded in hyperlipoproteinemia type IV). The controls showed decreased coronary reserve in 8%, type IIa patients in 36%, IIb patients in 18% and type IV patients in 23%. The frequency of restriction in coronary flow increased with age: in hyperlipoproteinemic patients of 50 years and more it was found in nearly 40%. Further interesting results were the significantly higher systolic and diastolic blood pressures reached during exercise in our hyperlipoproteinemic patients.

[Ultrasonic methods in the differential diagnosis of the "postcholecystectomy syndrome" (author's transl)].

The "postcholecystectomy syndrome" as a collective term for all upper abdominal complaints following cholecystectomy is a common clinical picture. For the purpose of clarification the easily practicable sonography offers itself in addition to conventional cholangiography and to endoscopic retrograde cholangiopancreaticography (ERCP). By means of echography, pathological changes in the upper abdominal organs can be detected or excluded. Chronologically, echography should be carried out at the beginning of diagnosis as an informative, harmless and rapid method. The most invasive investigations can then be omitted or performed specifically as required.

[Behaviour of plasma renin activity during long-term treatment with propranolol (author's transl)].

17 patients (40 +/- 9 years) with essential hypertension were included in the study. A monotherapy of 120, 160 or 240 mg propranolol per day was administered orally according to the antihypertensive effect. Four weeks after treatment, blood pressure and heart rate showed a statistically significant decrease and remained unchanged over a period of six months. Plasma renin activity decreased significantly from 5.7 +/- 6.3 ng/ml/h at the beginning of the study to 1.8 +/- 1.3 ng/ml/h after 4 weeks. 5 months later however plasma renin activity increased again to 5.0 +/- 0.9 ng/ml/h. The difference was statistically significant. From 17 patients 7 (41%) had lower levels of plasma renin activity after 6 months treatment when compared with pretreatment values. In only 3 patients (18%) was plasma renin activity lower after 6 months than after four weeks. We conclude that the increase in plasma renin activity is a reactive mechanism to the reduced blood pressure under long-term conditions. The decrease of plasma renin activity in short-term treatment of essential hypertension is not a mechanism responsible for the antihypertensive effect of propranolol during long-term treatment.

[Raynaud's phenomenon in the carpal tunnel syndrome (author's transl)].

The case is reported of a female patient with bilateral carpal tunnel syndrome, the diagnosis confirmed on surgical exploration, associated with Raynaud's phenomenon. A review is given of the relatively sparse literature on the subject, followed by a discussion of the pathogenetic relationship and therapy.

[Treatment of essential hypertension with propranolol (author's transl)].

Effect of long-term administration of propranolol was studied in 90 unselected patients with essential hypertension. At a dose of 180 and 360 mg daily it dropped the blood pressure to at least 155/95 mm Hg in 31 patients (34.4%). Those who responded to propranolol had lower systolic and diastolic blood pre-treatment pressures than those who failed, were younger by an average of eight years, and in no case had decreased plasma-renin activity and demonstrated greater plasma-renin activity after furosemide. After a titration phase with increasing doses of propranolol those who responded were treated in a crossed double-blind test with propranolol or placebo. There was a significant difference in systolic blood pressure (standing and lying) and diastolic pressure (standing) between the two. There was also a highly significant difference in heart rate. During chronic beta blockade the basal and stimulated plasma-renin activity fell. But percentage stimulation of renin secretion was fully maintained. However, aldosterone response was decreased. Serum potassium rose slightly but significantly, cAMP 24-hour urine excretion fell significantly. Electrolyte excretion did not differ between the two treatment periods.

[Possibilities of differenciating mycoplasma serologically and by protein analysis (author's transl)].

The problem of serological differentiation of the various mycoplasma strains and their demarcation from the L-phase variants of bacteria are based on the fact that it is difficult to obtain highly purified antisera which are only directed towards the cell membrane. For this reason an attempt was made by means of diselectrophoresis to determine the plasma and membrane proteins of various mycoplasma strains and L-phase variants and to evaluate them from differential diagnostic point of view. The results suggest that an unequivocal and rational differentiation is possible by this method of examination.
