[Appendicitis in pregnancy. Complications and treatment].

In order to establish guidelines for the management of a pregnancy that is complicated by acute appendicitis we carried out a retrospective review of 16 patients that had their appendix removed at the Department of Obstetrics and Gynaecology, Rigshospitalet, University of Copenhagen in the period 1974-1988. The diagnostic accuracy was 12/16 (75%). The signs and symptoms were classic. Uterine contractions may occur. There was one foetal death in a case complicated by an intraperitoneal abscess, and two patients delivered prematurely. Three patients had infectious complications, none of these patients were given prophylactic antibiotics. In all uncomplicated cases the pregnancy proceeded to term and the deliveries were normal. The pregnancy ought not to divert the surgeon's attention from performing prompt appendectomy once the diagnosis is suspected. Prophylactic antibiotics and tocolytic drugs should be used in all cases. Simultaneous caesarean section should be performed only on obstetric indications.

Distinct RB1 gene mutations with low penetrance in hereditary retinoblastoma.

The interfamilial diversity in penetrance and expressivity of hereditary retinoblastoma was investigated in 29 families. By using a simple parameter for estimating the severity of the disease (diseased-eye-ratio), we were able to identify four families with a discrete low-penetrance phenotype. The underlying genetic defect was identified in three families. One family has a 3-bp deletion in exon 16 that results in the deletion of Asn480. In two further unrelated families, the identical missense mutation at codon 661 in exon 20 (CGG to TGG, Arg to Trp) was identified. These mutations are distinct from the majority of retinoblastoma gene alterations, as they do not result in the disruption of the gene product. We propose that reduced penetrance of retinoblastoma is the result of a residual function of these alleles in retinoblastoma precursor cells.

Spectrum of small length germline mutations in the RB1 gene.

A screening method based on multiplexed automated fragment length analysis of polymerase chain reaction products was used to identify germline mutations in the RB1 gene. By screening 106 unrelated patients with hereditary retinoblastoma, 20 small deletions (1-18 bp) and seven insertions (1-5 bp) were identified. When collating our data with reported mutations, recurrence of small length mutations was observed at nine sites within the RB1 gene. Most of these contained monotonic runs or direct repeats embedded in homocopolymer tracts. While the majority of mutations resulted in premature truncation, two mutations caused an in-frame loss of F755 and G540 to E545, respectively. A genotype-phenotype comparison of patients carrying different small length mutations did not reveal any consistent relation. Particularly, the two patients with in-frame mutations showed a high number of tumours consistent with regular-penetrance retinoblastoma.

Endocrine management of breast cancer.

Hormonal therapy for breast cancer began more than a century ago with the observation that bilateral oophorectomy caused tumor regression in selected premenopausal patients. In the first half of this century, besides extending ablation of ovarian function to photon irradiation, surgical adrenalectomy and hyophysectomy were introduced, and hormonal additive therapy was established. Regression rates for advanced breast cancer with all types of endocrine therapy at this point did not exceed 35%. The demonstration that adjuvant systemic therapy can prolong the disease-free interval and improve overall survival has been a major advance in the management of breast cancer, the rationale was to control or eliminate micrometastases before tumor recurrence. The nonsteroidal antiestrogen tamoxifen was chosen for the majority of studies since the mid-1970s. Since the first report of successful treatment of metastatic breast cancer, the number of treated women worldwide has reached over 3,000,000. Objective response rate (CR+PR following UICC) in unselected patients is 34%. Tamoxifen has been used successfully to treat both pre- and postmenopausal women with all stages of the disease. In an overview analysis of 30,000 patients from 40 trials of adjuvant tamoxifen, a significant increase was found in both disease-free and overall survival. When patients were separated by nodal status, statistically significant increases were observed in disease-free and overall survival for both node-positive and node-negative patients. Women over 50 appear to benefit most from tamoxifen treatment experiencing highly significant increases in disease-free and overall survival regardless of nodal status. However, since tamoxifen primarily acts as a cytostatic and not cytotoxic agent, most patients ultimately experience disease recurrence or progression during or after therapy. Newer antiestrogens include trioxifene, toremifene, and droloxifene (3-OH-tamoxifen). Randomized, prospective studies are still under way to establish their clinical superiority (or lack of it). Progestins exert direct antiproliferative effects on human breast cancer cell lines. They may also exert direct antiestrogenic action by increasing the oxidative activity of 17 beta-hydroxy-steroid-dehydrogenase, thereby facilitating the conversion of estradiol to estrone. Progestins may exert additional antiestrogenic effects by suppressing estrogen receptor levels. As they also cause estrogen deprivation indirectly through suppression of pituitary ACTH secretion, resulting in reduced production of adrenal androgen precursors, both low- and high-dose regimens have been studied. Aromatase inhibition in premenopausal women interrupts estrogen biosynthesis; the reflex rise in FSH then stimulates production of new aromatase enzyme, and the LH increment results in enhanced ovarian steroidogenesis, counteracting the inhibitory action of aromatase-blocking drugs on the ovary.(ABSTRACT TRUNCATED AT 400 WORDS)

[Value of renal scintigraphy in the diagnosis of vascular complications of renal transplantation].

Nuclear medicine can make an efficient contribution to the diagnosis and monitoring of renal disease and to the assessment of therapeutic interventions in the field of renal transplantation. The new radio pharmaceutical MAG 3 labelled with 99mTc provides renal imaging of quality in patients with impaired renal function and enables quantitative evaluation of renal function. We report on radionuclide evaluation, with special emphasis on the analysis of the vascular component of the scintigram, in the different clinical situations (i.e. renal failure) that may compromise the outcome of a successful renal transplant.

Effect of prolonged postischemic perfusion on neonatal hearts.

Recent research on myocardial preservation has emphasized the importance of events occurring early in the reperfusion period, which may be of less importance to myocardial outcome in the neonate. We therefore wished to study the contribution of prolonged postischemic perfusion. This situation may occur during repair of congenital heart defects when in an attempt to reduce the period of aortic cross-clamp, the surgeon completes right-sided repairs on a beating empty heart. We used an isolated working rabbit heart model to compare recovery after various periods of ischemia and postischemic perfusion. After 30 min in the working heart mode, hemodynamic measurements were made. The hearts were then rendered globally ischemic, with the exception of cardioplegia. At the conclusion of the ischemic period, the hearts were returned to the 37 degrees C chamber, and reperfused with buffer. After 30 or 60 min retrograde perfusion, the hearts were converted to the working mode again. The working heart period continued until the heart was no longer able to overcome its afterload ("pump failure"). We recorded the time to this point as survival time. Hearts were divided into four groups: Group I (n = 12), 60 min ischemia, 30 min nonworking perfusion; Group II (n = 11), 90 min ischemia, 30 min nonworking perfusion; Group III (n = 8), 60 min ischemia, 60 min nonworking perfusion; and Group IV (n = 11), 0 min ischemia, 130 min nonworking perfusion. Multivariate analysis showed that four factors influenced survival time: aortic flow, age, left ventricular end-diastolic pressure, and mean aortic pressure. Painful caliceal calculi. The treatment of small nonobstructing caliceal calculi in patients with symptoms.

From 1984 to 1989 35 patients presented with uncharacteristic flank pain or recurrent urinary tract infections and small nonobstructing caliceal calculi. Thirty patients were treated; 13 with percutaneous stone extraction, 8 with extracorporeal shock wave lithotripsy, 3 with ureteroscopic stone manipulation and 2 by open surgery. Stone removal was successful in 39 patients and they were all relieved of their symptoms (86%). Stone size was decreased in 2 patients who felt a marked alleviation. Where the stone remained unaffected the symptoms persisted unabated. These observations suggest that small caliceal stones can cause pain. The results of treatment are such that it is justifiable to treat a small caliceal stone in patient suffering from flank pain of no other known cause.

[Prognosis of surgery performed in premature infants with birth weights below 1500 gram].

On account of improved management in neonatal intensive care units, the survival rate is presently nearly 50% in babies with birth weights below 1,000 grams and 80% in the group with birth weights from 1,000 to 1,500 grams. A number of these babies require surgery, either for correction of congenital anomalies or for acquired necrotizing enterocolitis, which is an increasing burden in neonatal units. The present analysis of 24 babies weighing under 1,500 grams, operated on in the period 1981 to 1988 confirms that even extremely small babies tolerate the performed anesthesia and surgery well. In the group with anomalies (7 of 24) three died (43%). The causes of death do not appear to be related to the anesthesia or surgery. In the group with necrotizing enterocolitis (15 of 24) six died (40%). The cause of death was directly related to the septic condition in all cases, and the surgical trauma does not appear to have any negative influence. Unfavourable prognostic factors in this group were low Apgar scores within the first minute and preoperative weight loss.

Assessment of voiding dysfunction in men with acute epididymitis.

Fifteen men with a recent attack of acute epididymitis were examined urodynamically and endoscopically after the symptoms had resolved. Thirteen of the 15 patients had abnormal voiding. Two patients had stricture formation in the posterior urethra, one had benign prostatic hyperplasia, one had bladder neck sclerosis, and 4 had detrusor-external sphincter dyssynergia. Three patients including the one with prostatic enlargement presented abdominal straining exceeding 50 cm H2O during micturition. The cause of an obstructive voiding pattern could not be established in 3 patients. The results support the theory of urethrovasal reflux due to a high hydrostatic pressure at the collicular level as a pathophysiological factor in acute epididymitis.

[The control of storage mites in the mycological laboratory].

Mycological cultures in the laboratory are threatened by storage mites who feed on fungi and cause contamination by transferring bacteria and fungi from culture to culture. To eliminate mites from fungal cultures without damage to the fungi themselves several chemical and physical procedures were tested. The following substances were added to Sabouraud glucose (2%) agar at concentrations of 0.1 and 1.0%: gamma benzene hexachloride (lindane), N-ethyl-o-crotonyltoluide, pyrethrins, dieldrin N,N-diethyl-m-toluamide, benzyl benzoate, and--as popular household remedies--Eau de Cologne, neutral soap and garlic. The media were inoculated with 4 dermatophytes (T. rubrum, T. mentagrophytes, M. gypseum and E. floccosum). Two to three weeks later the fully grown cultures were contaminated with mites and their eggs. In other test series, mite infested cultures were exposed to heat (40 degrees C and 60 degrees C) or cold from -12 degrees C to -28 degrees C over different periods of time. Finally, the protective value of sealing culture dishes with adhesive tape was investigated. The most useful method to control mite infestation in fungal cultures seemed to be exposure to heat over a short time.

[Reconstruction of the bladder neck in exstrophy of the bladder or epispadias using Young-Dee's method].

Young-Dees' method of bladder neck reconstruction is described. A material of 17 cases with complete or almost complete incontinence on account on a deficient sphincter who were submitted to this operative method is presented. It is concluded that the method can be recommended as the primary operation for incontinence, particularly in patients with exstrophy of the bladder or epispadias as satisfactory continence could be obtained in 60%. No deterioration of renal function was observed and the patients functioned well both socially and sexually.
