Effects of terbutaline on the pressure volume relationship in fetal rabbit lung.

The pressure-volume relationship in preterm rabbit lung was studied at 28 days of gestation. Injection of 0.1 mg terbutaline, a selective beta2-receptor stimulating drug, significantly increased the volume of air at equivalent low transpulmonary pressures, compared to a saline treated group and an untreated group. These findings indicate an increased pulmonary distensibility of the fetal rabbit lung after terbutaline administration. The mechanism of action is discussed and surfactant mediated effects are suggested to be the probable explanation.

Antepartum administration of terbutaline and the incidence of hyaline membrane disease in preterm infants.

The incidence of hyaline membrane disease in 41 preterm infants born between January 1975 and January 1976 was investigated in a retrospective study. Terbutaline, a beta2-receptor stimulating drug, had been administered to the women in order to arrest premature labour. In spite of the treatment 24 preterm infants were delivered. The incidence of HMD in this group was 1/24. In a group of 17 neonates whose mothers had received no such treatment the incidence of HMD was 5/17. Comparison between the two groups revealed a significantly lower incidence of HMD in the terbutaline treated group than in the controls (p less than 0.05). There were no significant differences in maternal age, gestational age, birth weight or Apgar score between the two groups. The lower incidence of HMD in the terbutaline treated group is suggested to reflect a rapidly induced release of pulmonary surfactants in the preterm infants. Evidence for a similar sequence of events has earlier been shown to occur in animals under standardized experimental conditions.

Ureteral reflux from ileal conduit.

In 14 patients with ileal conduit urinary diversion the frequency of ureteral reflux was studied and the intraluminal conduit pressure was registered. Reflux was found in 20 (67%) of the 30 ureters (2 patients with 3 ureters each). The frequency was approximately the same in the erect and supine body positions. Low pressure reflux (less than or equal to 12 cm H2O) was observed in about 50%. No correlation between the conduit pressure and the occurrence of ureteral reflux was found.

Isolated FSH deficiency in a male. A case report.

This report describes the case of a 26-year-old man investigated because of infertility. The patient appeared healthy with normal sexual activity, physical appearance and karyotype. Repeated semen analyses showed a marked oligozoospermia. A testicular biopsy specimen revealed arrested spermatogenesis and normal Leydig cells. Hormone analyses were normal except for a markedly reduced serum FSH level. Only a few such cases have been previously reported.

Back-flow from urinary collecting devices into the ileal conduit.

The occurrence of back-flow from urinary collecting devices into the ileal conduit was observed in 9 out of 46 patients. In 6 of them back-flow to the upper urinary tract also occurred. Nipple stomas of at least 2 cm height were found to resist back-flow into the conduit better than any flat stoma.

Caries and periodontal status in patients fitted with removable partial dentures.

A 6-year follow-up study was carried out on 28 patients fitted with removable partial dentures. Prior to the prosthetic treatment all patients were given oral hygiene instruction and periodontal therapy where indicated. The removable partial dentures were carefully planned and designed. During the follow-up period the patients were examined at yearly intervals when remotivation, scalings, conservation and prosthetic adjustments were carried out as required. Patient cooperation was excellent and no significant deterioration of the periodontal status of the remaining teeth could be found. In addition the number of newly registered carious lesions was low. A certain degree of deterioration of occlusion, articulation, stability and clasp retention of the dentures fitted did occur, however.

Dissolution and uptake of cadmium from dental gold solder alloy implants.

Pure metallic cadmium was irradiated by means of thermal neutrons. The irradiated cadmium (115Cd) was placed in bags of gold foil and the bags were implanted subcutaneously in the neck region of mice. Two and 3d respectively after implantation the mice were killed, the bags removed and the animals subjected to whole-body autoradiography. The autoradiograms revealed an uptake of 115Cd in liver and kidney. In another experiment specimens of a cadmium-containing dental gold solder alloy, a cadmium-free dental casting gold alloy and soldered assemblies made of these two alloys were implanted subcutaneously in the neck region of mice. The animals were killed after 6 months; cadmium analysis showed significant increases in the cadmium concentration in liver and kidney of those mice which had been given implants of gold solder alloy. The study clearly shows that due to electrochemical corrosion cadmium can be released from implants and accumulated in the kidneys and the liver.

Duplication of the renal pelvis and blind-ending bifid ureter in twins.

Blind-ending bifid ureter in a woman with urinary tract infection and a duplicated renal pelvis in her twin sister are reported. The blind-ending bifid ureter terminated as a fibrous cord with a terminal mass of immature renal tissue. This type of ureter may be a transitional form between histologically blind-ending bifid ureter and duplication of the renal pelvis and/or kidneys. Findings in twins support this opinion.

Controlled treatment of primary hypertension with propranolol and spironolactone. A crossover study with special reference to initial plasma renin activity.

Twenty-seven patients with hypertension were randomly allocated to a 10 month crossover study. Treatment consisted of spironolactone (200 mg/day for 2 months), propranolol (320 mg/day for 2 months) and combined administration of both drugs at half the dosage. Between treatment periods placebo was given for 2 months. Fourteen patients were previously untreated. The average pretreatment blood pressure for the entire group was 188/114 +/- 16/7(mean +/- standard deviation) mm Hg supine and 188/118 +/- 20/9 mm Hg standing. Both spironolactone and propranolol reduced blood pressure significantly in both the supine and standing positions. Upright plasma renin activity was determined by radioimmunoassay of angiotensin I. The average initial level was 1.9 +/- 1.2 (range 0.4 to 5.0) ng/ml/hr. There was a close correlation between plasma renin activity and the effects of the drugs: With increasing renin level the response to propranolol was better whereas the opposite was true for spironolactone. The combination of spironolactone and propranolol decreased the blood pressure still further in the supine and standing positions, irrespective of initial plasma renin activity. All patients achieved a normal supine pressure. Blood pressure and plasma renin activity returned toward pretreatment values during placebo administration. It is concluded that pretreatment levels of plasma renin activity can predict the antihypertensive response to propranolol and spironolactone. The combination of the two drugs, which have different modes of action, will effectively reduce blood pressure in hypertension. The results support the concept that the renin-angiotensin-aldo-sterone system may be involved in primary hypertension.

The incidence of venous thrombosis in the lower limbs following elective gallbladder surgery.

The frequency of postoperative venous thrombosis and the prophylactic effect of dextran 40 and 70 were studied in 90 otherwise healthy patients undergoing cholecystectomy. The 125-I-fibrinogen test was employed as the diagnostic method. The frequency of thrombosis was very low (1.1%) compared with other investigations on patients undergoing elective general surgery. Thus nothing can be concluded about the prophylactic effect of dextran from this series, but it can be presumed that this type of patient does not need routine prophylaxis against deep venous thrombosis.
