HSJ managers network. Stuck in the middle.

HSJ has launched a unique regular survey based on a network of 400 middle managers. Respondents are feeling insecure, ill-informed and uncertain about the future--but remain loyal to the NHS. Flexibility and recognition are the prime motivators for middle managers.

Local commissioners will require improved analytical skills to identify the specific health needs of their populations. Commissioning must be a counterweight to acutes that will be pushing for more work under contestability.

Equality. No through road.

NHS Employers is pushing to promote equal employment opportunities for disabled people. The Disability Discrimination Act allows disabled people to follow medical careers, but progress remains patchy and varies by area. There have been calls for the General Medical Council to issue national disability guidance for medical schools.

Primary care. Ringing the changes.

A 2000 DoH review called for out-of-hours services to be straightforward for users to access, and for service planning to involve multi-disciplinary co-operation from health professionals. Out-of-hours provision has evolved since then, but missed targets, insufficient staff numbers and inequalities between different providers continue to hamper progress.

Practice-based commissioning. Ready or not?

As the practice-based commissioning revolution gets underway this month, our survey of primary care trusts and practices finds that two-thirds of PCT chief executives feel practices are not yet ready to deal with the new system. Meanwhile, half of practices are intending to manage commissioning budgets this year, with 98 per cent intending to be involved by 2008
