Intrathecal steroid therapy for post-traumatic visual disturbance.

We have given intrathecal injections of dexamethasone more than 2000 times for approximately 200 cases with visual disturbance and 20 cases with sciatic pain. An injection of 8 mg was given once or twice a week until a total dose of 80 to 100 mg was given. Six of the 14 cases that were thus treated in their chronic stage for post-traumatic visual disturbance showed various degrees of improvement in visual fields and acuity. There were no serious complications in this series. It is concluded that intrathecal steroid injection is effective in certain cases of post-traumatic visual impairment.

A simple hemostatic method using a metal shield for injury of a major cerebral artery.

A new hemostatic method has been devised that uses a metal shield for treating accidentally injured major cerebral arteries. The metal shield is made of stainless steel, and has a small plate on its outer surface that allows it to be held in a regular aneurysm clip holder. Three different sizes of shield are available. The metal shield is applied to an injured artery with Oxycel and Biobond and held for a few minutes. Bleeding stops instantaneously. This is a simple and effective hemostatic method for an injured artery of small size. Experimental results and clinical application of this method are described.

Floating operator chair.

A newly designed operator chair is suspended by a floor stand and a horizontal arm that rotates and is adjustable for length. The suspension system is equipped with magnetic locks that enable the surgeon instantaneously to take desired positions in a horizontal plane. The height of the chair is controlled electrically. The foot plate, which is in one piece with the chair, has 18 switches for the microscope, chair, bipolar coagulator, camera, and so on. This comfortable and multifunctional chair is an important microneurosurgical tool not only for minimizing the physical fatigue of the surgeon but also for performing refined techniques.

Direct retraction method in aneurysm surgery. Technical note.

A new technique of direct retraction of an aneurysm or major arteries using tapered brain retractors is introduced. The system requires three separate instruments: 1) a tappered brain retractor, 2) a flexible self-retaining retractor of light weight, and 3) a multipurpose head frame.
