Primary osteoma cutis of the back.

A case of primary osteoma cutis on the back is reported. This case showed an unusual form of calcification on soft x-ray. Histologic examination disclosed mature bone in the subcutis. There was no history or evidence of any skin disease prior to the development of the osteoma. The literature is reviewed and discussed.

Flexible rhombic flap.

A flexible rhombic flap which can be designed in accordance with the degree of tension of skin bordering a part of the defect has been described. This procedure provides sufficient coverage of a skin defect with very little dog-ear.

Method of tying up hair during hair transplantation.

We have reported on a simple and secure method of tying up hair during transplantation surgery for alopecia. The implements used were Kocher's forceps and rubber bands which are readily obtainable in the operating theater.

Problems encountered in contouring a reconstructed ear of autogenous cartilage.

One-hundred and sixty-six patients were followed up for a year following transplantation of costal cartilage, and we studied the relationship between the clarity of contours of the reconstructed ear and the condition of the scar on the anterior chest following removal of the cartilage. The contours of the reconstructed ear were evaluated on a four-point scale, and the condition of the scar on the anterior chest was evaluated on a three-point scale. Our results showed that in patients with a predisposition to develop wide or hypertrophic scars on the anterior chest, the contours of the ears were often observed to be poorly defined.

Hamartoma of the tongue: two case reports.

Hamartoma designates an excessive focal overgrowth of mature normal cells and tissues in an organ or tissue composed of identical cellular elements. Two cases of hamartoma at the back of the tongue are reported. Histologically they comprised fibrous tissue, fat, salivary gland lobules, and smooth muscle. The literature is reviewed and discussed.

Argon laser therapy of port-wine stains: effects and limitations.

One hundred and one patients (112 parts) with port-wine stains were treated by argon laser therapy. We strictly evaluated all patients according to color shift, scar formation, and pigmentation. We observed good to excellent results in 44 percent of patients, and less satisfactory results in 56 percent. The relationship between the effects of treatment and several factors, such as patients' age, anatomic site, and histologic type of the lesions, were investigated. More satisfactory results tended to be obtained in patients with neck lesions and with histologic type III lesions (dilated type). To date we have not discerned any definite criteria for precisely predicting the response of port-wine stains to argon laser therapy. Consequently, the performance of a test seems to us to be the best way of discovering the response and course of healing of an individual port-wine stain after laser therapy. Thirty-six consecutive patients were studied to determine whether compression and cooling would improve the outcome of argon laser therapy. The use of this method did not produce any noticeable effects.

Inverted-U parascapular flap for the treatment of axillary burn scar contracture.

In this paper we report the technique of using an inverted-U parascapular flap for treating axillary scar contracture. The advantages of using this inverted-U flap are that it is possible to close the donor site by primary suturing, it is possible to cover a large skin defect, and it is possible to construct either a cavity or a swelling in the skin-defect region.

A nail Island flap for treatment of macrodactyly.

A case of macrodactyly of the middle finger is presented. An easy and versatile one stage surgical method, using an Island axial nail flap is proposed. This method eliminated the need for secondary intervention.

Combined frontal bone reshaping and forehead lift for frontal sinus hypertrophy.

A technique to correct frontal sinus hypertrophy is presented. The outer table of the frontal sinus is removed and divided into three pieces with an oscillation saw. The pieces are trimmed at the margin, fixed with two microplates, and regrafted. Bilateral lateral thick portions of the supraorbital rim are shaved with a surgical burr. At the same time, a forehead lift is performed. A satisfactory result with a flat and wide forehead can be obtained by employing this technique.

3D-CT stereoscopic imaging in maxillofacial surgery.

We obtained stereoscopic 3D-CT images in maxillofacial bone fracture patients. These images are made at two different angles. One is equivalent to the view obtained by a subciliary incision during surgery. Another is equivalent to the view obtained by oral incision during surgery. A stereoscopic image is created with a pair of images that differ from each other by a 6 degree shift of the z axis.

Detection of the preneoplastic lesions of small hepatocellular carcinoma in cirrhotic livers.

To identify the preneoplastic lesions of hepatocellular carcinoma and the fine structure of preneoplastic hepatocytes, we studied proliferative conditions in cirrhosis of the liver. In all, 46 foci of cellular alteration (FCA), three regions of adenomatous hyperplasia (ADH), and 21 small hepatocellular carcinomas (sHCC) were studied by published criteria for sHCC and by the proliferative activity of the lesions as examined with monoclonal antibodies against DNA polymerase alpha and proliferating cell nuclear antigen. The four patients with FCA composed of basophilic hepatocytes were classified by the criteria as having sHCC; cells had features similar to those of sHCC. Two of these four patients with FCA were found to have HCC several years later. The number of hepatocytes stained for proliferating cell nuclear antigen was 72 and 81 per 1000 hepatocyte nuclei in the two patients who developed HCC. In one of the three patients with ADH, a sHCC was found 1 year later, and dysplastic hepatocytes from the region of ADH in this patient had features similar to those of HCC cells by light and electron microscopy. In this patient, the number of hepatocytes stained for DNA polymerase alpha was 452 per 1000 nuclei. Therefore, FCA and ADH might be preneoplastic lesions of sHCC in cirrhosis of the liver. Preneoplastic hepatocytes seem to be small cells with basophilic cytoplasm, with a large nucleus to cytoplasm ratio, finely indented nuclei with a smaller amount of condensed chromatin than normal, and poorly to moderately developed organelles.
