Bioavailability of a film-coated tablet of valproate in nonfasting volunteers.

The bioavailability of a film-coated tablet of valproate (VPA) in nonfasting volunteers was studied. Ten healthy adults received a single 200 mg (mean 3.55 mg/kg) film-coated tablet, 15 minutes after breakfast. Venous blood was then taken at regular intervals of up to 4 or 7 hours after the dosage, and the serum VPA concentrations were determined by means of a fluorescence polarization immunoassay. VPA was slowly absorbed with the peak concentrations occurring 1.5 to 4 hours after the administration. The mean concentration (Cmax) and extent of absorption (area under the concentration-time curve) between 1 and 4 hours were 22.2 micrograms/ml and 59.3 micrograms.h/ml, respectively. It was concluded that Cmax with a film-coated tablet of VPA can be detected if blood is taken at around 3 hours after the dosage under nonfasting conditions.

Identification of porins in outer membrane of Proteus, Morganella, and Providencia spp. and their role in outer membrane permeation of beta-lactams.

Proteus mirabilis, Proteus vulgaris, Morganella morganii, Providencia rettgeri, and Providencia alcalifaciens, which were once classified into the same genus, Proteus, were studied. Cefoxitin-resistant mutants from these species were isolated, and it was confirmed that the resistance was attributed to the lack of an outer membrane protein, resulting in a significant decrease in the penetration of hydrophilic cephalosporins through the outer membrane. Comparison of the mutant strains with their parental strains in the diffusion rates of six monoanionic cephalosporins, a zwitterionic cephalosporin (cephaloridine), and a divalent anionic cephalosporin (cephalosporin C) suggested that each species had only one kind of porin protein, with molecular weights of 40,000 (Proteus mirabilis) or 37,000 (the other four species) and that the porins formed channels with cation selectivity, except for Proteus vulgaris. Porin proteins were purified from all the bacterial species except Providencia alcalifaciens, and the radius of the pores formed by the purified porins was estimated by the use of the liposome swelling assay. The pore radii were estimated to be approximately 0.59 nm (Proteus mirabilis), 0.63 nm (Proteus vulgaris), 0.58 nm (Providencia rettgeri), and 0.60 nm (M. morganii), similar to the size of the pore radius of Escherichia coli porins.

Liver metastases from colorectal cancers: detection with CT during arterial portography.

A total of 45 metastases to the liver from colorectal cancer were resected in 22 patients. The detectability of these lesions with the following modalities was determined: real-time ultrasound (US), computed tomography (CT), selective celiac arteriography (SCA), infusion hepatic angiography (IHA), CT during arterial portography (CTAP), and CT following intraarterial injection of iodized poppyseed oil (Lipiodol). The total detection rate (sensitivity) was 58% for US, 63% for CT, 27% for SCA, 50% for IHA, 84% for CTAP, and 38% for CT with iodized oil. Ten of 18 lesions less than 15 mm in largest diameter were demonstrated preoperatively by CTAP only. CTAP is useful in clarifying the locations of the lesions in the liver and should always be performed before liver metastases from colorectal cancer are resected.

Thromboxane A2 synthesizing activity of platelets in coronary artery diseases.

To observe platelet TXA2 synthesizing activities in acute myocardial infarction (AMI, 38 cases) and effort angina (EA, 23 cases), radioimmunoassay was used to measure the amount of synthesized TXB2 at 5 min after an addition of arachidonic acid or thrombin to the platelet suspension. The amount of TXB2 in AMI patients did not show a significant difference from that of 13 healthy controls. However, there were significant changes during the time course of AMI. It increased in the super acute phase within the first 12 hrs from the onset of AMI. The activity decreased in 4 days, increased again in 10 days and then gradually recovered to the normal value. Platelet aggregation was elevated immediately after the onset of AMI and recovered during the time course. A significant negative correlation was observed between aggregation and TXA2 production in AMI, but not in the healthy controls, suggesting that platelets with hyper TXA2 synthesizing activity are consumed selectively in AMI. In EA, activity increased after treadmill exercise. Under ticlopidine-treatment, activity was depressed, and these patients were able to tolerate a longer exercise time than before ticlopidine. Since the pressure rate products did not change under treatment, changes in microcirculation such as the appearance of platelet aggregates may be important in the occurrence of anginal attacks.

Establishment and characterization of T-cell lines (Yana-I and Yana-II) derived from a patient with chronic adult T-cell leukemia (ATL).

ATL-cell lines producing C-type retroviruses were established by culture of the peripheral blood mononuclear cells from a patient with chronic ATL. These cell lines had the surface feature of inducer/helper T-cells with electron microscopic finding of C-type retrovirus on the cell surface. Chromosome analysis revealed the presence of a marker chromosome, but did not identify 7 trisomy nor 14q+. Of the cell lines, the line established by culture with TCGF was named "Yana-I" and the line established by culture of Yana-I without adding TCGF was named "Yana-II". Serum ATLA-antibody was positive in 1.5% of the healty individuals examined (4/257). Among malignant lymphoma patients, ATLA antibody was positive in none of the patients with B-cell type lymphoma (0/10) and Hodgkin's disease (0/2). In contrast, ATLA antibody was found in half of the patients with T-cell type lymphoma (3/6) and all with ATL (4/4). This suggested that C-type retrovirus in closely associated with ATLL.

A simple method for quantification of biotinidase activity in dried blood spot and its application to screening of biotinidase deficiency.

A simple and reliable method for quantification of biotinidase (EC.3.5.1.12) activity in dried blood spot was devised by a modification of the colorimetric screening test developed by Heard et al. (1984). The enzyme reaction and hemoglobin denaturation were carried out in a U-bottomed microplate. An aliquot of the reaction solution was transferred to a flat-bottomed microplate. After the coupling reaction was started, the adsorbance was measured in situ by a microplate-reader. Both intra- and inter-assay coefficient of variation (CV) values were less than 10%. Biotinidase activity in dried blood spot showed a good correlation to that in serum (r = 0.912, n = 8). This method was applied in a pilot screening of 18,945 newborns in Sapporo City. No positive results have been obtained as yet.

Bolus methylprednisolone therapy in adult idiopathic thrombocytopenic purpura.

Eleven patients with adult idiopathic thrombocytopenic purpura (ITP) were treated with bolus methylprednisolone at doses of 1 g/day for 3 days. In nine of eleven patients the platelet count rose to more than 100 X 10(9)/l during 2-9 days. In two other patients, it rose to 81 X 10(9)/l and 43 X 10(9)/l. In six of eleven patients splenectomy was carried out without any platelet transfusion when the platelets rose to more than 100 X 10(9)/l after bolus mPSL therapy. Bolus mPSL therapy is efficient as a pre-treatment for a planned splenectomy in adult ITP.

Lysozyme secreting tumor: a case of gastric cancer associated with myelofibrosis due to disseminated bone marrow metastasis.

One patient with gastric carcinoma and secondary myelofibrosis due to disseminated bone marrow metastasis had markedly elevated lysozyme (LZM) levels in serum and urine with the intense presence of LZM within tumor tissues. It is considered to be a case of gastric carcinoma with LZM secreting functional capacities. To date, there were many reported cases to verify the LZM positive cells by LZM staining in the tissue of gastric carcinoma and to demonstrate the elevated serum levels of LZM in malignant tumor bearing patients, whereas no papers to disclose the elevated levels of LZM in serum and urine originated from the productions and secretions of gastric carcinoma cells. So, this report might be the first reported case of LZM secreting tumor verified by LZM staining of carcinoma cells except for haematological malignancies such as acute monocytic leukemia or myelomonocytic leukemia.

A novel antigenic determinant of the T200 glycoprotein expressed exclusively by Jurkat cells.

This report describes the production and characterization of monoclonal antibody specific for an antigenic determinant, designated Y1 antigen, present on Jurkat cells. Mo-Y1 specifically reacts with Jurkat cells, but not with other cell lines. Immunoprecipitation and subsequent immunochemical analysis demonstrate that Mo-Y1 defines an antigen with a molecular weight of 200,000 and a pI value of 5.3 to 5.6. Rabbit antibody against affinity-purified Y1 antigen was prepared. A sequential immunoprecipitation study confirmed that Po-Y1 reacted with the same antigen molecule recognized by Mo-Y1. Interestingly, Mo-Y1 reacted only with Jurkat cells, whereas Po-Y1 reacted with various hematopoietic cells including T cells, B cells, macrophages, and granulocytes. Po-Y1 precipitated a 200,000-dalton component from Jurkat cells and T cells. In contrast, Po-Y1 precipitated a 230,000-dalton component from B cells. These data led us to examine the relationship between T200, leukocyte-common antigen and Y1-defined antigen. Sequential immunoprecipitation analysis clearly indicated that T200 antibody and Y1 antibody recognized the same antigen molecule. Peptide mapping of T200- and Mo-Y1-defined antigen digested with V8 protease demonstrated that there were two common major peptide chains, but one peptide chain was missing in Jurkat cells that was present in T200 glycoprotein of other cells. It is possible that minor molecular modification of T200 glycoprotein occurred in the process of oncogenesis of T cells and that the altered epitope of T200 glycoprotein was recognized by Mo-Y1. The Y1 antigen can be classified as a tumor-specific antigen.

[A case of parotid tumor showing remarkable regression following hyperthermo-chemo-radiotherapy].

A 72-year-old woman developed adenocarcinoma of the left parotid gland. Because of the excessive size of her tumor and the fact that she suffered from severe liver dysfunction, she was treated by hyperthermo-chemo-radiotherapy (HCR therapy). After ten sessions of radiofrequency hyperthermia with HEH 500 (13.56 MHz radiofrequency wave), 50-Gy irradiation from a linac and administration of 33.0g of tegafur in suppository form, the tumor mass showed remarkable regression decreasing in size by as much as 84% on computed tomography. Histologically the tumor which was resected under local anesthesia, showed almost total necrosis. The multidisciplinary HCR therapy was well tolerated and effective as a therapy for cancer in this case.

[Functional and biochemical characterization of interleukin 2 receptor-associated molecules].

The interaction of interleukin 2 and the interleukin 2 receptor plays a crucial role in T cell proliferation. Unlike other growth factor receptors, the interleukin 2 receptor has a rudimentary intracytoplasmic portion that is too short to mediate any kinase activity. There are two classes of interleukin 2 receptor, with either high or low affinity. Only the high-affinity receptor mediates interleukin 2-driven transmembrane signal transduction. However, the mechanisms that control the affinity of the receptor and mediate transmembrane signal transduction are unknown. We report here that the rat interleukin 2 receptor has an associated molecule. The association of the rat interleukin 2 receptor with rat lymphocyte activation antigen, 5C6-F4 was demonstrated using the chemical cross-linking reagent, DTSSP. It is likely that the formation of a complex between the receptor and 5C6-F4 is important for the generation of the functional interleukin 2 receptor.

[Clinicopathological studies of nuclear DNA distribution in colorectal cancer].

The nuclear DNA content of paraffin-embedded materials was measured by microfluorometry in 108 colorectal (59 colon and 49 rectal) cancers and 29 metastatic liver tumors from the colorectum. The classification of the nuclear DNA distribution is as follows: Low ploidy, 10% greater than over 4C; intermediate ploidy, 10% less than or equal to over 4C and 10% greater than over 6C; high ploidy, 10% less than or equal to over 6C. In primary lesions, thirty-one (29%) were found to contain cells with high ploidy. The ploidy pattern was not related to pathological stage. However, in curatively resected cases, only 24% of patients with high ploidy survived 10 years. This 10 year-survival rate was significantly lower as compared with 80% and 88% of patients with intermediate and low ploidy, respectively (p less than 0.05). In non-curatively resected cases, no patients with high ploidy survived 2 years, although 21% and 67% of patients with intermediate and low ploidy survived, respectively. In metastatic liver lesions, eleven materials (42%) were found to contain cells with high ploidy. Prognosis of patients with high ploidy was worse than that with intermediate and low ploidy (p less than 0.05). These observations suggest that the nuclear DNA distribution in colorectal cancers may be an important prognostic factor, independent of pathological stage.

[A long-surviving case of gastric cancer with liver metastasis without signs of recurrence for six years after palliative operation].

A 51-year-old man was admitted because of Borrmann 2 type gastric cancer located in the antrum. Since he had bilateral liver metastasis, palliative gastrectomy and liver biopsy was performed. Histopathologically, the primary cancer showed poorly differentiated adenocarcinoma and the liver biopsy showed the same. This patient has been alive without sign of recurrence for more than six years after he was given 40 mg of MMC, 12.5 KE of Picibanil and 528 g of PSK postoperatively. In addition, eight similar cases, with a survival rate of more than 5 years after resection of liver metastasis, have been reported in Japan and are reviewed.
