[Computer-assisted documentation and therapy planning in pediatric oncology--introduction of a nationwide solution].

In Pediatric Oncology in Germany, 90% of the patients are treated according to multicenter clinical trials, which means an enormous effort for documentation in the participating clinics. In order to enable multiple use of data for patient records as well as for clinical trials a computer-based documentation system for pediatric oncology (DOSPO) is being developed, which can be used nationwide. DOSPO currently comprises a minimum basic data set, which represents the common core of all multicenter trials and which has been approved by the German Society for Pediatric Oncology and Hematology (GPOH). It is intended to enhance the documentation by specific items of each clinical trial. Functions for computer-aided chemotherapy planning and medical report writing have already been implemented in the documentation system. Various centers in Germany are currently validating the system in routine use.

Induction in chimpanzees of antibodies inhibiting receptor-mediated cell fusion by HIV glycoprotein.

Cell fusion of HTLV-IIIB-infected EBV-transformed B cells and CD4+ T cells was inhibited by sera from eight of nine HIV infected chimpanzees. Syncytia formation was reduced by sixty percent relative to control after only 5 minutes of preincubation of the HIV infected cells with immune primate serum, indicating that these antibodies have high affinity for HIV protein on the surface of infected cells. Serum dilutions that blocked formation of syncytia irreversibly within 24 hrs also blocked expression of HIV antigens by the target CD4+ cells. Three of four animals inoculated with the LAV or HTLV-IIIB strain of HIV developed antibodies inhibiting CD4-dependent cell fusion by HIV glycoprotein (CFI-antibodies) 2-3 months after inoculation coincident with development of HIV specific IgG antibodies. Similar early CFI-antibody responses occurred in two second passage chimpanzees. In contrast, a chimpanzee infected with a third passage of LAV had a delayed CFI-antibody response, indicating that variants of HIV with divergent CFI epitopes did eventually emerge. Delayed development of CFI-antibodies (6-11 months after inoculation) relative to HIV specific IgG ELISA antibody was also seen in a chimpanzee on primary passage and a chimpanzee on second passage of HTLV-IIIB. No CFI-antibodies were detected in a chimpanzee following inoculation with human brain tissue, while antibodies to other structural proteins were recognized by immunoblotting. These results indicate that changes in CFI epitopes occur under immune pressure and that the appearance of CFI-antibodies depends on the time after infection and on the degree to which CFI epitopes of the inoculum strain diverge from those of the test strain.

Can salivary composition and high flow rate explain the low caries rate in children with familial dysautonomia?

PURPOSE: Extremely low caries rate and increased major salivary gland flow rare have been previously reported in children with familial dysautonomia (FD). The purpose of this study was to explore the possibility that, in addition to increased salivary flow, children with FD have alterations in their salivary components, which may suggest an explanauon to their low caries rate. METHODS: Whole unstimulated and stimulated saliva samples were collected from 13 children with FD who were found to be caries free, and from 28 age- and ethnic-matched healthy children, 15 caries-free children and 13 caries-affected children. The electrolyte and protein content of the unstimulated saliva and the microbial count and buffering capacity of the stimulated saliva were determined. RESULTS: Children with FD had the highest salivary flow rate and the lowest levels of mutans streptococci and lactobacilli, as well as the lowest concentration of chloride, magnesium, total protein and IgA. Healthy caries-affected children displayed the highest mutans streptococci and lactobacilli levels and lysozyme concentration, concomitantly with the lowest potassium and calcium concentrations. CONCLUSIONS: The results of this investigation suggest that the caries-free state in FD may be associated with high salivary flow rate, while in healthy children, low caries rate may be associated with high salivary calcium concentration.

AIDA: Web agents to support dental treatment planning.

Dental treatment planning is highly dependent on the educational background and personal experience of the dentist and on the thoroughness with which findings are taken. The objective of research in the AIDA (Artificially Intelligent Dental Agents) project is therefore the analysis of dental decision making and the design of a decision-supportive software module to test the decision-making process and make it transparent for both dentists and patients. In the AIDA project (aida.uni-hd.de), an expert system has been set up to identify treatment alternatives with integrated decision rationale for experts, practicing dentists, and eventually also patients. It is based on a top-down structure for dental decision-making, which was developed to standardize argumentation. For prosthetic dentistry, how one uses individual patient findings to arrive at possible treatment alternatives was clarified in greater detail. The planning rules for fixed prostheses have already been integrated into a software agent. Other modules should soon specify and verify these suggestions in terms of further fields of dentistry (e.g., restorative dentistry). In the near future, AIDA will be able to deliver dentally-founded justifications for every individual decision.

[Purification and characterization of "Clostridium perfringens" BP6K-N5 strain bacteriocin N5 (author's transl)].

The bacteriocin N5 produced by Clostridium perfringens, strain BP6K-N5, after UV irradiation induction, was purified by ammonium sulfate precipitation, followed by ion-exchange chromatography on DEAE cellulose and gel filtration on Sephadex G100. By polyacrylamid-gel electrophoresis and immunodiffusion analysis the purified material was shown to be homogenous. The purified bacteriocin N5 is inactivated by proteolytic enzymes and by heat treatment at 50 degrees C for 15 minutes. It is a simple protein with a molecular weight of approximately 82.000. The protein appears to be a single polypeptide chain, as no dissociation is obtained by sodium dodecyl sulfate and beta-mercaptoethanol disc-gel-electrophoresis.

The dental status of children with chronic renal failure.

We evaluated the dental status of thirty children with chronic renal failure (CRF): 15 managed conservatively, 9 on dialysis replacement therapy and 6 after kidney transplant. Dental age was delayed, but to a lesser extent than bone age; this delay was most marked where CRF occurred before 7 years of age. Eruption age was only minimally delayed. Eighteen patients (60%) showed enamel defects, mainly hypoplasia, the location of which was related to the age at which major metabolic changes of uremia appeared. In 9 children (30%) there was intrinsic discoloration of the teeth--this was also related to the severity and duration of CRF. Compared to normal Israeli children our patients showed poor oral hygiene and gingival condition, but a significantly lower prevalence of caries. Seven patients (23%) showed mild radiologic changes in the jawbones (mainly loss of lamina dura), the severity of which was related to the severity and duration of CRF. None of the above findings could be related to specific metabolic disturbances.
