[Recurrent periprosthetic capsular retraction under the pectoralis major muscle. Treatment, 2 unusual clinical cases].

Capsular contracture around breast prostheses still represents a challenge, in spite of the numerous papers published on this topic. The etiology and pathogenesis of this condition are perplexing resulting in considerable controversy concerning the rationale for surgical treatment. The authors report two cases of recurrent disease: one patient with recurrent contracture after breast augmentation, the other patient with recurrence after breast reconstruction. Both patients were operated according to an original technique which consists in a square-shaped incision of the pectoralis major muscle via an inframammary approach. This surgical technique allows the prosthesis to sink into deeper layers leading to a better shape of the breast and providing satisfactory skin cover of the prosthesis.

[Surgical treatment of saddle nose, our experience apropos of 19 cases].

19 patients presenting with saddle deformity of the nose were operated upon between 1981 and 1988. Either a bony or a cartilagenous autograft has been employed according to the nature of the deformity. The mean follow-up was two and a half year. The results confirm the high rehability of cartilagenous grafts in the treatment of partial deformities. Treatment of total saddle deformities proved to be less satisfactory with spontaneous resorption of two grafts out of six, one of which was a homologue bony graft from the bank.

Gated magnetic resonance imaging of metastatic intracardiac malignant fibrous histiocytoma.

We report a patient with right intraventricular metastatic sarcoma who underwent gated magnetic resonance (MR) imaging. The study provided good images of the tumor, though definition was degraded by signal from slow flowing blood. The demonstration of cardiac anatomy was excellent, and this was helpful in planning subsequent radiotherapy. We conclude that gated MR imaging is a valuable supplement to two-dimensional echocardiography in the diagnosis and management of cardiac metastases. This case also illustrates that extensive intracardiac disease can be present in the absence of marked clinical symptoms.

[Role of the free forearm flap in reconstruction after total circular pharyngolaryngectomy. Review of the literature and a case report].

Methods of surgical repair after total pharyngo-laryngectomy have considerably improved since the introduction of micro-surgical techniques. The use of free autografts for reconstruction of the pharynx is now preferred in most centers to the use of musculo-cutaneous flaps. The free forearm flaps appears to be particularly suitable for this type of repair. The authors report on their experience in performing this procedure with one of their patients. The results of this technique in major published series are then compared with those achieved with similar procedures employing free jejunal interposition grafts.

The isopropoxyacetic group for convenient base protection during solid-support synthesis of oligodeoxyribonucleotides and their triester analogs.

Isopropoxyacetic anhydride was successfully used for protection of exoaminofunctions of 2'-deoxyadenosine, -guanosine and -cytidine. N-isopropoxyacetylated nucleosides are stable under the conditions of the synthesis of oligodeoxyribonucleotides on the solid support. Removal of N-isopropoxyacetyl is much faster than that of commonly used benzoyl or isobutyryl groups viz. it is completed within the operation of cleavage of the oligodeoxyribonucleotide from the solid support. This observation enabled synthesis of -OCH2CH3 and -OCH2CF3 triesters, which hydrolyse partially or completely when standard deprotection conditions are applied.

Complex structural behavior of oligopurine-oligopyrimidine sequence cloned within the supercoiled plasmid.

Synthetic sequence GATCC(AG)7ATCG(AT)4CG(AG)7 was cloned into plasmid and its structural behavior under the influence of supercoiling was analysed by chemical modification at variety of experimental conditions. It was found that this sequence adopts at least two different non-B conformations depending on -delta and pH values. Moreover, 12 nucleotide long non-pur.pyr spacer region separating two identical (AG)7 blocks does not provide a significant energy barrier protecting against unusual structures formation.

Diastereomers of thymidine 3'-O-(methanephosphonothioate): synthesis, absolute configuration and reaction with 3'-methoxyacetylthymidine under conditions of triester approach to oligonucleotide synthesis.

Diastereomers of the title compound were obtained and absolute configuration was assigned by means of stereochemical correlation. Their reaction with 3'-O-methoxyacetylthymidine in the presence of triisopropylbenzenesulphonyl (4-nitro) triazole is neither chemo- nor stereo-selective and leads to diastereomeric pairs of dithymidyl (3',5')methanephosphonate and -methanephosphonothioate. Obtained results are discussed in terms of mechanism of activation of phosphodiesters under conditions known as "phosphotriester approach to oligonucleotide synthesis".

Antibodies against nuclear components in schistosomiasis. Results compared to values in patients with rheumatoid arthritis, systemic lupus erythematosus, and osteoarthrosis.

Occurrence of autoantibodies against nuclear material was compared in groups of patients with rheumatoid arthritis (RA n = 22), systemic lupus erythematosus (SLE n = 24), osteoarthrosis (OA n = 25), and chronic schistosomiasis mansoni (CSM n = 28). Anti-ds DNA antibody was detected by an ammonium sulphate precipitation radioimmunoassay antibodies against extractable nuclear antigen (ENA) were detected and differentiated in RNAse-resistant and RNAse-sensitive components (Sm and RNP antigens) with an ELISA technique. IgG organ-non-specific and granulocyte-specific antinuclear antibodies (ANA) were detected by immunofluorescence technique with quantitative titration of positive reactions and determination of complement-fixing properties. The results in groups of patients with SLE, RA and OA were of confirmative nature and supported that the different methods detect different systems of autoantibodies and nuclear autoantigens. In CSM it was demonstrated that 23 of 28 cases had positive reactions to the RNAse-resistant part of ENA (the Sm-antigen), a significant difference from the three other groups of patients (P less than 0.001). The antibody was in all cases of IgM class, in seven cases also of IgA class. Antibodies against nuclear material in CSM are probably a consequence of heavy disturbance of the immune system in this chronic infection with great permanent antigen load. It is a matter of discussion, whether production of these antibodies is induced by nuclear material from the host or from the parasite.

Abortion in mice induced by intravenous injections of antibodies to type IV collagen or laminin.

Purified antibodies to laminin or Type IV collagen administered intravenously to pregnant mice were found to localize in the basement membranes of all maternal tissues as well as the parietal and visceral yolk sacs and trophoblast basement membranes but not in embryonic tissues. Antibodies to Type IV collagen induced a higher incidence of abortions, retroplacental hematomas and fetal deaths. When administered intraamniotically, both antiserums were embryotoxic. The functional consequences of the attachment of antibodies to these specific basement membrane antigens appear to be hemorrhage within the parietal and visceral yolk sacs and separation of fetal from maternal tissues. Complement activation appears to play an important role in the interruptions of pregnancy, because this was not observed in strains of mice lacking C5, the fifth component of complement, in mice depleted of C3 by administration of cobra venom factor, or in mice injected with the F(ab) fragments of antibody to Type IV collagen or laminin.

[Repair of soft palate deficiencies in adults by Orticochea's method (author's transl)].

The authors describe their experience of an elegant technique described by Orticochea in 1968, and which consists of conducting a transposition of the posterior palatine pillars onto a posterior pharyngeal flap, closing the cleft laterally, and thus producing a dynamic pharyngoplasty. This method was applied in 6 exceptional cases with very severe sequelae from clefts or injuries, and results were assessed as being of good quality.

[Treatment of a compound fracture of the mandible with a comminuted compound fracture of the maxilla (author's transl)].

A case is reported of a comminuted compound facial fracture associating a multisplintered comminuted compound fracture of the right palate, with a quadrifocal fracture of the mandible including severe displacement of fragments of a bi-subcondylar fracture. Treatment was difficult and consisted of producing a stable solid osteosynthesis of the body of the mandible allowing rapid extra-oral symphysial traction to reduce the condylar fractures for 3 weeks. Hospitalization lasted 33 days and the final buccal opening was 34 mm, with satisfactory dental articulation. The authors emphasize the need for a solid, stable osteosynthesis of the body of the mandible by using miniaturized plates serewed into the splintered facial complexes.
