Determination of alanine, lactate, pyruvate, beta-hydroxybutyrate, and acetoacetate by flow injection analysis (FIA).

The flow infection analysis (FIA) described by Ruzicka and Hansen was adjusted for lactic acid determination by Rydevik et al. We were able to elaborate some other NAD or NADH-dependent enzyme reactions with the FIA system. The reliability of the alanine assay corresponds to that of the lactate FIA method. The coefficient of variation was on an average 2.8%; th sample rate was 60/h with consistent duplicates. The beta-OHB assay had almost the same validity. Pyruvate, however, was less reliable with higher coefficients of variation. ACAC FIA assay, which was suitable for the determination in acid solution after neutralization, could not yet be employed for the determination in serum. In comparison to the manual enzyme methods, the FIA assays described here indicated a higher sampling rate, a lower reagent consumption, a lower coefficient of variation, a better reproducibility, and a greater consistency of duplicates.

Survival data of Israeli Jewish patients with squamous cell carcinoma of the cervix treated by irradiation.

During a 9-year period 61 Israeli Jewish Patients with histologically confirmed invasive squamous cell carcinoma of the uterine cervix (SUC) were treated by irradiation alone. A large proportion of the patients (44.2%) were more than 60 years old and only 27.9% were diagnosed as having stage IB disease. This is attributed to low awareness and infrequent cytologic screening. There is a trend for a large proportion of stage IB patients among those younger than 59 compared to those older than 60 years. The overall 5-year survival rate was 52.2% and the survival of patients with stage IB (72.6%) was significantly higher than in more advanced stages. In addition to clinical stage, age at diagnosis and method of irradiation also influenced survival rates.

Symposium on the teaching of psychiatry. USA: Michigan State University. Coming to grips with the psychosocial.

Problems in psychiatric and psychosocial education arise from the absence of agreement on curricular content, failure to emphasize observable skill development, and the ambivalent views on psychosocial material found in modern medicine. The curriculum for medical students presented here emphasizes an integrated approach to teaching psychiatry and psychosocial material. Great emphasis is placed on interviewing and history-taking as well as on problem-solving. Specific courses in development, psychiatry and ageing are prescribed. Preclinical education in psychiatry emphasizes a bio-psychosocial view of mental disorder.

Prophylactic low-dose post-operative orthovoltage radiotherapy in stage II breast cancer.

Between the years 1974 and the beginning of 1977, 199 patients with stage II infiltrating duct carcinoma of the breast were treated with post-operative radiation, using a 250 kVp X-ray machine. Three treatment schedules were used consisting of 22, 26.2 and 29.6 Gy (2200, 2620 and 2960 rad) total tumour dose, calculated at 2 cm depth, with 5, 3 and 3 fractions a week respectively. The frequency of local recurrences, mainly in the chest wall, were 20.3%, 8.3% and 8.1% respectively. The latter two frequencies were not different from those reported for the commonly used high dose treatments of 45-60 Gy (4500-6000 rad) delivered either as given doses or tumour doses. An approximate 70% five year survival rate was similar in the three groups and does not differ from results observed with high doses. Low dose treatment is advantageous, because it has fewer complications and it can be delivered in a smaller number of sessions, making it more convenient to the patient, without loss of efficacy. The results presented of the low dose treatment may reflect the existence of an optimal dose level of radiation.

Demonstration of myoglobin in formalin-fixed renal sections by immunoperoxidase technic.

The use of an immunohistochemical technic offers a very sensitive method to detect myoglobin in formalin-fixed, paraffin-embedded tissues. Thus, the technic is suitable for retrospective studies. Myoglobin can be demonstrated at the brush border and apical cytoplasm of the tubular epithelium of proximal tubules. Granular substances and casts in the lumens also react positively with antimyoglobin. Our findings confirm the results of a previous animal study, showing that myoglobin is filtered through glomeruli and reabsorbed by the proximal tubules.

Exposure to radioiodine in the preconception and conception periods. A case report.

A case of radiation exposure shortly before conception is presented. The patient refused abortion and was delivered of a healthy infant. On reconsideration of the case, it was decided that the recommendation to terminate the pregnancy on the grounds of excessive exposure to radiation was unjustified. We feel that physicians should be aware that abortion in such cases is not always necessary.
