Laparoscopic sterilization with therapeutic abortion versus sterilization or abortion alone.

Records of 606 therapeutic abortions performed by suction curettage, 693 interval laparoscopic sterilizations, and 442 combined procedures were analyzed and compared. The following conclusions were obtained: 1) Abortion alone and abortion with laparoscopic sterilization are similar in rates of postoperative morbidity (4.3 and 7%, respectively) and early complications (3.3 and 2.9%, respectively). 2) The risks in laparoscopic sterilization alone are significantly lower for postoperative morbidity (2.2%) and early complications (0.9%) than for abortion alone or for the combined procedure.

[Epidemiological aspects of idiopathic early birth (author's transl)].

Epidemiological data of 338 preterm deliveries (excluding multiple gestations) were compared with the same number of term deliveries. Both groups were matched for age and parity. In one third of the cases only, possible organic factors could be attented to preterm delivery. In the remaining patients the etiology was unknown e.g. "idiopathic". These results confirm the difficulty to identify the patient at risk for preterm delivery, especially in the nullipara.

[Folic acid and pregnancy, a real problem?].

The efficacy of prophylactic treatment with ferrous sulfate (80 mg Fe++) and the combination of ferrous sulfate with folic acid (80 mg Fe++ and 350 microgram folic acid) were studied in a double blind trial in the 20th, and again in the 30th and 36th week of pregnancy (29 patients). At the beginning of the treatment (20th week) 29% of the patients showed a pathologically reduced serum folic acid level. Combined therapy has a favorable effect on the folic acid levels in the serum and red blood cells; this was confirmed statistically. At the end of pregnancy, 69% of the patients of the control group (ferrous sulfate) showed a pathologically reduced serum level and 23% a subnormal level in erythrocytes. These results unquestionably justify the prophylactic prescription of folic acid during pregnancy.

Skin distribution of fipronil by microautoradiography following topical administration to the beagle dog.

To investigate the localisation of fipronil in dog skin, [14C]-fipronil was topically applied to a male beagle dog (spot-on administration) at the therapeutic dose of 10 mg/kg. By means of autohistoradiography, the radioactivity was precisely detected in the skin and appendages at various intervals after application. Radioactivity was predominantly observed within the stratum corneum, the viable epidermis, and in the pilo-sebaceous units (mainly in the sebaceous glands and epithelial layers). [14C]-fipronil was significantly detected in these structures up to 56 days post-treatment, in the application zone (neck) but also in the lumbar zone, thus indicating the mechanical displacement of fipronil. No radioactivity was detected in either the dermal or the hypodermal layers, confirming the low percutaneous passage of fipronil.

Increments toward what?

Incremental proposals to expand health insurance coverage, such as expansions of the State Children's Health Insurance Program (SCHIP) or creation of new tax credits, should be examined for the values that underlie them and for how they structure future options for additional incremental coverage expansions. This paper examines five design issues in incremental reform: who determines coverage options for the newly insured; what risk pool do they enter; what is the government's contribution toward their coverage; what barriers are created by efforts to encourage efficiency; and how are issues of federalism handled? Tax credits are a departure from past approaches, while an SCHIP expansion is a continuation of current policy directions.
