Azygos vein laceration following a vertical deceleration injury.

A 41-year-old man presented with respiratory distress and hypotension after a 30-foot fall from a tree. Despite fluid resuscitation, the patient expired in the operating room. Autopsy revealed an azygos vein laceration at the junction of the superior vena cava as the cause of death. Azygos vein and superior vena caval lacerations are rare following blunt chest trauma, including vertical deceleration injury. Early suspicion of vascular injuries with aggressive fluid resuscitation and surgical intervention remain the only hope for survival from this highly lethal injury.

Parental attachment, drug use, and facultative sexual strategies.

It is often asserted that sexual promiscuity and drug abuse appears to share a common etiology in poor parental attachment. This study explores this claim empirically among 480 college students. Other variables--religiosity, masculinity/femininity, sex, age, and physical appearance--that may enhance or reduce the incidence of promiscuity and drug use were included in multivariate analyses. Parental attachment was significantly related to both variables, and the combination of poor parental attachment and drug use was a strong predictor of promiscuity for both males and females. In multivariate analyses, religiosity was the most important predictor of promiscuity for males, and attachment was the most important for females. The findings are examined guided by the three desiderata commonly accepted as relevant to biosocial attachment theory.

Living kidney donor experiences: implications for counselling.

This study adds to previous, mostly quantitative, investigation into the experiences of living related kidney donors. Such investigation is important so that potential donors are supported effectively and donation programmes remain relevant and specific to need. Exploration takes place into donor decision-making processes and the most effective forms of professional support. A non-probability sampling technique highlighted eight living related kidney donors who were interviewed using a semi-structured interview format. Raw data was analysed through the qualitative technique of Interpretative Phenomenological Analysis. The decision to donate is made rapidly, decisively and rationally. Professional support provides reassurance to donors, particularly when experiencing acute psychological reaction. The need to provide support to the parents of living donors is specifically highlighted. A comprehensive range of Master Themes are generated through Interpretative Phenomenological Analysis and these reflect the complexity of the donation experience. This suggests donors are concerned with the management of psychological experience rather than with reviewing the appropriateness of an original decision to donate. Evidence indicates that concepts of attitude and self-efficacy belief can develop understanding of the psychological experience of being a living kidney donor. A counselling perspective, with Social Cognitive Theory at its core, is highlighted as a valid method for providing professional support to donors before and after surgery.

Probucol decreases apolipoprotein A-I transport rate and increases high density lipoprotein cholesteryl ester fractional catabolic rate in control and human apolipoprotein A-I transgenic mice.

Probucol effects on lipoprotein metabolism were determined in control and human apolipoprotein A-I transgenic (HuAITg) mice. In control mice, probucol reduced total cholesterol from 67 +/- 2 to 25 +/- 2 mg/dl by reducing high density lipoprotein (HDL) cholesterol from 46 +/- 20 to 14 +/- 1 mg/dl and low density lipoprotein (LDL) cholesterol from 11 +/- 1 to 5 +/- 1 mg/dl. Apolipoprotein (apo) A-I levels were reduced from 122 +/- 8 to 56 +/- 5 mg/dl. In HuAITg mice, probucol reduced total cholesterol from 121 +/- 5 to 77 +/- 3 mg/dl by reducing HDL cholesterol from 84 +/- 4 to 56 +/- 3 mg/dl and LDL cholesterol from 19 +/- 2 to 11 +/- 2 mg/dl. Human apo A-I levels were reduced from 267 +/- 13 to 144 +/- 12 mg/dl and mouse apo A-I levels from 18 +/- 2 to 9 +/- 2 mg/dl. Control animals have primarily a monodisperse HDL with a particle diameter of 10 nm. Probucol did not appear to change the particle size distribution in the control animals. The HuAITg mice have a polydisperse HDL with particle diameters of 10.1 and 8.5 nm. Probucol treatment of these animals resulted in HDL with particle diameters of 9.4 and 8.5 nm, apparently reducing the size of the larger particles. In vivo turnover studies revealed that the reduction in apo A-I was primarily due to a decrease in transport rate, whereas the reduction in HDL cholesterol was primarily due to an increase in HDL cholesteryl ester fractional catabolic rate.(ABSTRACT TRUNCATED AT 250 WORDS)

Practice development units: a study of teamwork.

In this report of a study using the Team Climate Inventory (TCI) tool, the researchers explain how the tool can be used in preparation for creating a practice development unit (PDU). The TCI provides a picture of the level and quality of teamwork in a unit using a series of Likert scales. The ward in this study was found to lack the necessary level of teamwork for successful PDU development and the researchers show how this information shaped trust plans. They recommend that units contemplating PDU accreditation should assess their level of teamwork prior to proceeding with bids.

Evidence-based practice: is patient satisfaction evidence?

Patient satisfaction is a popular way of evaluating nursing practice, but there are doubts about its reliability and validity. This article describes a study which assessed the usefulness of one tool for measuring patient satisfaction--the Newcastle Satisfaction with Nursing Scale.
