Residence time distributions of solutes in the perfused rat liver using a dispersion model of hepatic elimination: 2. Effect of pharmacological agents, retrograde perfusions, and enzyme inhibition on evans blue, sucrose, water, and taurocholate.

The effect of altered physiological conditions on the residence time distributions of sucrose, water, and taurocholate in the rat liver were studied using a bolus injection and quantifying fraction of total outflow per ml-time profiles. Retrograde perfusions increased the residence times of sucrose and water markedly and were associated with very low hepatic availabilities for taurocholate. Resistance by the inlet sinusoids sphincters, which become outlet sphincters during retrograde perfusions, is suggested as the explanation for the observation. Infusions of noradrenaline, propranolol, and lidocaine resulted in relatively small changes in the mean residence times for sucrose and water with no apparent relationship existing between the efficiency number of taurocholate and volumes of either water or sucrose. Taurochenodeoxycholate resulted in an increase in the availability and mean residence time for taurocholate relative to no infusion.

Exercise capacity, hemodynamic, and neurohumoral changes following acute and chronic administration of flosequinan in chronic congestive heart failure.

We evaluated the responses to 90 minutes and 8 days of therapy with a new long-acting vasodilator flosequinan in ten patients with moderate chronic congestive heart failure in an open, uncontrolled study. Acute administration of 100 mg orally resulted in a decrease of preload, with a reduction of left ventricular end-diastolic volume, left ventricular end-diastolic pressure, pulmonary capillary wedge pressure, and right atrial pressure. Following the acute administration, we found no significant changes of heart rate, cardiac index, stroke volume, peripheral vascular resistance, ejection fraction, and dp/dt. Chronic application for 8 days (100 mg/day) showed persistent effects on preload, with a significant decrease of pulmonary capillary wedge pressure, right atrial pressure, and pulmonary arterial pressure. After 8 days of treatment, cardiac index was significantly increased from 2.2 +/- 0.2 l/min/m2 to 2.8 +/- 0.2 l/min/m2 (p = 0.013) and stroke volume from 57 +/- 10 ml to 74 +/- 9 ml (p = 0.022). Peripheral vascular resistance decreased by ml (p = 0.022). Peripheral vascular resistance decreased by 28%. After 8 days, bicycle exercise capacity increased significantly from 383 +/- 44 sec to 422 +/- 43 sec (p = 0.01) and the patients were able to increase their walking distance over a 6-minute exercise test from 426 +/- 46 m to 477 +/- 33 m (p = 0.007), with a concomitant decrease of dyspnea (p = 0.013). Plasma renin concentration showed only a rise 90 minutes after the acute administration on day 8 of the study, and atrial natriuretic peptide and 6-keto-prostaglandin F1-alpha decreased significantly.(ABSTRACT TRUNCATED AT 250 WORDS)

[Importance of DNA-cytometric parameters in the prediction of blast crisis in the course of chronic myelogenous leukemia].

In a retrospective study the DNA-cytometric parameters stemline ploidy, stemline shoulder fraction and proliferative fraction were followed during the course of the disease in 20 patients with chronic myelogenous leukemia. Stemline ploidy and stemline shoulder fraction significantly increased whereas the proliferative fraction steadily decreased during the disease most of these changes taking place during chronic phase before the clinical onset of blast crisis. Prognostically relevant cutpoints indicating disease transformation during the next 12 months could be defined.

[Relation between electrical and mechanical muscle activity in myotonia].

The relationship between electrical and mechanical muscle activity was investigated in the case of twenty patients with myotonia congenita. Statistically significant correlations were found to exist between the myotonic discharge spasm and the phases of tension and relaxation in the mechanogram. In vitro examination for 9-anthracene carbonic acid of individual muscle fiber bundles and the examination of a patient with recessive generalised myotonia revealed that the incidence of myotonic discharge spasms is accompanied by development of tension in the muscle. These findings lend support to the assumption that in both forms of myotonia it is the electrical activity that induces delay of contraction, and thus accounts for the pathological muscle behavior in myotonia.

[Primary sclerosing cholangitis in isolated IgA deficiency].

Primary sclerosing cholangitis was diagnosed in a patient with isolated IgA deficiency. Similar reports in the literature suggest that the two conditions are related. Patients with primary sclerosing cholangitis associated with isolated IgA deficiency are distinguished by the fact that it is mainly the intrahepatic bile ducts that are narrowed by the fibrosing process. The disease manifests itself comparatively early. Recurrent bouts of fever, in combination with elevated AP, can become the leading symptom even before the onset of jaundice. The prognosis seems somewhat more favourable than in other patients with primary sclerosing cholangitis.

[Prolonged skin reactions after myelography].

Lumbar myelography was undertaken in a man aged 28 years. Six hours after the myelography, he developed a papular skin rash which became generalized during the subsequent 36 hours. During the subsequent 16 days, the skin rash remitted completely. The incident is interpreted as a non-allergic adverse reaction to intrathecal iohexol.

Mutants of luminous bacteria selected for bioluminescent toxicity tests.

Mutants of the luminescent bacterial strain NRRL B-11177 were isolated with pleiotropic hypersensitivity towards hydrophobic antimicrobial agents. SDS-PAGE analyses of outer membrane proteins and lipopolysaccharides revealed that the outer membrane structure of the ahs-mutants was altered. QSAR analysis showed that the inhibitory effect of chloro-substituted phenols on bioluminescence of the ahs-mutants depended on their hydrophobicity. The effect of chlorinated phenols and detergents on bioluminescence was increased in the ahs-mutants. The potential use of these mutants in bioluminescent toxicity tests was discussed.

[The radioecology of the grapevine. 1. The transfer of nuclear weapons fallout from the soil into wine].

In a field investigation (1983-1985) comprising eight places of the most important viticultural regions in the Federal Republic of Germany, the contents of the radionuclides tritium (3H), carbon-14 (14C), strontium-90 (90Sr), and cesium-137 (137Cs) in air, soils, leaves of the vine, grapes and wine were measured and site-specific transfer factors were calculated. Data concerning soil parameters, climatic conditions, cultivation and vinification were collected. The tritium content of all samples was 10 Bq/l water of combustion, independent of location and year. The specific activity of 14C in the atmosphere and in biological material was 0.22 Bq/g carbon, independent of site and year. 90Sr contents of soils fluctuated between 0.7 and 3.5 Bq/kg dry matter. The mean content of leaves was 2 Bq/kg fresh material, of grapes 0.035 Bq/kg and of wine 0.008 Bq/l. 137Cs content of soils fluctuated between 1.3 and 7.9 Bq/kg dry matter. The mean content of leaves was 0.098 Bq/kg fresh material, of grapes 0.021 Bq/kg and of wine 0.0085 Bq/l. A relation between transfer of radionuclides and soil parameters and between the contents of grapes and wine was not recognizable. While cultivar-specific differences were not observed in grapes, red wines contained somewhat more 137Cs than white wines. Transfer factors soil grapes were 0.027 for 90Sr and 0.0057 for 137Cs. Site-specific influences such as soil parameters, climate, cultivation, vinification and differences between years led to a relatively small fluctuation of values. An influence of the nuclear power station Neckarwestheim has not been found in any of the radionuclides.

[Animal experiment studies of the effect of the hazardous substance carbon tetrachloride on peripheral nerves].

The deleterious effects of Carbon Tetrachloride, widely used industrially, on the peripheral nervous system was investigated by experimentation on rabbits. Long-term infusion simulated various conditions of exposure to obtain pharmacological and electroneurographic data. Tetrachlorethen is shown to induce reversible dysfunctions of the peripheral nervous system depending on the degree of concentration. The mode of molecular action remains open. Our results are compared with references in existing literature.

[Development of a rational monitoring strategy for workers exposed to tetrachloroethylene].

A 5 year follow-up of 130 workers exposed to tetrachlorethylene (dry cleaning) was performed, including measuring of exposure conditions (personal dosimetry, exposure tests) and clinical investigations. No signs of hepatoxicity of tetrachlorethylene could be detected, but slight derangements of nerval functions may develop in long-term exposed workers. Nerve motor and sensory conduction velocity shows a tendency to diminution. Recommendations are derived for biological monitoring and health surveillance of workers exposed to tetrachlorethylene.

[Facioscapulohumeral neuromuscular syndromes--problems of differential and early diagnosis and genetic counseling].

Among 20 kinships affected by facioscapulohumeral and scapulohumeroperoneal neuromuscular syndromes 6 probands with autosomal dominant transmission and 5 obviously sporadic cases revealed myopathic changes on biopsy and were classified to have facioscapulohumer (operone)al muscle dystrophies. 5 autosomal dominant cases and one sporadic case with neurogenic muscle changes and 3 other non-dystrophic phenocopies emphasize the necessity of muscle biopsy for nosological assignment. Definite correlations between mode of inheritance, clinical and morphological findings could not be found. Pathological ultrasound and EMG findings in parents and children of seemingly sporadic cases suggest to assume autosomal dominant inheritance with incomplete penterance in such cases.

[Maternal behavior toward her newborn infant. Potential modification by peridural analgesia or childbirth preparation].

The effects of sophrology and epidural analgesia on early relationship between the mother and her child were studied on a simple of 190 deliveries. The mothers were observed during and just after delivery. Mothers who had been separated from their child before the end of the observation were excluded from the study. The patients had the choice between epidural analgesia or prenatal care with sophrology. Participation to prenatal courses has statistically a positive effect on the relation between the mother and her child (p less than 0.01). Instead, epidural analgesia and posture have very limited effect on this factor. However, a trend to more interaction is found in multipari and patients who didn't choose epidural analgesia.

Pigmentary changes in acquired retinoschisis.

We reviewed retrospectively the records of patients referred for retinal examination and found 375 eyes (233 patients) with acquired retinoschisis. Of the 375 eyes, 85 had outer layer retinal breaks, 27 had retinal detachments, and 29 had pigmentary lines. Twenty-five of the 29 pigmentary lines (86.2%) demarcated either retinal detachment or outer retinal layer breaks.

[Proliferative vitreoretinopathy: relation to the extent of retinal detachment, size of retinal tears and coagulation surface].

Preretinal dispersion of stimulated RPE cells has been suggested as a possible cause of the development of proliferative vitreoretinopathy (PVR). Retinal detachment (RD) and retinal breaks are associated with an exposure of corresponding areas of retinal pigment epithelium (RPE) which has been mechanically stimulated by coagulation. In a prospective, controlled study of 141 patients between 20 and 75 years of age, PVR grades were determined before and RD surgery and within six months following it PVR grades were correlated with the extent of RD, size of breaks, and area of coagulation. The severity of the PVR correlated significantly with both the extent of RD as well as the total number of coagulation exposures. There was a tendency toward higher grades of PVR with larger retinal breaks. The results are compatible with the hypothesis that at least certain variations of PVR are caused by preretinal dispersion of stimulated RPE cells.
