[Pathological anatomy of a herpes infection caused by herpes simplex type 2 virus in infants].

Materials of autopsies of 52 children suffering from severe herpetic infection, intrauterine as a rule, were analysed. The etiology of the process was proved by immunofluorescent and serological examinations. The disease was markedly generalized, however, with predominant involvement of the liver, lungs, and central nervous system of the type of pneumonia, hepatitis and encephalomyelitis. Early in the disease, changes of alveolocytes, hepatocytes, and other epitheliocytes were observed, consisting primarily in a peculiar alteration of their nuclei. Simultaneously, moderate signs of inflammation were observed, but in most severe cases it was alterative.

[Intrauterine infections in the perinatal period].

The results of examinations of autopsy materials in Leningrad and the data from the literature indicate that infectious diseases occupy a significant place in perinatal mortality. It is emphasized that if the autopsy materials are not sufficiently examined using not only light but also immunofluorescent microscopy, a considerable portion of these infections is not diagnosed and the pathological proprocess in such cases in considered to be aspiration pneumonia, hyaline membrane disease, etc. The paper briefly characterizes intrauterine bacterial infections (listeriasis, syphilis, staphylococcal infections, etc.) with special reference to virus diseases (cytomegaly, herpes infection, lesions caused by respiratory viruses, etc) and mycoplasmosis. It is shown that a relatively high incidence of lesions is caused by respiratory viruses and particularly mycoplasma. The possibility of protozoan and mycotic lesions is also indicated. The frequent occurrence of combined intrauterine infections is emphasized.

[Lung surface tension in acute respiratory infections].

Experiments in 9 dogs showed that 24 hours after death there was only an insignificant decrease in the activity of surfactant which remained within normal limits. Similar data were obtained in examinations of the lungs of 14 children dying without respiratory pathology. In the main part of the study lungs from 20 children and 1 fetus dying with various respiratory infections were examined. It was found that the greatest effect on the decrease of formation of the surfactant was exerted by crude damage of the epithelium of distal parts of the respiratory ways. No relationship of this phenomenon to the etiology of the process was established, although there was a trend for retention of the normal surfactant activity in mycoplasmal and mycoplasmal-bacterial involvement of the lungs, whereas in viral and viral-bacterial processes the surfactant activity was more frequently decreased. This, however, seemed to be due to differences in the predominant localization of the process.

[Problem of toxoplasmosis].

The paper presents brief characteristics of the causative agent and methods of its detection in tissues. The time course of the development of morphological changes in the central nervous system (CNS) is characterized on the basis of the authors own studies. In particular, the possibility of formation of congenital CNS defect upon intrauterine infection with toxoplasma has been demonstrated. A direct correlation between the results of morphological and biochemical examinations of the CNS has been found. A case of toxoplasmosis developing postnatally as a result of exogenous infection with a description of the primary focus in the lungs is presented.

[Intrauterine mycoplasmosis].

Comparative studies of pathological anatomy of intrauterine mycoplasmosis of man and cattle were carried out. On the basis of investigations of 22 deceased newborns and of 3 stillborns with uterine infection caused by Mycoplasma pneumonia descriptions of lesions of various organs are presented. They most regularly were defected in the liver. In the respiratory organs in this infection, in contrast to respiratory mycoplasmosis, lesions of respiratory departments, not of the respiratory tract, were predominantly observed. The investigation of 18 liveborn and 5 stillborn calfs with intrauterine infection caused by Mycoplasma bovirhinis revealed the same regularities of the pathological process. The localisation of lesions and the presence of changes in the placenta justify the conclusion that the disease developed, as a rule, as a result of hematogenic transplacental infection.

[Role of viral-bacterial associations in meningitis in children].

A total of 50 section cases of meningitis in children were investigated. All patients revealed acute viral respiratory infections with generalization, including lesions of the central nervous system (CNS). In 14 children moreover meningococcal infection was diagnosed. In 30 children lesions of the CNS were due to mixed bacterial microflora. In 6 children along with acute viral respiratory infections (AVRI) mycoplasmosis was also revealed. Etiology of the process was determined on the basis of characteristic structural changes in the CNS and other organs, findings of virological and bacteriological investigations. In the majority of children the intravascular blood coagulation was observed. Waterhouse-Friderichsen's syndrome was revealed mainly in meningococcal infection. In order to ascertain the data obtained case records of 120 children, who had undergone treatment in connection with meningococcal infection, were analysed. It turned out that all 42 children, who had developed this disease independently, recovered. Combination of the disease with AVRI led to fatal outcomes in 12 cases out of 72.

[Intestinal localization of shigellae in dysentery and experimental shigellosis].

The investigation of 53 section cases of dysentery and 18 biopsy specimens of the large intestine in acute dysentery in children showed that intrapithelial distribution of Shigella was not always regular. In studies of experimental Shigellosis on isolated loops of the intestine 459 segments of the intestine of 51 rabbits were investigated. The results obtained showed that Shigella multiplied mainly in the lumen of the intestine. The increase in number of Shigella in enterocytes was by 8-9 orders lower than that in the content of the ligated segment, whereas the number of enterocytes with Shigella was many times less than the total number of damaged cells. This confirms the conclusion that intraepithelial multiplication of Shigella is of no considerable significance in pathogenesis of dysentery.
