Reappraisal of preoperative carcinoembryonic antigen levels as a prognostic factor in resectable colorectal cancer.

Preoperative carcinoembryonic antigen levels in sera were measured in 124 cases of primary colorectal cancer, and their usefulness as a prognostic factor were evaluated. There were no significant differences in the CEA levels with regard to age, sex, size of the primary tumor, histological differentiation, peritoneal dissemination or DNA ploidy, but CEA levels were significantly higher in patients with liver metastasis. By using Cox proportional hazard model, the most significant discriminant in prognosis was liver metastasis, followed by preoperative CEA level, depth of invasion and DNA ploidy pattern. Preoperative CEA measurements were applied as a parameter of biological behavior of colorectal cancer in the follow-up course.

[Diagnosis in early breast cancer].

To improve the survival in breast cancer, it is important to detect and treat breast cancer early. Physical examination (PE), mammography (MMG), ultrasonography (US), and aspiration biopsy cytology (ABC) are routinely assessed in the diagnosis of breast cancer. When three procedures including ABC are positive in a diagnosis of breast cancer, it is an absolute indication for surgery. Of all 238 breast cancers between 1987 and 1992, 86 cases were T1. The sensitivity of each procedure was PE 58.9%, MMG 70.0%, US 74.4% and ABC 86.2%, indicating that PE was less sensitive for small cancer. The recent accuracy of ABC in our series was satisfactory, and the combination diagnosis had a good result for T1 tumor because the findings from these procedures were complimentary. The high accuracy of ABC therefore suggests that lumpectomy has two aspects, definite diagnosis, and therapy as conservative surgery for breast cancer. The nineteen cases of T0 cancer encountered were almost entirely detected by nipple discharge or MMG. In conclusion, the combination diagnosis including ABC was very effective, and it is very important to detect and diagnose clinically small or non-palpable breast cancer.

[Clinical significance of flow cytometric DNA analysis in metastatic lymph node of colorectal cancer].

Significance of flow cytometric DNA analysis in metastatic lymph nodes for assessing malignant potential of colorectal cancer was investigated using paraffin-embedded materials of primary lesions and metastatic lymph nodes from 65 patients who had been treated between 1975 and 1990. The DNA ploidy patterns of metastatic nodes were identical in 61.5% with those of primary lesions. Diploid cancers were significantly more frequent in metastatic nodes than in primary lesions. There were significantly more aneuploid cancers in proximal nodes than in distant nodes. There was no relation between ploidy patterns in primary lesions and survival. However, a significant relation was found between ploidy patterns in metastatic nodes and survival. Diploid cancer in metastatic nodes had a significantly better survival than aneuploid cancer, in all patients as well as those with curative resection. In patients with stage III and in those with the same depth of invasion, the survival rate of diploid cancer in metastatic nodes was significantly higher. There was no correlation between ploidy patterns in metastatic nodes and clinicopathological variables in primary lesions, such as histological type, depth of invasion, nodal involvement, peritoneal or hepatic involvement and stage. These results suggest that nuclear DNA content in metastatic lymph nodes may be a prognostic indicator in colorectal cancer with nodal involvement.

Giant adenoma of the parathyroid: report of a case.

We report herein the case of a 56-year-old woman who was diagnosed as having a parathyroid tumor after she presented with hip joint pain and gait disturbance. She underwent surgery and the resected tumor, which weighed approximately 70 g, was found to be an unusually large adenoma of the parathyroid, detected on the basis of clinical findings and dual-tracer parathyroid and CT images.

Immunohistochemical overexpression of C-erbB-2 in the prognosis of breast cancer.

Immunohistochemical c-erbB-2 protein overexpression was detected in 34 of 124 (27.4%) paraffin-embedded breast cancer specimens. Although no difference was seen between the c-erbB-2 positive and negative groups in 5-year disease-free survival, 5-year overall survival was significantly less favorable in the c-erbB-2 positive group. Furthermore, patients graded as having positive c-erbB-2 staining and aneuploid DNA showed significantly poorer survival than those in other categories. The significant prognostic factors, determined by a multivariate analysis, were nodal status and c-erbB-2 overexpression. Our findings therefore suggest that c-erbB-2 expression is a prognostic factor in breast cancer and that it could be useful in the determination of postoperative adjuvant therapy.

The neurosurgical management of brain metastasis from colorectal cancer: a report of three cases.

We report herein three cases of brain metastasis from primary colorectal cancer in which the metastatic lesion was resected to effectively relieve neurological symptoms. Case 1 was a 61-year-old woman with a solitary brain metastasis from colorectal cancer and no other metastases; case 2 was a 59-year-old woman who died from liver metastasis 11 months after resection of the metastatic brain tumor; and case 3 was a 68-year-old woman with multiple metastases to the lungs and bones detected before the brain metastasis. According to 16 cases previously reported in the Japanese literature and our 3 cases, the interval between diagnosis of the primary cancer and discovery of brain metastasis was 23 months on average, while the median survival after the discovery of brain metastasis was 7 months. Brain metastases with liver and lung metastases were seen more frequently than brain metastases alone. In these three cases, chemotherapy appears to have been of no use in preventing recurrence. Thus, we believe neurosurgical management to be appropriate for a solitary lesion and that it should be actively pursued to prolong survival and improve quality of life.

[Different growth rate and contracting activity of collagen gel between fibroblasts from breast cancer tissue and normal breast tissue].

We have already reported that stromal fibroblasts of breast cancer tissue (CaF) promoted the growth of breast cancer cells in vitro. It was found that CaF and fibroblasts in normal breast tissue (NF) showed the different growth and contracting activity. Materials were obtained from cancer tissue and normal breast tissue. Cells were prepared by collagenase digestion. The CaF's growth was more rapid than NF's. After a reconstitution of type I collagen, monolayer cultures of CaF and NF were done on collagen gel for 24 hours, then collagen gel was detached from the dish and was being float. After 2 days incubation, an area of collagen gel was measured. A contraction of CaF's gel was greater than that of NF. This showed that CaF had more contracting activity than NF. These results might be indicated that the function of CaF was to contribute the invasive growth of breast cancer cell as well as to secrete the growth factors. Therefore, it is considered, a control of the fibroblasts derived from cancer stroma may play an important role in the treatment of breast cancer.

[S-phase fraction and malignant potential in DNA diploid colorectal cancer].

Patients with DNA diploid tumor have a significantly longer survival than those with DNA aneuploid in colorectal cancer, but some patients with diploid tumor show poor survival. For assessing malignant potential in diploid colorectal cancer, flow cytometric analysis of S-phase fraction (SPF) was investigated using paraffin-embedded materials from 79 carcinoma patients who had been treated from 1971 to 1989. There was no significant correlation between SPF and clinicopathological factors (such as sex, tumor size, tumor location, macroscopical type, depth of invasion, node metastasis, peritoneal dissemination, liver metastasis and clinical stage). Mean SPF was significantly higher in poorly differentiated type carcinomas than in well differentiated type carcinomas. Patients with higher SPF had a tendency to poorer survival than those with lower SPF. From these results, it was concluded that SPF represents histological differentiation and that SPF may be a prognostic indicator, in diploid colorectal cancer.

Separation of closely related large peptides by micellar electrokinetic chromatography with organic modifiers.

Large peptides with similar electrophoretic mobilities were separated by micellar electrokinetic chromatography (MEKC) with organic modifiers. [Leu13]motilin and [Met13]motilin differ by only one neutral amino acid residue. Because the electrophoretic mobilities of these peptides are almost identical, these peptides were not separated by capillary zone electrophoresis (CZE). Such large peptides have not been separated by conventional MEKC either, because they interacted strongly with the micelle. However, they were completely separated by MEKC when an organic solvent was added to the micellar solution. Some insulins, larger peptides than motilin, from different origins, which have very similar electrophoretic mobilities, were also successfully separated by the same technique. The size of peptides which were separated without organic modifiers was examined.

The relationship of estrogen receptor status to DNA ploidy in breast cancer.

The relationship of estrogen receptor (ER) status to DNA ploidy was investigated in 121 patients with breast cancer who underwent surgery. Lymph node status was evaluated histologically and ER levels were determined by the dextran-coated charcoal method, with a level of 3 fmol/mg.protein being considered positive. Flow cytometric DNA content was analyzed using paraffin-embedded tissue blocks. Sixty-three percent of the specimens were ER+, while 37 percent were negative. Sixty-one patients (50.4 percent) were diploid and 60 aneuploid. A statistically significant correlation between the ER positivity rate and diploid DNA was found. Higher ER levels were seen in the postmenopausal patients with diploid tumors than in those with aneuploid tumors and there was a significant tendency for ER levels to be higher in the diploid tumors. Nodal status was not correlated with ER positivity or ploidy pattern. The present results indicate that ER levels are correlated with DNA ploidy, and reflect the degree of functional differentiation.

Temperature-gradient DNA probe chromatography of nucleic acids on nonporous supports.

Temperature-gradient DNA probe chromatography of nucleic acids on a nonporous support with a homogeneous particle size of 2.5 microns showed a higher base sequence discriminating power and a larger linear capacity than that on a porous support with a larger and less homogeneous particle size. The resolution on the nonporous support was high enough to separate samples with a single-base mismatch of the less destabilizing groups, including G-G and G-T base mismatches, even when it locates very near the end of the probe.

A novel low molecular weight factor detected in the cytosol of guinea pig neutrophils to enhance superoxide anion production.

Cytosolic low molecular components in guinea pig neutrophils were examined for the activity to enhance superoxide anion (O2-)-generating NADPH oxidase activity. A component was separated by Sephadex G-25 gel filtration from high molecular weight components, the latter revealed NADPH oxidase activity in a cell-free system in combination with the membrane fraction and arachidonic acid. Addition of this cytosolic low molecular weight component to the cell-free system significantly enhanced NADPH oxidase activity, though this component did not substitute the high molecular weight components in constituting the system. The low molecular weight NADPH oxidase activation factor (LMWAF) found here was not of protein nature, since protease treatment failed to reduce its activity. This factor did not contain phosphate, and was neither flavin nor guanine nucleotide. Though LMWAF was extractable with chloroform-methanol, it was not identical with diacylglycerol.

Flow cytometric DNA analysis in Japanese colorectal cancer. A multivariate analysis.

Using Cox's proportional hazard model, we performed a multivariate analysis of survival data from 126 patients who underwent curative resection for colorectal cancer from 1971 to 1985. Flow cytometric DNA measurements were carried out using paraffin wax-embedded tissue blocks. Fifty-four per cent of the cases were found to be aneuploid and 46% were diploid. The depth of invasion of the tumor, nodal status, liver metastasis, peritoneal dissemination, DNA ploidy pattern, histology, macroscopic classification of the tumor, age, sex and site were tested in a survival analysis. The initial seven factors were found to be the significant prognostic variables in a univariate analysis. A multivariate analysis shows liver metastasis, DNA ploidy pattern and peritoneal dissemination as being the significant discriminants of survival (P = 0.0001, 0.0022, 0.0119, in this order). Therefore, nuclear DNA ploidy pattern in colorectal cancer is considered to be an independent prognostic factor.

A case of mucosal prolapse syndrome.

A case of mucosal prolapse syndrome in the rectum is reported. A 61-year-old female presented complaining of bloody discharge and constipation for 2 months. Colonoscopy revealed an elevated lesion on the left anterior wall of the rectal canal. Biopsy showed glandular proliferation and fibromuscular obliteration of the lamina propria, findings characteristic of mucosal prolapse syndrome. It is a rare disorder affecting the rectum because the lesion looks macroscopically like a protruding adenoma.

[A case of intrascrotal neurilemmoma].

The case was in a 65-year-old male who complained of right scrotal painless swelling. Surgical excision was performed. The tumor was placed in tunica dartos but did not connect with the testis, epididymis and spermatic cord. The tumor was 2.8 x 2.0 x 1.5 cm and 10 gr. in weight. Histological diagnosis was neurilemmoma. To our knowledge, this is the first report about intrascrotal neurilemmoma in Japan.
