[Atopic dermatitis: role of oxatomide].

Oxatomide is an antiallergic molecule which acts mainly as a calcium blocker. This article is a review of oxatomide treatment in atopic dermatitis patients. First, an overview of oxatomide treatment hypersensitivity reactions in atopic dermatitis and of their supposed immunologic mechanisms. The total number of patients treated so far in published studies is too low to fully appreciate the therapeutic benefit of this molecule. However, encouraging results were reported in two controlled studies and 3 open studies originating from four different groups. Tolerance was excellent (of 200 patients evaluated in 5 studies, transient sleepiness was noted in 5, and weight gain was noted in 3 children).

[Hepatic infarction of arterial origin. X-ray computed tomographic aspects].

Unlike other major abdominal organs, infarctions affecting the liver is extremely rare. In humans, the rarity of hepatic infarction is explained commonly on the basis of the double blood supply via the hepatic artery and portal vein. The imaging studies and clinical records of seven patients with hepatic infarcts were reviewed. These infarctions were essentially observed during post-operative periods: after aortoceliac graft and after accidental ligation of the hepatic artery (4 cases). The other etiologies included: acute hypovolemic shock in patients with atherosclerosis (2 cases) and finally one case of acute thrombocytosis. Dynamic CT was performed initially in all the cases and during the evolution, demonstrating peripheral low attenuation areas without enhancement. Associated splenic infarcts were present in 4 patients and renal infarcts in two. While the clinical and laboratory findings in hepatic infarction can be completely mimicked by hepatic abscess, CT-guided aspirations were performed in 6 patients demonstrating in only one case the presence of infection.

[Analysis of bone-implant stress distribution. Finite element modeling].

A two-dimensional finite element partial mandibular model allows a first comparison between two types of titanium cylindrical implants, one full, the other hollow. The study cases correspond to a distal edentulous rehabilitated with a three-unit bridge supported mesially by a healthy second premolar and distally by one of the two concerned implants. The model includes the mandible from the canine region to the condyle, and allow to investigate the interactions between implants and bone tissue, under a normal biting load (100 N) distributed over the different teeth of the model. On a general way, the highest stresses magnitudes are in the implants, so there are no pathologic actions in bone, even if the distal cervical region of the implants are more stressed than any place else of the alveolar bone. It is well known that bone resorptions are often found in that region. Furthermore the bone trunk of the hollow implant is no more stressed than the rest of the mandibular, that is good for bone affixing. The outcome of this work, over the first results exposed, is that it gives the opportunity to understand the complexity of such a problem.

[Acute infectious cellulitis. Apropos of 28 cases].

Acute infectious cellulitis is a common condition in pediatric patients. We retrospectively studied 28 cases seen over the last four years. Average age of patients was four years. Fever and a decline in general health were often found. Streptococcus pyogenes and Staphylococcus aureus were the most common organisms in our series. Outcome was favorable under early antimicrobial therapy selected according to age and clinical features. Before three months of age, cellulitis suggests a streptococcus B infection and should be treated by ampicillin or parenteral penicillin, combined with an aminoglycoside if called for. Between 4 months and 2 years of age, Haemophilus influenzae b or a pneumococcus are the most likely organisms; Haemophilus cellulitis should be treated by a third generation cephalosporin for ten days or longer, followed by ampicillin or ampicillin-clavulanic acid per os for ten or 15 days. Beyond 2-3 years of age, all organisms may be the cause of cellulitis, but the most likely are Streptococcus pyogenes and Staphylococcus aureus, treatment consists in administration of either a penicillin M or a macrolide, the route being selected according to the general condition and site of the cellulitis.

[Renal angiomyolipoma. Diagnostic value of magnetic resonance imaging].

Owing to the wider use of ultrasonography, renal angiomyolipoma is now diagnosed with increasing frequency. Within a few months, 10 female patients were explored by ultrasounds, computerized tomography and nuclear magnetic resonance (Magniscan 5,000, 0.5 Tesla). At ultrasonography the lesion was hyperechogenic in 9 cases and heterogenic in 1 case. At computerized tomography 7 patients had a strongly hypodense lesion and 3 had a lesion of slightly positive density. At magnetic resonance imaging the lesion produced a characteristically loud signal similar to that of fatty tissues. In practice, ultrasonography and computerized tomography combined provide the diagnosis in most cases. In case of doubt, especially since the tumour may be small and asymptomatic and the lesions multiple and/or bilateral, magnetic resonance imaging with its fat-like loud signal confirms the diagnosis and leads to partial resection. When no complications are present, it may even suggest that surgery is unnecessary.

Clinical epidemiology of symptomatic primary herpetic infection in children. A study of 50 cases.

We have studied a series of 50 children with clinical primary herpetic infection during 1975-1985. Our data confirm: absence of sex differences and seasonal variations, a peak age of incidence between 6 and 24 months, a prevalence of patients of lower social status, evidence of recurrent herpes labialis as the most frequent source of infection, and frequency of herpes simplex virus 1 gingivostomatitis. The relevant findings of this study were as follows: herpes simplex virus 2 was isolated in 10% of the patients, 6% of cases occurred in the first six months of life, infection was multifocal in 36% of cases, autoinoculation was a frequent route of transmission of genital primary infection in young children, herpetic Kaposi-Juliusberg's pustulosis in infants with atopic dermatitis was the most severe presentation of primary infection and should be more adequately prevented.

[3 cases of neonatal cyto-steatonecrosis. Physiopathological discussion].

Three cases of subcutaneous fat necrosis of the newborn (SFNN) are presented. They occurred after a therapeutic coma induced by intravenous barbiturates to control convulsions unresponsive to conventional treatment. Two patients had widespread lesions. Histopathology showed prominent vascular involvement. Since 1970, 26 cases of SFNN have been published. Among the aetiological factors already described, hypothermia and ischaemia play a major role. It is suggested that brown adipose tissue might be of importance in the pathophysiology of subdural fat necrosis of the newborn.

[Magnetic resonance imaging of pelvic tumors in men. Preliminary results in a series of 28 patients].

The authors conducted a retrospective evaluation of the diagnostic value of nuclear magnetic resonance in comparison with other imaging techniques in a series of 28 pelvic tumours, with the exclusion of gastrointestinal tumours. The positive diagnosis of the lesion was obtained in every case. The variation of the signal provided information concerning the tissues, while the sections in three planes, the spontaneous visibility of the vessels and the very good natural contrast related to the presence of fat which gives a high signal, facilitated staging of the tumour.

[Post-angina septicemia caused by Fusobacterium necrophorum in a 7-year-old child].

A fusobacterium necrophorum septicemia due to a neglected peritonsillar abscess is reported in a 7 year-old boy with no significant past medical history. Osteo-articulary, hepatic and pleuro-pulmonary septic localizations, with an otherwise favourable outcome left severe orthopedic sequelae in the right hip. This resembles the post-peritonsillar abscess septicemia described by Lemierre in 1936 which was due to an anaerobic bacillus (fusobacterium). The reappearance of this pathology should lead to systematic anaerobic blood and abscess studies. Penicillin G and Metronidazole are still efficient in controlling this organism.

[NMR Tomography of the penis].

The magnetic resonance tomogram visualises the corpus spongiosum, the corpora cavernosa, the fascia penis profunda, the septum and the dorsal vein of the penis. These structures can be separated from the skin and from the glans penis. Changes of shape are still the essential criteria of assessment. At present it does not seem possible to be able to differentiate between diseased and healthy areas if the connective tissue structures of the penis (e.g. in induratio penis plastica) do not show any change of shape. Compared with other imaging methods, imaging on different levels offers advantages, and so does the simultaneous imaging of the structures of the penis, of its environment and of the adjacent organs. We can assume that the magnetic resonance tomogram will supply essential additional information for clinical diagnosis and practice in conditions like induratio penis plastica, intersexuality, tumours of the prostate gland, and in postoperative follow-up, for example after surgery for priapism.
