[Myocardial revascularization with bovine internal thoracic artery graft (BIOFLOW): a case report].

Bovine internal thoracic artery graft (BIOFLOW) was used for coronary artery bypass grafting (CABG). A 64-year-old woman who had undergone triple CABG with saphenous vein five years ago was referred to our hospital again with anginal attack. Coronary angiogram showed the obstruction of the graft for LAD and new stenosis at right coronary artery (RCA). Gastroepiploic artery was not available because the patient had a history of gastric ulcer several times, and saphenous vein had harvested initial operation. We planned re-CABG for LAD with left internal thoracic artery and RCA with BIOFLOW. But at the operation, LAD was not graftable because of severe calcification. So that we performed single CABG to RCA with BIOFLOW. The BIOFLOW was patent at the 28 th postoperative day. From our experience, BIOFLOW can be expected as a graft for CABG when saphenous vein, internal thoracic artery and gastroepiploic artery are not available.

Clinical significance of thallium-201 single-photon emission computerized tomography (Tl-201 SPECT) in the evaluation of viability of gliomas.

The clinical significance of Thallium-201 single-photon emission computerized tomography (Tl-201 SPECT) in the evaluation of viability of gliomas was studied comparatively with histological examination of tumor tissue using Ki-67 and proliferative cell nuclear antigen (PCNA) monoclonal antibody. The relationship between radionuclide uptake of Tl-201 in tumor specimens and labeling indices of special staining using Ki-67 and PCNA monoclonal antibodies were also studied. The population studied consisted of 17 patients with glioma. Tl-201 indices obtained from early and delayed images and its washout rates were used for quantitative analysis of Tl-201 SPECT findings. Tl-201 indices showed high values according to the histological malignancy of gliomas. Radionuclide uptake of Tl-201 in the tumor specimens were also high in those with high labeling indices of Ki-67 and PCNA. The lesions with marked Tl-201 uptake on early and delayed images had numerous Ki-67 and PCNA positive-stained cells. The lesion with low Tl-201 washout rate therefore reflected well the more viable lesion in the tumor tissue. It is concluded that the viability of gliomas including anaplastic changes and response of gliomas to the treatment can be detected by serial study with Tl-201 SPECT.

[Anesthetic management of a hemophilia A carrier for palliative operation and radical repair of a cardiac anomaly].

We experienced the anesthetic management of a hemophilia A carrier for two palliative operations (left and right unifocalization) and radical repair of cardiac anomaly. The patient was a ten-year-old girl and her father had hemophilia A. She was diagnosed as tetralogy of Fallot, major aortopulmonary collateral arteries and pulmonary atresia. Laboratory findings showed prolonged bleeding time and activated partial thromboplastin time, and decreased levels of factor VIII activity of 26%. When she was eight and nine years old, she underwent unifocalization. At both occasions, excessive bleeding tendency continued into postoperative period. This time she underwent radical repair in which her bleeding tendency was successfully controlled by use of a usual dose of concentrated factor VIII for twice.

[The evaluation of pre and intraoperative factors influencing the false lumen after graft replacement surgery to the extended dissecting aneurysm].

We evaluated the residual false lumen of type I and IIIb dissecting aneurysm by CT, MRI and angiography postoperatively. The 19 patients with type I dissecting aneurysm were included eleven men and eight women, the average age was 55.8 +/- 10.2 years old. The 20 patients with type IIIb dissecting aneurysm were included sixteen men and four women, the average age was 56.2 +/- 8.5 years old. The rate of distal patent false lumen was 52.6% of type I and 35% of type IIIb dissecting aneurysm after graft replacement surgery. In type I dissecting aneurysm, the rate of distal patent false lumen was 40% of acute stage vs 66.7% of chronic stage, 66.7% of ascending and partial arch replacement vs 46.2% of ascending and total arch replacement, and 90% of graft inclusion technique vs 11.1% of graft exclusion technique. The distal patent false lumen was the lowest (12.5%) with type I dissecting aneurysm of ascending and total arch replacement using graft exclusion technique. In type IIIb dissecting aneurysm, the rate of patent false lumen was 66.7% of acute stage vs 29.4% of chronic stage, 30% of graft exclusion technique vs 40% of graft inclusion technique. The size of false lumen preoperatively were larger (11.1 +/- 4.5 cm2) in patients with distal patent false lumen than that (6.7 +/- 3.2 cm2) of in patients with distal occlusive false lumen.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical experience of continuous retrograde cerebral perfusion for assisted circulation during the surgical treatment of acute dissection of aorta: Stanford A type].

Surgical treatment of the dissection involving the ascending aorta or aortic arch represents one of the most complicated technical challenges. Recently continuous retrograde cerebral perfusion (CRCP) is sometimes applied in order to avoid cerebral complications. We compared two surgical groups, one of which was performed graft-replacement of ascending aorta using CRCP and another was utilized normograde cerebral circulation, about the technique of operation, total extracorporeal circulation (ECC) time, aortic clamp time, separate ECC time, total bleeding volume, and urination during operation. In the group using CRCP, separate ECC time was 53 min (mean), and intraoperative bleeding volume was only 1,170 ml. Good urination was obtained during operation. Postoperative courses were uneventful, and consciousness was smoothly and soon recovered. The new method of separate ECC has some advantages over the previously described methods, particularly for avoidance of cerebral complications. As it is not necessary to require dissecting, taping, and clamping of cephalic branches, operative procedure will be very simplified, easier and safer, leading to intraoperative bleeding is a fewer. It is our belief that this method should be widely applied in the operation of graft-replacement of ascending aorta or aortic arch in the near future.

[Intraperitoneal chemotherapy using CBDCA for malignant gynecological tumors].

At our clinic for peritoneal dissemination cases of gynecological malignant tumors, we have been using intraperitoneal administration (ip) of anticancer agents such as CDDP with favorable results. However, since CDDP cannot be used for patients with renal dysfunction, we have administered CBDCA ip, and along with determining drug concentration, we also studied therapeutic effects. At the time of laparotomy in 5 cases of malignant tumors (ovarian cancer 4 cases, oviduct cancer 1 case), we subcutaneously implanted a reservoir port for the peritoneum in the upper inguinal region. Through this completely open port we administered by natural dripping 200-450 mg/body of CBDCA dissolved abdominal fluid and peripheral venous blood, and we determined the concentrations of total and free platinum. The ip concentration of platinum reached a peak of 142-19.8 micrograms/ml immediately after administration, and then gradually declined; at 8 hours it became 20.1-2.23 micrograms/ml, and was still detectable at 48 hours. In the peripheral venous blood peaked at 2 hours at 4.78-1.2 micrograms/ml, and was still observed at 48 hours. One 84-year-old patient with stage III oviduct cancer and renal dysfunction showed a marked reduction in ascites and improvement in PS from 4 to 1, so she is being treated on an outpatient basis. The efficacy rate was 60.0% with 2 CR and 1PR. Repeated ip administration of CBDCA was possible even in cases with renal damage rather than CDDP, but the side effects on the blood were severe. CBDCA ip achieves an effective level of free platinum in both the peritoneum administration methods for treating peritoneal disseminated cases.

[A successful surgical treatment of ascending aortic aneurysm, left common carotid artery aneurysm and left subclavian artery obstruction due to aortitis syndrome in active phase].

A 24-year-old female was admitted complaining of coldness of left upper extremity and pulsating tumor of the neck. She was diagnosed as ascending aortic aneurysm, left common carotid artery aneurysm and left subclavian artery obstruction due to aortitis syndrome on examinations. Although steroid treatment appeared to be effective in controlling inflammatory reaction, the left common carotid artery aneurysm increased in size and severe neck pain started. The risk of rupture was feared, and surgical intervention was carried out in spite of aortitis in active phase. The patient underwent surgery where aneurysmectomy and graft replacement for ascending aortic aneurysm, aneurysmectomy and graft replacement using autogenous saphenous vein for left common carotid artery aneurysm and bypass grafting for left subclavian artery obstruction were performed. The histology of resected specimens of aortic wall showed active aortitis. The postoperative course was uneventful and the patient was discharged on steroid.

[Microcirculation during inhalation of 100% oxygen in rabbits].

Using a rabbit ear chamber (REC), the author compared the effect of air inhalation and that of 100% oxygen inhalation on microcirculation of rabbits. Eight rabbits, weighing 3-4 kg, were used for this study. After observing the microcirculation under air inhalation, the inspired gas was changed to 100% oxygen and the microcirculation was again observed. This procedure was repeated twice to confirm any changes that had been observed. No significant changes were observed in the blood pressure, heart rate and pulse pressure by inhaling either O2 concentration. During the inhalation of 100% oxygen, the diameter, blood-flow velocity and blood-flow rate of the arterioles were markedly reduced (P less than 0.01). All these parameters observed in the venules during air inhalation showed no significant differences compared with the values taken during 100% oxygen inhalation. This study using REC has shown that inhalation of 100% oxygen causes microcirculatory changes especially affecting the arterioles.

Effect of size of region of interest on precision of bone mineral measurements of the mandible by quantitative computed tomography.

The effect of size and shape of the region of interest (ROI) on the precision of the measurements of bone mineral content of mandibular bone by quantitative computed tomography (QCT) has been studied. A dry mandible and a series of calcium carbonate rods (density: 119.6 mg cm-3, 461.2 mg cm-3 and 939.0 mg cm-3) were used. The precision of measurements of the ROI for sizes less than 1 cm2 was very low, but the shape did not have any influence on the precision if the material examined was either homogeneous or if it was heterogeneous from always the same area. Higher precision was found when the ROI was more than 1 cm2. The size of ROI is a critical factor in the measurement of bone mineral content in the mandible by QCT.

Image salvage in panoramic radiography using histogram optimization.

Optical density and contrast recovery from poor quality panoramic radiographs was studied using the histogram modification technique. A series of experimental images of varying density and contrast were obtained by exposing a phantom in 4 kV steps between 50 and 94 kV. A standard of good image quality was selected by 10 observers grading the radiographs. Images were processed using the histogram specification technique and re-evaluated. All the processed images were found to be of acceptable quality. This technique was also shown to be applicable for salvaging unacceptable clinical radiographs.

[A successful surgical treatment of impending rupture of the thoracoabdominal aneurysm in the aged].

In a case with impending rupture of the thoracoabdominal aneurysm of a 78-year-old aged who was admitted to our hospital as chief complaints of dysphagia, sense of pressure in the thoracic region and bloody sputum, resection of the aneurysm and patch graft aortoplasty were carried out with the aid of partial cardiopulmonary bypass. The maximum diameter of the aneurysm was 12 cm, and adhered partially with lung, and a very thin ejected region of the wall was noted. Enlargement of the saccular aneurysm in the false lumen of the dissecting aortic aneurysm accompanying with massive mural thrombus was noted. The patch graft aortoplasty was performed because back bleeding from four sets of intercostal arteries (Th 9-Th 12) was remarkably noted. The postoperative course was uncomplicated, and dysphagia was disappeared without paraplegia. This case is considered to be the oldest one who was undergone the the thoracoabdominal aneurysm in our country.

Posterolateral fusion of the lumbar and lumbosacral spine--a review of long term results.

1. Long-term results of posterolateral lumbar and lumbosacral spinal fusion were evaluated in 40 patients who had been treated by this technique 10 years or more (mean: 14 years and 6 months). 2. In the clinical evaluation employing the criteria for assessment by the Japanese Orthopaedic Association (JOA score), the total score averaged 24.2 points. The roentgenographically assessed bone union rate was 93.2%. At the site of operation, the angular displacement was within 5 degrees and the horizontal displacement ranged from 0.7% to 1.4%. Thus, a long-term stability was confirmed. 3. Although age-related problems such as osteoporosis remain unsolved, the long-term results were so good that this technique can be recommended as a salvage operation in patients with an unstable spine or those requiring decompression from the posterior approach.

Scintigraphical observation to predict fracture healing in intracapsular fracture of the femoral neck.

Predictive clinical value of scintigraphical classification of the femoral head in intracapsular fracture of the femoral neck, as classified by Hirano, et al. in 1987, was investigated by long-term follow-up (mean; 4 years and 7 months). Normal healing was achieved in 21 of 24 patients. In those cases, early 99mTc-MDP scintigraphy revealed an over-all increase of radionuclide uptake (Type A) and band-like decrease along the fracture (Type B1). Late segmental collapse of the femoral head was found in 3 patients who showed either a decrease in the weight-bearing area (Type B2) or over-all decrease of radionuclide uptake (Type C). The results of fracture healing were well correlated to each type of scintigraphical classification. The changes of radionuclide uptake in the fracture site were also followed by serial 99mTc-MDP scintigraphy.
