[Awareness of health and welfare planning for elderly and methods to increase awareness: based on social marketing].

To clarify the socio-psychological factors affecting awareness of health and welfare planning for the elderly, a survey of community residents was performed in the Tokyo metropolitan area. The results obtained were as follows: 1. There were three factors which affected awareness concern about health and welfare services for the elderly as a social issue: concern about the local community; and concern about the local government. 2. Two other factors which did not affect the level of awareness were: anxiety about the health and welfare services for the elderly as a personal issue; and having personal experience of nursing care. From these results, a method to increase awareness was studied based on social marketing methods. Two primary target groups for increasing awareness were identified. One target was people who have the socio-psychological factors described above. Another target was people who have the potential need for health and welfare services but who were not aware of it. That is, they have relatively high anxiety about health and welfare services for the elderly as a personal issue and have personally experienced nursing care. The method of approach for these targets were studied. For the first group, the amount of the information available seemed to be important because these persons are ready to recognize the need for planning. Therefore, efficient information channels should be selected. For the second group, approaches that generate greater consumer participation by presenting this as an efficient method for solution of the problem should be adopted.

Localization of novel receptor tyrosine kinase genes of the eph family, MDK1 and its splicing variant, in the developing mouse nervous system.

In the course of studies to identify new members of the eph family of receptor tyrosine kinases, MDK1 and one of its splicing variants lacking a kinase domain, MDK1-T1, were identified. To gain insight into the functions of these subtypes, expression patterns of their mRNAs in the developing mouse nervous system were examined by Northern blotting and in situ hybridization histochemistry. Colocalization of their mRNAs was observed, but the levels of expression of each mRNA were developmentally regulated. These findings suggest functional differences between full-length and truncated forms of MDK1 receptor tyrosine kinase.

Differential expressions of the eph family of receptor tyrosine kinase genes (sek, elk, eck) in the developing nervous system of the mouse.

To examine the roles of the eph subfamily of receptor tyrosine kinase (RTK), we isolated mouse cDNAs for sek, elk, and eck and localized their mRNAs in the developing mouse, with particular reference to the CNS development, by in situ hybridization. sek mRNA is most abundantly expressed throughout development; sek was detected in the germinal layer of the embryonic CNS during mid- to late-gestation and was widely expressed in the early postnatal brain. elk was expressed in the mantle layer of the embryonic CNS and showed a distribution complementary to that of sek. Differential expression of sek and elk was also observed in the early postnatal cerebellum; sek was expressed in the Purkinje cells, while elk was detected in the granule cells. eck was moderately expressed in the germinal layer of the embryonic CNS at mid-gestation, but its expression decreased as development proceeded. These spatio-temporally different patterns of gene expression suggest that these RTKs have distinct roles in mouse development despite their structural homology.

Mucosal condition of the oral cavity and sites of origin of squamous cell carcinoma.

PURPOSE: This study investigated the clinical appearance of the affected mucosa in patients with squamous cell carcinoma (SCC) in the oral and oropharyngeal region. PATIENTS AND METHODS: The mucosal conditions of 396 patients was classified into two types. Type A, showing ulcer formation and/or tumor formation, and type B, showing only mucosal enlargement without any other abnormality. RESULTS: Type A was detected in the oral cavity and oropharynx, and type B was observed only in the upper and lower alveolus and gingiva. Of 14 type B patients histologically evaluated for the relationship between the tumor cells and surface oral epithelium, 10 showed a disconnection between the epithelium and the tumor cells, whereas in two the tumor cells extended into the epithelium. CONCLUSION: It was concluded that SCC of type B is not of oral epithelial origin, but is of maxillary sinus epithelium or odontogenic cell origin. In the mandible, type B SCC originates from odontogenic epithelium (odontogenic carcinoma).

Tooth loss and mandibular osteopenia.

The relationship between mandibular bone mass and tooth loss was studied in 269 patients who had neither metabolic disease nor local lesions affecting the mandibular cortex. In all of the subjects, the outline of the mental foramen was distinctly disclosed on unilateral or bilateral panoramic radiographs. Mandibular bone mass was evaluated by determining the mandibular cortical width in the mental region with the use of panoramic radiographs. The relationships of mandibular cortical width to patient age and sex and the number of teeth present were also investigated. In male subjects, there was no significant correlation between the number of teeth present and the mandibular cortical width. Among women in their seventh decade, those with 15 or more teeth showed significantly greater mandibular cortical width than those with fewer teeth. Decrease of mandibular bone mass was positively correlated with tooth loss in female subjects.

Oral signs as indicators of possible osteoporosis in elderly women.

The relationship between oral signs and osteoporosis was investigated to assess the possibility of using this as an indicator of postmenopausal osteoporosis. Sixty-four women between the ages of 50 and 70 years were evaluated. Osteoporotic signs consisted of thoracic spine fracture as demonstrated on lateral chest radiographs. Oral signs were the number of teeth present, mandibular cortical width, alveolar bone resorption, and the morphologic classification of the inferior cortex on panoramic radiographs. The number of teeth present (N) was highly related to the probability of thoracic spine fracture and was used to derive the probability equation for the presence of thoracic spine fracture: probability value = 1/(1 + e-z), Z = 18.68-0.29 age -0.27N. A probability value higher than 0.5 suggests the possibility of thoracic spine fracture. It was concluded that this equation could serve as a simple and useful tool for dentists to assess the possibility of latent osteoporosis.

Possible identity of diffuse sclerosing osteomyelitis and chronic recurrent multifocal osteomyelitis. One entity or two.

On the basis of the findings of nine of our patients and our review of previously reported cases of diffuse sclerosing osteomyelitis and chronic recurrent multifocal osteomyelitis, we discuss the similarity of these two entities. Our nine patients had initially been given diagnoses of diffuse sclerosing osteomyelitis on the basis of their clinicopathologic findings. However, technetium 99m-MDP bone scans performed on four of them revealed multiple bone lesions leading to the diagnosis of chronic recurrent multifocal osteomyelitis. Furthermore, no clear difference between clinical features in the patients with multiple bone lesions and those in the patients with diffuse sclerosing osteomyelitis was found. We conclude that diffuse sclerosing osteomyelitis is an expression of chronic recurrent multifocal osteomyelitis.

Thallium-201 in brain tumors: relationship between tumor cell activity in astrocytic tumor and proliferating cell nuclear antigen.

UNLABELLED: This study was performed to assess the relationship between 201Tl chloride uptake and brain tumor proliferation using monoclonal antibody Ki-67 and proliferating cell nuclear antigen (PCNA). METHODS: Thirty-four patients with brain tumors were studied. Serial SPECT images were recorded and thallium uptake (Tl index) and washout rates in the tumors were calculated. Imaging results were compared with those from biopsy and histology. Cell proliferation was determined by PCNA or Ki-67 monoclonal antibody staining. RESULTS: Thallium-201-chloride indices of the astrocytoma were 1.73 +/- 0.17 and 1.48 +/- 0.07 on early and delayed images, respectively. On the other hand, 201TI indices for anaplastic astrocytoma were 2.60 +/- 1.05 and 1.76 +/- 0.93 and 3.26 +/- 1.63 and 2.23 +/- 0.56 for glioblastoma. The correlation between the 201TI (delay) and Ki-67 indices for astrocytic tumor. There were no significant differences between Ki-67/PCNA indices and washout rates. CONCLUSION: There was a positive correlation between PCNA but not the Ki-67 labeling index and the 201Tl index. With the use of a noninvasive technique, 201TI index supports the PCNA index.

[The importance of SpO2 and FIO2 for anesthetic management for Norwood's operation].

The systemic and pulmonary vascular resistance needs to be balanced before cardiopulmonary bypass (CPB) and high flow pulmonary circulation must be avoided after CPB during anesthetic management for Norwood's operation. The level of SpO2 was controlled at 75-80% to maintain systemic and pulmonary circulation. The relationship between the change of color of the lung and level of SpO2 through gradual increase of pulmonary blood flow was carefully observed. As SpO2 level increased, lung changed color from white, pink to red. The SpO2 level of lung in red color was above 85%, suggesting high flow pulmonary circulation. To decrease the level of SpO2, FIO2 was decreased. The lung changed to pink and balance of systemic and pulmonary circulation was maintained, reconfirming that control of SpO2 and FIO2 levels is important for this operation.

Localization of pleiotrophin and midkine in the postnatal developing cerebellum.

We demonstrated that pleiotrophin (PTN) and midkine (MK), proteins which constitute a novel heparin-binding growth factor family, are expressed in the postnatal developing cerebellar cortex by an immunohistochemical method. The expression patterns of both factors were quite similar both temporally and spatially; the levels of protein expression reached a peak at P7, and strong immunoreactivities were localized primarily in the premigratory zone of the external granular layer, the molecular layer, and the white matter. These findings suggest that PTN and MK are co-localized and thereby exert synergistic functions in the developmental events such as the migration of granule cells and the process outgrowth of neurons.

Chronic recurrent multifocal osteomyelitis involving the mandible.

A patient with chronic recurrent multifocal osteomyelitis is presented for the first time in the dental literature. On the basis of the initial diagnosis of diffuse sclerosing osteomyelitis of the mandible, the clinical course was protracted and refractory to surgical and antibiotic therapies. The literature is reviewed to evaluate the relationship between chronic recurrent multifocal osteomyelitis and diffuse sclerosing osteomyelitis. Distinguishing features between the two entities could not be found. It is recommended that a skeletal survey be performed when the diagnosis of diffuse sclerosing osteomyelitis is established.

Primary intraosseous carcinoma: review of the literature and diagnostic criteria.

Twenty-four reports describing 39 cases of primary intraosseous carcinoma (PIOC) were reviewed and the clinicopathologic features were summarized. The mean age of the patients at the time of diagnosis was 51.0 years, and the male: female ratio was 2:3. The posterior mandible was the predominant site; in no patient was a lesion observed in the posterior maxilla. Twenty-five of 31 patients (80.6%) showed swelling of the oral mucosa. However, ulcer formation was observed in only 3 of 36 patients. Pain, sensory disturbances, and metastasis to regional lymph nodes were observed in 25 of 34 patients (73.5%), 9 of 15 patients (60%), and 13 of 33 patients (39.4%), respectively. Radiographically, most lesions produced bone resorption with ill-defined margins (51.6%) or with well-defined margins (19.4%). The diagnostic criteria proposed for PIOC are absence of ulcer formation, except when caused by other factors; histologic evidence of squamous cell carcinoma without a cystic component or other odontogenic tumor cells; and absence of another primary tumor on chest radiographs obtained at the time of diagnosis and during a follow-up period of more than 6 months.

Radiographic evaluation of bone invasion of adenoid cystic carcinoma in the oral and maxillofacial region.

PURPOSE: To investigate the relationship between clinicopathologic features and bone invasion in adenoid cystic carcinoma. PATIENTS AND METHODS: Thirty-two patients with adenoid cystic carcinoma were included. Of 17 patients with suspected bone invasion based on clinical and/or radiographic findings, 13 also underwent histologic evaluation. RESULTS: Bone invasion was detected in nine patients. Bone involvement was radiographically classified as erosive, diffuse invasive, or minimal change. No tumor infiltration into the surrounding bone marrow spaces was observed in the erosive-type tumors. However, tumor invasion through the resorbed cortex was observed with the diffuse invasive tumors and through the bone defects in the minimal change tumors. CONCLUSION: Histologically, diffuse invasive radiographic change was observed with solid lesions. However, minimal bone resorption was observed on radiographs of the glandular lesions, even when the tumor cells had infiltrated extensively into the bone marrow spaces. In the tumors of the tubular type, all three radiographic types were observed.

[Surgical treatment and long-term results of congenital heart disease in adults: early and late follow-up studies in 231 cases].

In a past one decade from January 1981 to August 1991, 231 patients over 18 years old with congenital heart disease (CHD) including 65 patients with cyanotic (C) and 166 with acyanotic (AC) diseases, were surgically treated. There were one operative death (0.6 percent mortality) and three late deaths (1.9%) in 166 patients with AC diseases, whereas there were four operative (6.1%), and four late deaths (6.1%) in 65 patients with C. These patients were followed for 3 to 124 months (the mean of 46.4 +/- 11.9) after the operation. Compared with group AC, group C showed a high rate of early postoperative deaths or late deaths. Group AC comprised patients in their forties (95 patients with ASD and 15 with PDA) and those in their thirties (26 with VSD and 13 with ECD) at the operation. On the contrary, except 5 patients with Ebstein disease, a mean age of patients of group C at the operation lay in their twenties. Surgical outcomes for adult patients of group C still pose much problems compared with AC group in terms of decreased heart function, development of collateral circulatory pathway, and impaired hepatic and renal functions by long lasting hypoxemia. In group C decreased heart function or association of abscess of the brain might be the main cause of postoperative LOS. These findings indicate that early diagnosis, recent advanced operative procedure and appropriate postoperative care can provide symptomatic remission for even adult patients with severe CHD with a low mortality.

Internucleosomal DNA cleavage involved in ischemia-induced neuronal death.

Pyramidal neurons of the hippocampal CA1 are known to be particularly vulnerable to transient ischemia resulting in "delayed neuronal death". Recent studies using aurintricarboxylic acid suggested that ischemia- or excitotoxin-induced neuronal death should share intracellular mechanisms in common with apoptosis. It is, however, unclear about involvement of endonucleases. Here using a transient (5 min) forebrain ischemia model in gerbils, we found that internucleosomal DNA fragmentation developed between 48 and 54 hr recirculations, accompanied with simultaneous or slightly preceding destruction of microtubule-associated protein 2. These results suggest that endonucleases, maybe activated by elevated intracellular Ca2+, play an important role in delayed neuronal death as well as in apoptosis.

Induced resistance and susceptibility to cerebral ischemia in gerbil hippocampal neurons by prolonged but mild hypoperfusion.

Brief periods of non-lethal cerebral ischemia can induce resistance against subsequent lethal ischemia. In this study, asymptomatic gerbils after unilateral carotid artery ligation were subjected to 5 min of forebrain ischemia. The prolonged but mild hypoperfusion, by carotid occlusion, induced susceptibility at 1 day and tolerance at 30 days to lethal ischemia in the hippocampal neurons. The neuroprotective effect correlated well with induction of heat shock protein 72 in the hippocampal neurons. These results suggested that neuronal cells possess a cellular response to sublethal hypoperfusion and can survive forthcoming ischemic stress.
