Transcatheter embolization of the renal artery with bucrylate in renal carcinoma.

In 5 patients with large renal carcinomas Bucrylate was injected into the renal artery. One to 3.5 ml Bucrylate were required to obtain occlusion of the arteries. At operation 14 to 38 days later the arteries were still occluded. One patient developed pulmonary embolism, other complications were of less importance. The main indication for the procedure is very large, highly vascularized carcinomas in the upper part of the kidney. Bucrylate seems to be a suitable embolus inducing agent.

Amipaque in coeliac angiography.

Coeliac angiography was performed in 12 patients using Amipaque 370 mg I/ml or Isopaque 370 mg I/ml. No significant biochemical alterations were found in serum. Amipaque improved the demonstration of small vessels in the liver and in the head of the pancreas. Amipaque caused no discomfort contrary to Isopaque, which gave moderate to severe heat during injection.

Dysphagia after proximal gastric vagotomy.

Ten patients were examined before and after proximal gastric vagotomy with videotape recordings of barium passage in esophagus, esophageal pressure recordings, and the insulin test. Four of the patients complained of dysphagia after the operation. The dysphagia subsided spontaneously after 1--2 months. Patients with dysphagia showed dilatation of the body of the esophagus and a tapered narrowing in the distal segment. The pressure recordings showed increased frequency and strength of rhythmic pressure waves compared with the preoperative findings. Our results support the theory of a neuromuscular dysfunction.

Amipaque as contrast medium for intraosseous phlebography.

The degree of pain produced by Amipaque 260 and Isopaque 260 at intraosseous phlebography in patients with varicose ulcerations in the ankle region was compared. Amipaque produced less pain than Isopaque. No complications or side-effects occurred during the observation period of about one year.

A comparison of a new non-ionic (metrizamide) and a dissociable (metrizoate) contrast medium in endoscopic retrograde pancreatography (ERP).

Endoscopic retrograde pancreatography (ERP) was performed on 12 patients with a non-ionic contrast medium (metrizamide) and a traditional dissociable medium (metrizoate). All patients were examined with both media at an interval of one week. The study was based on double blind and a cross-over randomization. There was a significantly lower rise in serum amylase 2 (p = 0.05), 4 (p = 0.04), and 6 hours (p = 0.03) after the examination using the non-ionic contrast medium than after using a dissociable one. The pain during contrast filling of the pancreas was significantly lower (p = 0.015) when using metrizamide than when using metrizoate. All, except one patient, preferred the examination with the non-ionic contrast medium (p = 0.0002). A positive correlation was found between pain and degree of pancreatic filling when using the dissociable medium (r = 0.77). Such a correlation could not be detected following the non-ionic contrast medium (r = 0.009). There was a positive correlation between rise in serum amylase and degree of pancreatic filling with both contrast media. The coefficient of correlation was lower after metrizamide (r = 0.21) than after metrizoate (r = 0.56). It is concluded that a non-ionic contrast medium in this study was better tolerated than a dissociable medium, and that metrizamide was more suitable for endoscopic retrograde pancreatography than the traditional hypertonic contrast medium.

Demonstration of pedal arterial arcades in occlusive arteriosclerotic disease. Conventional and digital subtraction angiography compared.

Conventional arteriography, intraarterial digital subtraction angiography (DSA), and intravenous DSA were used in the evaluation of the patency of the pedal arterial arcades in 15 patients with advanced peripheral arteriosclerotic disease. Intraarterial DSA was found to be superior to conventional arteriography. In 12 of 15 patients, excellent or good results were achieved by intraarterial DSA compared with 5 or 15 patients examined by conventional arteriography. All intravenous DSA studies performed in 6 patients were classified as poor.

Out-patient surgical biopsy for nonpalpable breast lesions. Technique, results and consequences.

During the last 5 years wire-marked biopsies of nonpalpable mammary lesions have been performed with use of local anesthesia in our out-patient department. The diagnostic accuracy and the complication rate were comparable with those reported from biopsies in general anesthesia. The patients tolerated the procedure well. A malignant: benign diagnostic ratio of 1:2.7 was obtained by close collaboration between the participating surgeons, radiologists and pathologists.

Non-palpable breast lesions. Diagnostic criteria and preoperative localization.

Marking of non-palpable breast lesions for biopsy has become a routine procedure in patients in whom mammography has suggested malignancy. Between October 1981 and December 1985 a localization method was used in 123 patients. In 35 per cent of the biopsies a malignant lesion was disclosed. In patients with a mass only, malignancy was found in 37 per cent of the biopsies. If microcalcifications alone were the main reason for the biopsy, malignancy was disclosed in 25 per cent. If both a mass and microcalcifications were present, 52 per cent of the biopsies disclosed a malignant lesion.
