Transhepatic endoprosthesis and surgical anastomosis in the palliative treatment of bile duct obstructions.

Ninety-six patients with non-resectable malignant tumours occluding the bile duct were treated either with a transhepatic endoprosthesis (n = 60) or a surgical biliointestinal anastomosis (n = 36). The mean survival time for patients treated with a transhepatic endoprosthesis was 5.8 months, as compared with 8,1 months for patients treated surgically. The mortality rates in the first 30 days were 18% and 17% for patients treated anastomosis, respectively. We consider that this militates in favour of an endoprosthesis rather than surgery.

Venous thrombosis following diagnostic transvenous catheterization by percutaneous catheter insertion: an evaluation of desmopressin as a thromboprophylactic agent.

The preventive effect of desmopressin with respect to catheter induced thrombosis was studied in a randomized double-blind trial, consisting of 30 patients undergoing percutaneous transcubital right heart catheterization. Phlebography of the catheterized arm was performed after five days. The frequency of post-catheterization thrombosis was reduced by 33 per cent, from 86 per cent in the treatment group to 53 per cent in the control group (0.1 less than p less than 0.2). This effect was restricted to minor thrombi, whereas major thrombosis could not be prevented. Patient materials such as that of the present study, may become useful in preliminary investigations of thromboprophylactic agents.

Comparison of iohexol and meglumine-Na-Ca metrizoate in visceral angiography.

Visceral angiography was performed in a double blind study in 54 patients using iohexol (350 mg I/ml) and meglumine-Na-Ca metrizoate (370 mg I/ml). No significant increase in serum values of either hepatic or pancreatic enzymes or electrolytes was registered. Both contrast media gave excellent demonstration of arteries as well as veins. Iohexol gave far less sensation of heat during the injections than metrizoate.

Lymphography in the evaluation of urinary bladder carcinoma.

Lymphography was performed in 41 patients with bladder carcinoma. The lymphographic findings were correlated to the clinical and histological staging and the pre-operative grading. Twenty-seven patients were operated upon. The findings of lymphography were compared with the findings at operation, and the pathological examination of the removed lymph nodes. The lymphographic diagnosis was proved to be correct in 77.7% of the patients. There were two false positive lymphographies, and one false negative. Lymphography in groups T3 or T4 yielded a high diagnostic accuracy.

Duodeno-jejunal intubation in examination of the small intestine.

The small intestine was examined with the duodenal intubation method in 113 patients. Patients admitted with acute abdominal disease were not included. The frequency of positive findings was 22%, which exceeds that of previous years before the method was introduced. Eight per cent were found to have Crohn's disease. Thirteen patients were admitted for gastrointestinal bleeding, and in two of them the cause was found. All patients with uncharacteristic abdominal symptoms, were normal. The results and advantages of the method are discussed.
