A fall of plasma osmolality created at dialyzer and its possible effect on circulating blood volume.

A mathematical analysis of fluid exchange during hemodialysis therapy was performed in order to determine which factors affect the circulating blood volume, and hence the circulatory stability. It was found that both fluid removal and also the fall of plasma osmolality during the passage of blood through the dialyzer can affect the circulating blood colume profoundly. The later was found to accelerate the decrease of circulating blood volume by an indirect action on capillary refilling. Some treatment systems such as those involving recirculation or ultrafiltration were found to show a small fall in osmolality with a correspondingly small fall in blood pressure. This result seems to be compatible with clinical experiences reported earlier and may explain why some methods of treatment are accompanied by stable circulatory function in spite of rapid removal of large amounts of fluid.

Clinical studies of "big ACTH": its physico-chemical characteristics.

The big ACTH fractions available from human plasma and pituitary glands and from porcine pituitary glands were physico-chemically characterized by gel filtration, disc electrophoresis and isoelectric separation. In the case of healthy human subjects, big ACTH fractions were isolated by gel filtration from plasma samples taken during states of acute ACTH hypersecretion such as the lysine-8-vasopressin, insulin or metopyrone tests though none of these fractions were isolated from plasma sampled under normal conditions. Even with no stimulation of ACTH secretion, patients with Cushing's disease gave plasma samples that contained an isolable big ACTH fraction, but such a fraction was hardly isolated from plasma taken from patient with Addison's disease. Both human pituitaries and porcine pituitaries contained an isolable big ACTH fraction. By a gel filtration analysis the molecular weight of the big ACTH was estimated to be higher than 20 000. Disc electrophoresis with an acrylamide gel indicated that big ACTH is strongly basic while small ACTH is more acidic than pH 8.3. Isoelectric separation revealed that the isoelectric point of human big ACTH is higher than pH 10.0 while that of small ACTH is about pH 6.8.

Hemodynamic effects of acetate in man.

To determine the action of acetate in man, 1.7mEq/ml of sodium acetate was infused in 10 patients on chronic hemodialysis. It was administered intravenously in 1.5mEq/min for 5 min followed by 3.8mEq/min for another 5 min. As control, 1.71mEq/ml of sodium chloride was infused in a similar manner. Following the infusion of sodium acetate, heart rate increased slightly but significantly (p less than 0.01). Limb blood flow measured in an upper limb by the venous occlusion method, showed an increase in all of them and it was significant( p less than 0.001). The peripheral vascular resistance (=mean arterial blood pressure/limb blood flow) showed a significant fall (p less than 0.001). The analysis of systolic time interval showed a change suggestive of depression of heart function at the end of sodium acetate infusion (p less than 0.05). Such a vasodilating effect or a change in heart function was not observed following sodium chloride infusion. A depressant action of acetate upon the cardiovascular system may be, therefore, concluded.
