Antisera against proline rich peptide P-C, recently isolated from human whole saliva were raised in rabbits by injections of peptide P-C-BSA conjugates. Immunohistochemical study using the antisera was carried out on human salivary glands, gut and pancreas. The results showed that peptide P-C like immunoreactivity was present not only in the salivary glands but also in the pancreatic islets, though not in the gut. Furthermore, immunostaining of adjacent thin sections revealed that cells reacting with antisera against peptide P-C were identical to those reacting with insulin antisera. As the antisera against peptide P-C did not have any cross-reactivities to insulin, glucagon, somatostatin, pancreatic polypeptide, VIP and human C-peptide, the antisera were considered to recognize specifically either peptide P-C related antigen or peptide P-C itself in human pancreatic B-cells. A novel substance, peptide P-C like immunoreactivity, may be present in pancreatic B-cells independent of pro-insulin and insulin. Morphological similarity between the salivary glands and the pancreas has been reported, and amylase, kallikrein and glucagon are present in both. These findings seem to suggest some functional relation between the pancreas and salivary glands. Detection of peptide P-C like immunoreactivity in the pancreas and salivary glands would be a additional support for this idea. Although it is suggested that peptide P-C like immunoreactivity in pancreatic B-cells may play some role in the function of B-cells, its exact pathophysiological role remains obscure.

A 74-year-old man with brady-tachy syndrome showed split atrial activities in electrophysiologic studies. The split intervals of atrial activities varied spontaneously or in response to electric stimulations. The shorter were the coupling intervals of atrial extrastimuli, the longer the split intervals of captured atrial activities. At a critical timing of atrial extrastimuli, atrial flutter-fibrillation was initiated reproducibly. During atrial flutter-fibrillation, the split activities showed Wenckebach-like phenomenon. These findings seem to result from depressed conductivity. The split atrial activity may reflect a diseased state of the atrium which predisposes to atrial arrhythmias.
This case report describes a 60 years old male with Wenckebach A-V block which progressed to high degree A-V block. Syncope and dizzy attacks were attributed to high degree A-V block in the study of ambulatory ECG (Holter). Wenckebach point was low; 90 bpm when the right atrium was paced. At 140 bpm, 2:1 A-H block with intermittent alternating and high degree A-H block were induced. Furthermore, a very wide His potential was recorded during the electrophysiological study. He received a permanent pacemaking. Since Wenckebach type A-V block is usually regarded as benign, this case seems to be rare.
Three patients with atrial myxoma, two in the left atrium and one in the right were studied using two-dimensional echocardiography. An analysis by four cross sections, long and short axis, subcostal and four chamber views of the left ventricle showed that the stalk of one left atrial myxoma adhered to the anterior commissure, the other to the left atrial wall underlying on the aortic posterior wall, and that of right myxoma to the free wall of the right atrium. The myxomas were successfully removed by surgical intervention in all cases, where the sites of stalk were verified to be identical with those diagnosed by echocardiography.

This study was designed to evaluate the efficacy of scintigraphy by using 111In-oxine labeled platelets for the detection of either intracardiac or intravascular thrombi. Fourteen cases of thrombi diagnosed or suspected on the basis of either angiography or two-dimensional echography were examined. They included four cases of left ventricular thrombi, three of left atrial thrombi, four of vascular thrombi and three of inflammatory disease. Platelet samples from the patients were labeled by 111In-oxine according to the method originated by Yui et al. The labeling efficiency was 72.4 +/- 9.8%. The platelets retained good function except for collagen aggregability. Hot areas of high quality in the scintigram were demonstrated in six cases including two with left ventricular thrombi, two with left atrial thrombi and two with vascular thrombi. These areas accorded well with the findings of angiography, echography or surgery. The removed thrombi had a layer of aggregated platelets on the surface. Platelet survival was shortened in a case of prosthetic valve endocarditis without a hot area, and was normal in four out of five cases including two with a hot area. It is concluded that this method is potentially predictive in detecting thrombi.

The clinical effects of a new anti-viral 9-(2-hydroxymethoxymethyl) guanine (Aciclovir) against Herpes virus infections have been investigated. The patients had malignant tumours or auto-immune disease complicated by shingles and chicken pox due to Vaicella zoster virus (VZV) (43 cases), Herpes simplex virus (HSV) (10 cases) and 9 cases which were clinically diagnosed as Herpes, though the virus was not confirmed as the causative agent. As a general principle the dosage of Aciclovir was 5 mg/kg, t. i. d. for 5 days by slow intravenous infusion. The clinically effective rate against VZV was 93%, being excellent in 42% and against HSV it was 80%, being excellent in 40% and when the results of the cases of unknown origin were included it was excellent in 40% and the cumulative effective rate was 88%. Concerning the efficacy in reduction of pain, swelling, disappearance of vesicles and new scab formation, the effect was most noticeable after the third day of treatment. Treatment given early in the disease is likely to provide better results. Concerning side effects, one of 62 patients had proteinuria and the other had a drug rash and an abnormal liver function test. It is likely that the combination of treatment and the primary disease had some influence, but the cause/effect relationship to Aciclovir treatment is not clear.

Eight patients with adult T-cell leukemia (ATL) were observed at Niigata University Hospital during the last seven years. They had typical hematological findings of ATL. All patients tested possessed both natural antibody against ATL-associated antigens (ATLA) in the serum and ATLA in peripheral mononuclear cells. These findings agree with the results of virological analysis in patients with ATL in other areas of Japan and human T-cell leukemia (HTL) in the U.S.A. Six out of the eight patients were native residents of Sado Island in Niigata Prefecture. They showed somewhat different clinical features from patients in other endemic areas and a variety of skin lesions. It was noticeable that the skin lesion regressed spontaneously in two patients. In view of the fact that the total population of this island is about 80,000, the incidence of ATL is thought to be high. Immunological studies were performed in our patients and the immunological background of ATL is discussed.
