The clinical use of CO2 laser beam in the surgery of kidney parenchyma.

Encouraged by the experimental work done on 40 kidneys in 20 dogs using the CO2 laser beam for partial nephrectomies, we decided to use the laser in four patients. Three of them underwent lower partial nephrectomy, and the fourth underwent bivalving of kidney through the anatrophic line. The use of this new instrument was very helpful in reducing blood loss, shortening the time of hemostasis, and minimizing parenchymal damage. Case No. 1 is the first published case of xanthogranulomatous pyelonephritis that underwent partial nephrectomy with success.

Percutaneous nephrostomy for assessment of functional recovery of obstructed kidneys.

The use of percutaneous nephrostomy to evaluate the recoverability of function of the obstructed kidney is described and examined as a practical clinical method. Long-term drainage was used to assess the functional status of four kidneys and found to be of great value. The use of percutaneous nephrostomy to evaluate function has been mentioned previously but has not become established. In view of inaccuracy of other methods of assessment, we consider percutaneous nephrostomy to be the most accurate predictive test available today and suggest criteria for its use.

In vivo and in vitro activity by diverse chelators against Trypanosoma brucei brucei.

A system of prescreens and screen has been developed to select chelators as potential drugs against Trypanosoma brucei brucei EATRO 110. The chelators tested were nearly all commercially available, low molecular, and having moderate to high affinity for Fe(III). We prescreened 70 compounds showing heme-sparing or inhibitory activity in a Crithidia fasciculata growth system having excess Fe and minimal hemin. Of these, 45 were highly trypanocidal for suspensions of bloodstream T. b. brucei; criteria of activity here were immobilization, lysis, and loss of infectivity. Eighteen of the chelators highly active in the suspension prescreen were tried in T. b. brucei-infected mice. Thirteen of these chelators were curative in mice with 24-h infections, that is, they allowed survival greater than 30 days beyond the untreated controls. 3,4-Dihydroxycinnamic acid (caffeic acid), 2,9-dimethyl-1, 10 phenanthroline (neocuproine), and 2-pyridinecarboxaldehyde-2-pyridyl-hydrazone cured five out of five mice after an i.v dose of 100 mg/kg. Salicylaldehyde thiosemicarbazone cured five out of five mice at an i.p. dose of 500 mg/kg. Lesser activity was shown by several other chelators.

The effect of salt loading diet on the intraocular pressure.

Salt tablets are habitually ingested by some people while doing physical exercise in hot climate, in order to prevent sodium depletion. The effects of supplementary salt on the intraocular pressure was investigated on 5 healthy acclimatized subjects who rode a bicycle ergometer at temperature of 35 degrees C and relative humidity of 40%. A significant decrease of 1.0 mmHg in their mean intraocular pressure was detected when 10.2 gm NaCl was added daily to the normal diet. The pressure reduction was associated with changes of the body fluid balance.

Do sex, ethnic origin or environment affect myopia?

The degree of myopia was investigated in males and females from the same ethnic group who were raised in the same environment, in subjects from the same origin who were raised in extremely different environments, and in subjects coming from different ethnic groups. No statistically significant differences were found between these groups. Most probably the degree of myopia is not influenced just by sex or ethnic origin or environmental conditions.

The alpha-adrenergic blocking effect of prazosin on the human prostate.

The effect of prazosin on the adrenergic receptors in the human prostatic adenoma and prostatic capsule was investigated by an in vitro isometric technique. The results showed an alpha-adrenergic blocking action on both tissues. The cholinergic response of the capsule and the direct response of the adenoma were not affected. It is suggested that prazosin may prove preferable to phenoxybenzamine for clinical use as a prostatic alpha blocking agent, because of the selective action on the alpha1 receptors.

Prolactin concentration in prostates with benign hypertrophy.

Twenty-two hypertrophied prostates removed in surgery were checked for prolactin concentration by the radioimmunoassay method. The mean concentration in the prostatic tissue was found to be 38.2 +/- 1.9 ng./Gm. while the mean basal serum level was 11.9 +/- 1.3 ng./ml. It is concluded that serum prolactin levels do not reflect prostatic prolactin concentration. The possibility that interrelationship in action exists between androgens and prolactin affecting each other's concentration, both influencing prostatic metabolism is discussed.

Phenoxybenzamine for benign prostatic obstruction. Review of 200 cases.

Two hundred patients treated with phenoxybenzamine for benign prostatic obstruction were reviewed. In 171 of these patients the treatment was given to relieve symptoms in patients not requiring operation, or in patients in whom operation was postponed. Eighty per cent of these patients derived benefit from the treatment, most commonly for the obstructive symptoms (78.7 per cent) but almost as often for nocturia (76 per cent). The treatment was also found to be useful in aiding catheter removal after an attack of acute retention, and in preventing recurrent attacks of acute retention. Side effects occurred in 30 per cent of patients, but necessitated cessation of treatment in only 10 per cent. They consisted of other alpha-blocking effects, and caused after stopping or reducing treatment.

Rapid in vitro prescreen for chelators as potential trypanocides based on growth of Crithidia fasciculata.

A rapid in vitro prescreen for Fe-binding chelators has been developed with growth of Crithidia fasciculata and the sparing of its heme requirement in a defined medium as a test system. The prescreen functions as an index of chelator-mediated Fe transport and as an index of growth inhibition, presumably by the interference with Fe and/or heme metabolism at intracellular chelatable sites. Of 161 chelators examined, 84 were active heme-sparers; 32 of these inhibited growth at low chelator concentrations. Twenty-eight other chelators inhibited growth and another 49 were inactive. Such chelating activity directed at Fe and heme targets in hemoflagellates may provide leads for chemotherapy.

Genetic analysis of Tourette syndrome suggesting major gene effect.

Data on Gilles de la Tourette syndrome are analyzed by multiple threshold models in inheritance that incorporate sex effect. The polygenic-multifactorial model is rejected. Single major locus inheritance can account for the data, although many of the occurrences of Tourette are due to nongenetic phenocopies. In both models, males and females share a common genetic environmental liability, but the less prevalent sex, that is, females, has a higher genetic loading for the disorder. The predicted population prevalences in the single major locus model are 2.3% for males and 0.8% for females. The implications for genetic and biological research in Tourette syndrome are discussed.

The relationship between body temperature and intraocular pressure.

A comparison of the intraocular pressures of well-hydrated nonacclimatized and acclimatized subjects during submaximal work load in a hot environment was made. This comparison attempts to eliminate all possible influences on IOP, except body temperature. A direct relationship between the IOP and rectal temperature is postulated.

The action of bromocriptine on human detrusor muscle.

The pharmacological effects of bromocriptine on human detrusor muscle were investigated by an in vitro isometric method. Both a direct stimulating effect and an anticholinergic effect were demonstrated. These effects, together with an alpha-adrenergic blocking effect reported in a previous paper, are correlated with the published clinical effects of bromocriptine in patients with primary and secondary unstable detrusors.
