[The effect of calf blood dialysate on the excretion of estriol in placental insufficiency].

The efficacy of a dialysate of calf blood in the treatment of chronic placental insufficiency is assessed using the urinary oestriol elimination as parameter. Compared with a control group, those cases receiving 2 times 250 ml protein-free dialysate of calf blood (Solcoseryl) in saline solution over 9 days showed a significant increase in the daily quantity of urinary oestriol elimination already from the second day of treatment onwards. Other conventional procedures in the medical treatment of placental insufficiency are discussed. Some of these are known to have secondary effects or must be regarded as invasive.

[Sonographic study of the growth of the uterus and ovaries between the ages of 1 and 14].

Ultrasonic median values of the dimensions of the uterus and volume of the ovaries were determined in a statistically sufficiently large group of girls in the hormonally inactive stage between 1 and 14 years of age who were healthy from the paediatric-gynaecological viewpoint. The graphic presentation of the results is an essential prerequisite for routine outpatient sonography. The most striking result is an almost linear, significant growth of the uterus and ovaries even before the menarche.

[Hydrops universalis and respiratory distress due to congenital lung tumor].

A case of an immature female newborn child is reported, with a solitary alveolarcell adenoma rising from the left lower lobe of the lung. The consequences of the congenital blastoma of the lung, which is to be regarded as hamartoma, are Hydrops fetalis, a polyhydramnion, respectively a Respiratory Distress not responding to treatment. Finally a survey of the available literature is given on non-immunologic Hydrops fetalis caused by congenital tumors.

[Lymphatic cysts after radical abdominal surgery--epidemiology and aetiopathogenesis].

In the last ten years have been 8 cases of lymphatic cysts among 225 regularly controlled patients after Wertheim's operation. There is a significant difference between a control group with and one without resection of the Rosenmüller's node, in addition to an obligatory extirpation of lymphatic nodes. The significance of this lymphatic node for the genesis of lymphocysts is supported by the localisation of small cysts in ultrasonography. The influence of other factors on the genesis of lymphocysts is discussed in our own cases and in those of the available literature.

[Surgery or radiotherapy for cervical carcinoma stage IIb].

134 cases of cervical cancer of the clinical stage IIb were examined with regard to the five-year survival rate. The report covers the time from 1964 to 1976. The patients had either been operated on primarily or had primarily undergone irradiation treatment. The five-year survival rates for the first group amount to 57.69% (n = 78), for the second group to 42.86% (n = 56). This difference is not significant under the restriction of limited comparability because of the different age of the patients. 65.38% (abs.: 51 from 78) of the operated cases turned out to be clinically overestimated cases of cervical cancer of the histological stages Ib, Ic and IIa. 27 cases belonged to the clinical and histological stage IIb and for this group of patients the five-year survival rate was 29.62% only. This and a number of other factors--the narrowing of the site of the operation with the growing danger for the ureter, increasingly affected lymphatic nodes--would justify restraint concerning surgical treatment of stage IIb.

[Etiology of uterine prolapse].

From the factors known to promote descensus those which may best be objectified--among them overweight, hard physical labour, weakness of the connective tissue and number of births--are examined in a group of women with descensus and a control group with regard to their valence for the development of a descensus. Log-linear models are applied in the calculation of qualitative variables. According to that, overweight means a fourfold, hard physical labour a threefold, weakness of the connective tissue a 2,7-fold and birth of more than one child a 2,2-fold risk of descensus.

[Uterine prolapse after episiotomy].

The value of episiotomy in regard to the prevention of descensus vagina et uteri is examined in a statistically reliable number of patients (64 cases). Women who were delivered without episiotomy provided the control group (70 cases). In both groups factors promoting pelvic relaxation were taken into consideration. According to the results of this examination the importance of episiotomy as prophylaxis of pelvic relaxation does not justify any obligatory use.

[Congenital abnormalities--risk, incidence and etiology. Catamnestic studies on causal teratogenesis].

Questionnaires were sent to the mothers of infants with congenital malformations delivered at the Second Department of Obstetrics and Gynecology, Vienna University Medical School, between 1972 and 1978 and to a group of controls. The response rate was 48%. On evaluation, the incidence of maternal disease and therapeutic drug intake was found to be significantly higher in the group with congenital malformations. In addition, malformations were significantly more common in planned pregnancies. A teratogenetic role of emotional stresses was not verifiable with statistical significance. As inheritance of numerous malformations tends to be multifactorial, endogenous factors were equally considered, but failed to show significant differences.

[Intrauterine pressure and fetal and maternal heart rate on administration of the betamimetic agent, hexoprenaline during labour (author's transl)].

The tocolytic effect of hexoprenaline was determined in 20 women by internal cardiotocography during labour. After 30 minutes of intravenous medication (0.3 microgram/min = dosage for massive tocolysis) the uterine activity decreased on average by 73.6%; the maternal pulse rate increased by 25.0%; the fetal heart rate showed no significant change.

[Pregnancy and delivery in 12 to 15 year-old girls (author's transl)].

From January 1st 1974 to March 1st 1980, altogether 51 girls under the age of 16 were admitted for delivery at the 2nd Department of Obstetrics and Gynaecology, University of Vienna, Medical School. By means of the following parameters, i. e. premature birth rate = 5.9%, rate of gestosis (GI greater than 3) = 3.9 and perinatal mortality rate = 0, the question of clinical assessment of gestation as high-risk pregnancy in the case of young girls in discussed yet again. For the total number of reported cases within the specified period, the rate of premature birth is 8.7%, the corresponding rate of gestosis is 6.7 and the overall perinatal mortality rate is 1.6%.

[Incidence of congenital malformations among births of foreign workers (author's transl)].

From 1967 to 1972 7.83% of the parturients delivered at the Department of Obstetrics and Gynecology II of the University of Vienna were foreign workers. From 1972 to 1978 the part of the foreign workers come to 21.31%. From 1967 to 1972 there was a highly significant prevalence of so-called "non-viable malformations" (p less than 0,001) among foreign workers and "severe external malformations" were significantly more common at the 5% level. In the period from 1972 to 1978 these differences were no longer as pronounced. Statistical significance was not reached for the so-called "non-viable malformations" in the period from 1972 to 1978. Only dysrhaphism continued to be significantly more common at the 1+ level. Among these malformations incomplete ventral fusion was significantly more frequent at the 5%.

Cyclopia with trisomy 13 under special consideration of dermatoglyphics and flexion creases.

Report on a case of cyclopia with trisomy 13 (Patau-syndrome with cyclopia completa microphthalmica) and survey of relevant cases published in literature are presented. In addition to a combination of dermatoglyphic patterns and flexion creases typical for trisomy 13, some--so far not described--varieties are shown, above all a missing triradius d on the left and additional digital flexion creases on three phalanges. The possibility that the last mentioned observation might rather be due to population differences is discussed.

[Continuous evaluation of the incidence of congenital malformations (author's transl)].

In a previous retrospective analysis the rate of congenital malformations in the infants delivered at the Department of Obstetrics and Gynecology II of the University of Vienna between 1961 and 1971 was found to be 4.52%. In a retrospective cross-sectional study conducted under identical conditions concerning the period from 1972 to 1978 a rate of 4.47% was computed. As the difference of 0.07% is below statistical significance, the incidence of malformations has apparently remained unchanged, the rate being approximately 4.5%.

[The pregnant and non-pregnant rudimentary horn of an uterus (author's transl)].

In the past 10 years three cases of pregnancy in a rudimentary horn of a double uterus and one case of haematometra were observed at the second university hospital for women in Vienna. The problems of the development of a pregnancy in the uterine cavity of a rudimentary horn which is not connected to the main uterine cavity are discussed. The clinical course of such a pregnancy is discussed reviewing our own cases and those reported in the literature.
