High prevalence of silent chlamydia colonization of the tubal mucosa in infertile women.

Thirty-four asymptomatic infertile patients without distal tubal occlusion were prospectively evaluated at laparoscopy for C. trachomatis infection. Samples were collected from the urethra, the cervical canal, the endometrium, the cul-de-sac peritoneal fluid and the endosalpinx. In 8 patients (24%), C. trachomatis was isolated from at least one site. In 6 patients C. trachomatis was isolated from at least one tube, demonstrating the possibility of silent chlamydial colonization of the tubal mucosa in the absence of signs of active pelvic infection.

[Behavior of ionized calcium in hypertensive and normotensive patients].

Serum concentrations of ionized calcium have been measured in a group of 38 patients with essential hypertension (26 women and 12 men with mean age 61 +/- 12 years) and in a group of 19 normotensives (14 women and 5 men with mean age 70 +/- 12 years). Serum Ca++ levels were significantly lower in hypertensives than in normotensives (4.16 +/- 0.43 mg/dl and 4.46 +/- 0.38 mg/dl respectively, with p less than 0.05). The lower serum concentrations found in hypertensives may be due ions in arterial smooth muscle cells may cause an increase of vascular tone and peripheral resistances. Our results support, in agreement with other Authors, the possible role of ionized calcium in the pathogenesis of essential hypertension.

[Coronary artery dissection secondary to selective coronary angiography].

Coronary artery dissection during coronary arteriography is a rare complication. We describe three cases occurred in our laboratory (0.18% of all the procedures). All the invasive procedures were performed with a femoral approach with Judkins catheters by three different surgeons with limited experience; all dissections involved the right coronary artery. Two patients were men with coronary artery disease and the third was a woman who underwent catheterization for mitral valve disease. An inferior acute myocardial infarction occurred in both men and one of them was surgically treated. No ischemia was observed in the female patient with mitral disease. According to previous reports, we believe that the patients at risk for such a complication (adults with diffuse coronary artery disease or young women with valvular disease and normal coronary arteries) must be more carefully considered. Moreover, in these patients soft-tip Judkins catheters should be used during femoral procedures in the right coronary artery.

Evaluation of Western immunoblotting technique in the serological diagnosis of human syphilitic infections.

Purified human syphilitic antibodies against both 15.5 Kd and 45 Kd treponemal antigens appear T. pallidum specific and do not cross react with antigens possessed by other treponemes (T. phagedenis, T. hyodysenteriae and a human intestinal treponeme). By using Western immunoblotting technique, 107 out of 110 syphilitic patients and 291 out of 294 subjects with serologically positive diagnostic tests for syphilis were found to have in their sera antibodies against a 15.5 Kd specific antigen of T. pallidum. These antibodies were present in 100% of the patient with secondary or early latent syphilis, both untreated and treated, in 98.24% of those with late latent treated syphilis and in 100% of patients with neurosyphilis. On the contrary, they were absent in 47 patients with false positive reactions for syphilis and in 121 healthy blood donors. For these reasons, the demonstration of these kind of antibodies in a patient's serum can be considered of high value in differentiating syphilitic patients from non infected individuals.

In-vitro activity of meropenem against clinical isolates in a multicentre study in Italy.

A multicentre in-vitro study was undertaken to evaluate the susceptibility of bacterial pathogens isolated in different Italian hospitals to meropenem. A total of 1399 aerobic and 452 anaerobic strains was analysed. Comparative agents were imipenem, cefotaxime, ceftazidime, ceftriaxone, piperacillin, ciprofloxacin, gentamicin, amikacin, plus vancomycin when appropriate. The MIC ranges (mg/l) of meropenem were: 0.015-2 for Klebsiella spp., Proteus spp., Morganella morganii and Providencia spp.; less than 0.008-1 for Escherichia coli; 0.016-32 for Serratia spp.; 0.03-2 for Enterobacter spp. and Citrobacter spp.; 0.03- greater than 128 for Acinetobacter anitratus; 0.03-32 for Pseudomonas spp.; less than 0.008-0.5 for Haemophilus spp. and Neisseria spp.; 0.015-64 for Staphylococcus spp.; 0.06- greater than 128 for Enterococcus spp.; less than 0.008-0.25 for Streptococcus spp.; 0.016-8 for Fusobacterium spp.; 0.03-8 for Bacteroides spp.; less than 0.06-0.5 for anaerobic Gram-positive cocci; 0.08-2 for Clostridium spp. Meropenem exhibited superior antibacterial activity against the aerobic and anaerobic strains tested when compared to the other beta-lactam drugs. The new carbapenem was as active as ciprofloxacin and more active than imipenem and the aminoglycosides against Enterobacteriaceae and Ps. aeruginosa. It was also more active than ciprofloxacin against most strains of Gram-positive cocci. Meropenem was slightly less potent than imipenem against staphylococci and enterococci, with the exception of oxacillin-susceptible Staph. aureus against which meropenem and imipenem exhibited similar antibacterial activity.

Comparative analysis of the genomes of intestinal spirochetes of human and animal origin.

The aim of the present work was to compare the genomes of 21 strains of intestinal spirochetes, which were isolated from patients suffering intestinal disorders, with those of Treponema hyodysenteriae (strain P18), the known etiological agent of swine dysentery (bloody scours), and of a nonpathogenic strain (M1) of Treponema innocens. The percent guanine-plus-cytosine value of the 23 DNAs was found to be 25.5 to 30.1, as determined by a double-labeling procedure based on nick-translation by DNA polymerase I. The genome size of two spirochetal strains, of human and porcine origin, was found to be similar (4 x 10(6) base pairs) and close to that of the reference bacterium Escherichia coli (4.2 x 10(6) base pairs). Restriction analysis showed the presence of two modified bases in spirochetal DNA. Methyladenine was present in the GATC sequence of DNA from 15 spirochetes of human origin, and methylcytosine was present in several sequences occurring in all strains. The DNA of T. hyodysenteriae displayed a 30 to 100% homology with respect to that of 21 spirochetes from humans, thus suggesting the occurrence of a genetic heterogeneity in the latter group. These data indicate that the intestinal spirochetes analyzed in the present work are related; hence there is a possibility of domestic animals being reservoirs of microorganisms pathogenic for humans. A classification of intestinal treponemes into subgroups has been proposed on the basis of restriction analysis and hybridization experiments.

[The effect of beta blockers on the QT interval: the possible role of mechanisms other than beta block].

The authors evaluated QT interval changes after 7 and 14 days of treatment with 3 different betablockers--acebutolol, atenolol and penbutolol--in 3 groups of hypertensive patients. Acebutolol (400 mg u.i.d.) prolonged QTc interval in a statistically significant fashion, atenolol (100 mg u.i.d.) induced a significant and constant reduction, whereas QTc interval was shortened by penbutolol only after the first week, shifting toward pretreatment values during the second week. The physiological correlation between QT and RR was slightly modified by acebutolol and penbutolol, but markedly reduced after atenolol. Such differences might be of importance in clinical practice because are likely to be linked to different mechanisms intrinsic to each agent.

Beta 2-microglobulins and renal dysfunction in burned patients.

Studies have been made of the serum and urinary beta-microglobulin levels in 18 burns patients in order to estimate glomerular and proximal tubular dysfunction. The greatest urinary excretion of beta 2-microglobulin was found in severely burned patients during the second and third weeks after injury. Urinary measurements of this protein in patients with renal tubular dysfunction have proved to be of value in the assessment of renal tubular damage.

Delineation of the molecular basis of delta- and normal HbA2 beta-thalassemia.

In this study, we used cloning and sequence analysis to define the molecular defect in two delta-thalassemia genes, one associated with reduced output of delta-globin chains (delta +thal) from a Sardinian and the other with a complete suppression of delta-chain production from the affected locus (delta zerp thal) from a Southern Italian. Sequence analysis of the delta +thal gene showed a G----T substitution at the first nucleotide of codon 27 (delta +27) which produces an amino acid change (Ala----Ser) and presumably activates a cryptic splice site located at this position. Therefore, only a fraction of the transcript is processed from this site, as indicated by the clinical phenotype of delta +thal. DNA sequencing of the delta zero thal gene revealed a T----C substitution at position 1 of IVS-1, which abolishes the splicing at this site and thus leads to complete deficiency of normal mRNA explaining the clinical phenotype of delta zero thal. Oligonucleotide analysis was used to confirm the coinheritance of the delta +27 mutation in a group of Sardinians with thalassemia like phenotype and normal HbA2 level who, on the basis of genetic criteria, were supposed to be double heterozygous for delta-thalassemia and beta-thalassemia. The definition of delta-thalassemia defects in each high-risk area facilitates identification of double heterozygotes for delta- and beta-thalassemia by DNA analysis and may thus improve genetic counseling.

Isolation from faecal specimens of new strains of human rotavirus primarily cytopathic for stationary cell cultures without trypsin.

Eight cytopathic human rotavirus strains were isolated in LLC-MK2 cells and in human embryo fibroblasts. The strains were isolated from faeces collected from pediatric and adult patients. Pretreatment of specimens with trypsin and trypsin incorporation in maintenance medium were not performed. Inoculated monolayers were not subjected to centrifugation and were incubated stationary at 36 degrees C. Viruses were identified by electron microscopy and by fluorescent antibody techniques. It is suggested that these rotaviruses are different from any previously recovered from man.
