[Prevalence of latex allergy among personnel at a hospital].

The aim of this study was to compile a register of all the employees of a University Hospital Centre who complained of dermatosis of the hands due to latex. Recruitment was made by their spontaneous presentation in the service of the Workers Doctor (Médecine de Travail). The prevalence of clinical signs of contact eczema or professional urticaria is 2.3% for all personnel. 2.7% for nurses, 4.4% for care assistants or ancillary staff of the hospital service. In this population, 73% of subjects have shown previous atopy. The positive predictive value of the allergy tests was 51.3%, 2.32% of ASH-ASI, 1.75% of AS, 1.47% of IDE were allergic to latex, about 1.06% of the total personnel. 80% had atopy and 36.66% had a crossed allergy (banana, kiwi, avocat, pollen). Prevalence was zero amongst the administrative officers, but not systematic enquiry was made in the professional category. The orthoergical dermatoses were more frequent amongst the ASH-ASI, though the positive predictive value of tests was less. The considerable exposure to a number of caustic substances, as well as absence of precautions such as rinsing and drying of hands may explain this. All employees with allergy to latex have been declared to have a professional illness a card that mentions this allergy has been given to them.

[Experimental study of peanut sensitization in guinea pigs].

An experimental study of sensitization to peanut has shown that this can be induced in guinea pigs by crushed peanuts ingested daily for three weeks although peanut oil, which is found in some preparations of vitamin D, did not sensitize the guinea pigs, when compared with a control group and a group of guinea pigs that had received only a "flash" dose of 1 ml peanut oil by the oral route.

[The ICAM-1 cell adhesion molecule in asthma and atopic dermatitis].

Measurement of serum ICAM 1, an adhesion molecule, by an enzymo-immunological method, compared with IgE and IFN gamma in three groups of subjects (reference, asthma and atopic dermatitis) shows a statistically significant raised titre in the atopic dermatitis group (377 ng/ml) in comparison with the reference group (300.4 ng/ml). The titre of ICAM 1 in the asthma group (317 ng/ml) was not significantly different from that of the reference. The difference between the latter two groups may be more clearly seen if the measurements were made not in serum but instead in the liquid from broncho-alveolar lavage (LBA) for asthmatics.

[The pork-cat syndrome: RAST inhibition test with Feld One].

This work confirms that the common allergen in subjects who are doubly sensitized to cat and pork is quite different from that of subjects who are mono-sensitized to cat. The common protein is Albumin MW 67 Kda in the pork/cat syndrome, while the major allergen Feld One is characteristic in patients who are monosensitized to cat.

[Diagnostic value of the sulfite skin test].

Nearly 1,200 patients were patch tested with sulfites in Angers during the last two years. A positive patch test reaction was noticed in 2% of cases. The diagnosis of sulfites sensitivity was then established (or not) either with provocation test or sulfites withdrawal during four weeks. The result is a high specificity of the test, 12 patients among 13 having a positive patch test and a sulfite sensitivity.

[The pork-cat syndrome or crossed allergy between pork meat and cat epithelia (1)].

This work is a study of the frequency of the association between sensitivity to pork meat and cat epithelia. Comparison with a reference population that was not sensitized to pork meat, shows that this association is highly significant. This suggests therefore the possibility of a crossed allergenicity. If there is a real crossed allergenicity between pork meat and cat epithelia, we suggest that this crossed allergenicity is without doubt much greater and concerns more the meats and epithelia of mammals.

[Survey of pathologies caused by sulfites in allergology].

Increased use of sulphites as a preservative for foods and drugs is partly the origin of increasing prevalence of allergy to this anti-oxidant. All organs that are concerned with allergic problems may be affected, especially skin and the digestive tract. We have noted a frequent association between the pathology to aspirin (ORL, bronchial, skin) and that of sulphites: 30.8%. The history is fundamental and compensates for the lack of effectiveness of skin tests.

[Diagnostic criteria for latex allergy].

The author has made an observation which seems to illustrate his proposition perfectly. A patient who was the victim of many acute episodes of rhino-conjunctivitis and severe Quincke's oedema has found benefit from two specialised assessments which established a diagnosis of allergy only to latex. The key to this diagnosis rested on unusual reasoning based on a search for a sufficiently "virulent" allergen to be responsible for the severity of the clinical signs. This idea and recent understanding of crossed allergy materials should aid detection of subjects who are allergic to natural latex.

[Dynamic diagnosis of allergy by the sequential measurement of mediators: apropos of a case of food allergy].

Objective improvement in the diagnosis of immuno-allergology can only be obtained by application of reliable and reproducible immuno-biological methods, but until now, only measurements of total and specific IgE can be used and this has certain limitations. Presence of specific serum IgE, correlated with skin tests, favours a sensitization and implies nothing about the responsibility of the allergens. This is why we must consider if a definite improvement of diagnostic methods can be obtained by measurement of mediators. From an observation of food allergy to pork meat, we now show that it is possible to use sequential measurements of the mediators plasma histamine and urinary methylhistamine, ECP and serum tryptase to refine the diagnosis and provide proof of the responsibility of the food allergen. We report here a didactic observation which is characterised by reproducibility and specificity of the measurements. It illustrates the progress in diagnostic methods in allergy: we give a statistical diagnosis by measurement of antibodies and a dynamic diagnosis by measurement of mediators.

[Importance of urinary methylhistamine measurements during allergy provocation tests].

Methylhistamine, histamine's metabolite, was measured in urine by radio-immuno-assay in 79 provocation test. Six of them were positive with clinical symptoms. All of the six were associated with a significant increase of urinary methylhistamine (UMH). Therefore, there is a good correlation between positive provocation tests and increase of UMH level. In these cases, the severity of clinical symptoms is related to the increase of U.M.H.

[Measurement of interferon gamma (gamma IFN) and interleukin 4 (IL4) in asthma and atopic dermatitis].

The recent contributions to the mechanisms of regulation of IgE by cytokines has produced development of kits that are usable in IFN gamma in atopic dermatitis where we have fixed a significant basal level in contrast to a reference group, though there is no possibility of making a modification in asthma. However, this seems to us to be logical since this change of IFN gamma was studied in peripheral blood when the study should be made of broncho-alveolar lavage in asthma. The assay kits for IL4 lack performance and "realism" in their threshold level of detection. This study has also given comparison of the variation of the two cytokines in comparison with total and specific IgE.

[Study of the cell cycle using flow cytometry in drug allergy].

Drug allergy has been studied by flow cytometry with the technique of cellular cycle, of which the principle is based on cellular activity. There were four groups of patients in the study: The first reference groups (A:20 patients) gave evaluation of phase S + G2-M without antigenic stimulation of monodisperse lymphocyte cultures. This was 4%. The second group (B:15 patients) presented a phase S + G2-M of 14.1% with PHA compared with the reference response significant at 2.5%. The third group (C) was composed of 5 wasp venom allergic subjects with cellular activity phase of 17.3% and 5 nonallergic subjects with an almost identical response of around 15%. The fourth group was 5 subjects who were allergic to drugs with clinical history that strongly indicated an allergy. The response to stimulation in these subjects was much greater for each of the drugs than that of a reference subject who was taking the same drug without problem. Wasp venom seems to behave like PHA, as a mitogen. In contrast, the subjects who were objectively and clinically sensitive to drugs, had an overall greater response to them than those shown by references. It is important to have a control for each drug used.

[Sublingual specific immunotherapy for rhinoconjunctivitis caused by grass pollens].

Fifty eight patients with rhino-conjunctivitis caused by grass pollen were included in a double-blind study in which they received, by the sub-lingual route over 5 months, either a solution of purified and standardised allergen or a placebo. Assessment of the effect of this immunotherapy, which was done with drops of Stallergenes "5-grass pollen" was by clinical symptoms and the use of authorized drugs and treatments. Compared with the placebo group, the patients on active treatment showed significantly less (P = 0.05 to P = 0.01) rhinitis symptoms (sneezing and rhinorrhea) and conjunctivitis (reddening and tears) during the pollen season. Consumption of nasal cromoglycate solution, of betamethasone and dexchlorpheniramine was significantly lower (p = 0.01) in the desensitised group. Secondary effects were negligible. From this study, it can be concluded that immunotherapy with grass pollen extract, by the sub-lingual route, of patients with rhino-conjunctivitis who were sensitive to these allergens, is efficacious, easy to do, economic and sure.

[Determination of total IgE (tIgE) and specific IgE (sIgE) using the IMX and MATRIX techniques].

This study, on 185 subjects, has given evaluation of total IgE by the IMX technique in comparison with CAP PRIST, and of the MATRIX* technique for the measurement of sIgE in comparison with the reference technique of CAP RAST**, skin tests and clinical history. The two techniques of CAP and MATRIX are judged to be comparable (Correlation percentage 88.2%). The correlation of MATRIX with skin tests and clinical history is satisfactory (respectively 85.9% and 95%). The results were obtained with four allergens (D. pteronyssinus, Timothy grass, cat and dog). Separate assays confirmed the reliability of the technique.
