[Correlative study of the anti-gluten IgA level in celiac disease. Apropos of some observations].

This work concerns the level of anti-gluten IgA in Coeliac disease, following subjects who have already been diagnosed by clinical signs, biopsy and measurement of anti-gluten IgA. Even if this study had been limited only to a single patient, it would have shown well the clinical sensitivity to these levels, so allowing the following of the development, correlated with improvement of the clinical symptoms with lowering of the level of anti-gluten IgA.

[Assay of IgE specific to dog epithelia by the Pharmacia CAP RAST technique: comparison of the two CAP RAST e2 and e5].

We present a comparative study of measurement of dog-epithelium-specific IgE by two Pharmacia CAP Rast techniques e2 and e5. For both measurements the technique is identical, but the allergen extracts are different: e2 = dachshund extract whilst e5 = a mixture of squames of german shepherd and poodle. We show that the e5-specific IgE seems to be more sensitive than the e2-specific IgE.

[How to diagnose food allergy or the importance of the underestimation of food allergy].

Food allergy has an origin of extra-digestive and/or digestive origin. Diagnosis includes the measurements of total and specific IgE, but especially an elimination test for at least a month, followed by re-introduction of the foods. Finally, the assistance is needed of a provocation test against placebo, under control of mediators of anaphylaxis, plasma histamine, tryptase, urinary methyl histamine. Treatment should include, apart from substitution of the food where necessary, the possibility of acceptance of the food under sodium cromoglycate (Nalcron 100 mg or Intercron 100 mg). This development is very desirable. Depending on the subject, and in a time that is difficult to evaluate, there may develop an immunological tolerance to the food.

[Overview on RAST inhibition].

The technique of RAST inhibition is always useful in two cases: to study cross-reactions between the allergens; for more specific use of allergen indicated for desensitization. The comparison made in this article concerns dog allergens. As can be seen in the tables, diagrams and graphs, RAST inhibition by the dog allergen from the laboratories of DHS-Bayer-Pharma was clearer than that from the Laboratoire Stallergènes.

[Apropos of drug allergy].

Immuno-biological diagnosis of allergies lo medicines can be carried out using a Flow Cytometer and by activating cells such as basophils and lymphocytes with membrane markers. A description is given of two cases of patients allergic to a myorelaxant: Rocuronium, who showed a correlation between the clinical history, skin tests and a positive basophil activation test, when compared to six negative controls.

[General considerations on asthma].

Asthma is an inflammatory illness of which the morbidity and mortality are on the increase. Its diagnosis is by clinical history, exploration of respiratory function, immuno-allergological investigation, skin tests and biological measurements that today are well-developed. Treatment of asthma must first fight the bronchial inflammation before a consideration of the etiological aspects. It uses inhaled corticosteroids (CSI), which is the step of etiological treatment, because of their good dose-efficacy properties. Bronchial penetration of CSI in an aerosol dose is often uncertain because of problems of hand-lung coordination. Beclojet 250, thanks to its Jet inhalation device, gives optimal broncho-pulmonary penetration.

[Importance of plasma (histamine and tryptase) and urinary (methylhistamine) in peri-anesthetic anaphylactic and/or anaphylactoid reactions].

Histamine and tryptase, released during anaphylactoid reactions in anaesthesia, can be measured out by radioimmunoassay, provided that their own pharmacokinetic is respected. For two years, we have used sample kits in order to realize the measuring out of these mediators. The aim of this study was to evaluate the interest of these mediators within investigational procedures for anaphylactoid reactions. Eleven anaphylactoid reactions were observed (0,03%). The early blood samples (the first ten minutes following onset of the reaction) were made only in 36% of the cases. Within the serious reactions (grade III), the raising of tryptase indicates the involvement of mast-cell activation. Within minor clinical reactions (grade I), plasma histamine and urinary methylhistamine were the only mediators detected. In an anaphylactic reaction of grade II, which happened after the administering of vecuronium, tryptase was not detected. Therefore, these mediators give the anaesthetists the possibility to prove quickly the severity of the reactions and to direct the investigations very early towards the right way.

[Western blotting or immunoblotting: application of the Alastat-Alablot to the study of cross reactions between cow's milk and goat's milk].

The Alastat-Alablot was used as immunoblot technique for detecting cross-reactivity between CM and GM. The study was conducted on 9 patients presenting with a food allergy well-documented on the basis of clinical history and elimination diet and re-exposure tests. The Alastat-Alablot technique proved to be sensitive in detecting low levels of specific IgE not detectable by the Cap System. The most frequently encountered proteins that are shared between CM and GM have a molecular weight corresponding to beta-lactoglobulin and casein in the majority of patients.

[Allergy associated with pepper and latex: new cross reaction?].

The aim of this study was to point out the frequency of associated sensitization to rubber latex and to sweet pepper. The study included three populations of patients: Patients allergic to rubber latex, patients sensitized to rubber latex and patients without any clinical symptoms for rubber latex and sweet pepper (negative controls). We pointed out a high frequency of patients sensitized to sweet pepper among the patients allergic or sensitized to rubber latex which lead to the hypothesis of a cross reaction between rubber latex and sweet pepper.

A double-blind, placebo-controlled trial by the sublingual route of immunotherapy with a standardized grass pollen extract.

Fifty-eight patients with well-documented history of seasonal rhinoconjunctivitis caused by grass pollens were allocated randomly on a double-blind basis to receive either sublingual therapy with a solution of purified, standardized allergen preparation (Stallergènes) or a matched placebo for 17 weeks. The assessment of the effect of oral immunotherapy, done with drops of five-grass allergen extract, was on the clinical symptoms and on the medication score of the authorized rescue treatments. The actively treated patients had significantly (P < 0.05 to P < 0.01) fewer symptoms of rhinitis (sneezing and rhinorrhea) and of conjunctivitis (redness and tears) during the pollen season than the placebo group. Consumption of nasal solution of sodium cromoglycate and of betamethasone and dexchlorpheniramine was significantly less in the desensitized group (P < 0.01). Side-effects were negligible. This study concludes that perlingual immunotherapy with grass pollen extract in grass-pollen-sensitive seasonal hay fever and conjunctivitis patients is effective, easy to perform, inexpensive, and safe.

[The pork-cat syndrome or crossed allergy between pork meat and cat epithelia (2)].

Is there a port-cat syndrome in food allergy? Many clinical observations have revealed a frequent association between allergy to cat epithelia in subjects who have allergy to pork meat. This second part, from clinical tests such as skin tests and laboratory tests such as CAP RAST, electrophoresis, Western blot and chromatography, confirms the existence of a crossed reaction between pork meat and cat extract. This crossed reaction is linked with a common epitope. A common protein has been found that has a molecular weight of 67,000 d. for subjects who are sensitive to cat extracts and pork meat.

[Latex and pollinosis].

Through the multidetection test for specific IgE to pneumoallergens (Aero-Matrix Plus, Abbott) our attention has been drawn to polysensitization to pollens. Amongst these pollen tests, plantain seems to be frequently associated with sensitization to latex. The aim of this work at first, is therefore to verify the reality of the frequency of the association between latex and plantain sensitization. However, the conclusions are difficult, since there are no patients who are only sensitive to plantain as sensitivity to this allergen is always found in a pollen polysensitization. at least associated with grass pollens; at most associated with a number of other pollens. The double profile (perhaps plantain + / grass pollen +, or perhaps plantain + / grass pollen + / other pollens +) is determinant in prediction of sensitivity to latex, which is found essentially in the group plantain + / grass pollen + / other pollens +. This study has also shown that amongst the population who have specific IgE to latex (IgEs) (sensitized or allergic): the patients who have a clinical history of allergy to latex have no profile of pollen polysensitization; the non-allergic but sensitive patients (IgE positive, but no clinical signs on contact with latex) show an extreme pollen polysensitization. This poses a problem of different antigenic determinants or perhaps two different pathways to sensitization to latex.

[Total serum IgE levels. Comparative study of the Kallestad and Pharmacia techniques].

The Kallestad technique for measurement of total IgE is based on a "sandwich" technique that uses a solid phase (wells in a sensitised microplate) as a separator. The samples that contain the IgE and monoclonal anti-IgE antibodies (AcM anti-IgE) are incubated in the wells sensitised with goat anti-mouse IgE. The AcM anti-IgE binds the IgG and the complex is then bound to the wells by the anti-mouse IgG. After washing, anti IgE marked with alkaline phosphatase is added and this binds to the previously linked IgE. The intensity of colouration is directly proportional to the concentration of IgE in the sample and is measured by spectrophotometry. The Kallestad technique has very great sensitivity (> 1 KUI/L) for a serum sample of 20 microliter. The measurement is quick, with 2 h incubation. There was good intra-assay reproducibility for values 2 to 1100 KUI/L with CV of 7.6. This good reproducibility was also found in inter-assays for values between 2 and 10 KUI/L. The Kallestad technique was evaluated firstly against the CAP RIA System of Pharmacia and secondly, for values against the kit RIA Ultra. For a population labelled "all comers" with values between 2 to 1100 KUI/L the statistical analysis showed no significant difference between the two techniques. The statistical analysis of the study of low values, made with the RIA Ultra kit showed that there was a significant difference between the two techniques. Exchange of standards between the two methods showed a certain over-evaluation of the values of Pharmacia whilst measurements with the Kallestad standards showed a relative under-evaluation. It should be noted that the comparison has been made between a RIA (Pharmacia) technique and one of EIA (Kallestad).
