Tamoxifen-induced remission of psoriasis.

Psoriasis vulgaris may be worsened or precipitated by numerous factors, including hormonal influences. Several lines of evidence suggest that female sex hormones such as estrogen and progesterone affect this condition. We describe a patient whose psoriasis responded favorably to administration of the antiestrogen compound tamoxifen.

Cigarette smoking-associated elastotic changes in the skin.

BACKGROUND: Clinical features of the skin in persons who smoke include increased wrinkling, gauntness, and discoloration that has been termed smoker's face. The histologic changes in the sun-exposed skin of these patients have not been previously elucidated. OBJECTIVE: The purpose of this study was to assess the amount of elastosis in the sun-exposed skin of smokers and nonsmokers. METHODS: We evaluated the skin from the forehead and cheeks of 17 smokers and 14 nonsmokers for the presence of elastosis. With the use of a computer-generated analysis of tissue sections at 4 different levels, the amount of elastotic material was expressed as an average percent of the field staining for elastic tissue. Patients were also evaluated for the presence of other malignancies, arsenic and radiation exposure, and previous skin cancers. RESULTS: There was a significantly greater amount of elastosis (P < .05) in the skin of patients who smoked compared with those patients who did not. No significant differences were found between the 2 groups with regard to the other parameters evaluated. CONCLUSION: Cigarette smoking is associated with an increase in elastosis, which may contribute to the clinical features of "smoker's face."

Basal cell carcinoma of the lower extremities.

BACKGROUND: The most common malignancy in the USA is basal cell carcinoma. It is most prevalent on the head and neck, but can occur elsewhere. We sought to determine the frequency of basal cell carcinomas arising on the lower extremities, as well as the gender of affected patients, the histologic subtype, and the specific sites of involvement. METHODS: We reviewed cases submitted to the Vanderbilt Dermatopathology Service between 1994 and 1997 which were diagnosed as basal cell carcinoma. A subset of 150 cases which arose on the lower extremities and a control group of 150 cases which arose at other anatomic sites were studied further. RESULTS: A significantly greater number of basal cell carcinomas of the lower extremities arose in women, were of the superficial subtype, and were found below the knee; 59% of basal cell carcinomas in both sexes arose on the right limb. CONCLUSION: Basal cell carcinomas of the lower extremities occur more often in women in the USA and are of the superficial subtype. This may be due to differing patterns of dress and exposure to the sun.

Recalcitrant acne vulgaris secondary to a dental abscess.

This case report describes a middle-aged man with acne that was recalcitrant to numerous medications, including three courses of isotretinoin. His condition cleared after an infected tooth was removed and recurred when another tooth became carious. Acne vulgaris is associated with several immunologic responses including the production of antibodies against Propionibacterium acnes, the gram-positive bacteria found in acne lesions. We believe that the presence of our patient's dental infections provoked a follicular inflammatory response resulting in his recalcitrant acne.

Basal cell carcinoma with ossification.

BACKGROUND: Osteoma cutis, the presence of lamellar bone in the skin, is relatively common. This process is divided into two categories: primary osteoma cutis and secondary osteoma cutis. OBJECTIVE: The purpose of this study was to describe the clinical and histopathologic features of patients in whom lamellar bone developed in cutaneous basal cell carcinoma. METHODS: We evaluated the features of five cases of osteoma cutis associated with basal cell carcinoma and obtained detailed clinical information from those patients. RESULTS: All five patients had significant underlying medical conditions, including two patients who were receiving interferon alfa-2b therapy. Three patients had been previously treated with electrodesiccation and curettage. The amount of sun exposure experienced by these patients varied. Histologically, the basal cell carcinomas were of the nodular or micronodular variety. Bone was found both in the stroma and intratumorally. CONCLUSION: The presence of bone within basal cell carcinomas is not uncommon and may be more prevalent in patients with an underlying medical disorder.

Colchicine in dermatology.

Colchicine is a medication most often used to treat symptoms of gout. This drug has also been shown to have beneficial effects on cutaneous conditions, including leukocytoclastic vasculitis, psoriasis, and Sweet's syndrome. Colchicine inhibits the function of polymorphonuclear leukocytes, and dermatoses with a strong presence of these cells may benefit the most from the administration of this medication. A review of the pharmacology, mechanism of action, and adverse reactions of colchicine is also presented.

Intravascular large cell lymphoma: a patient with asymptomatic purpuric patches and a chronic clinical course.

Intravascular large cell lymphoma (malignant angioendotheliomatosis) is a rare, multifocal, intravascular neoplasm of lymphoid cells that preferentially involves the vasculature of the skin and central nervous system. We describe a 54-year-old man with asymptomatic purpuric patches on the lower extremities for 10 years duration and a more recent lesion on the right arm. A biopsy specimen showed intravascular collections of tumor cells with irregular nuclear contours and prominent nucleoli. These cells were leukocyte common antigen (CD45), CD20, and CDW75 positive, but CD3, CD43, CD45RO, and cytokeratin negative. Polymerase chain reaction analysis of the skin for immunoglobulin heavy chain gene rearrangement detected a clonal population of B cells, supporting the diagnosis of a B-cell lymphoma. Peripheral blood showed no abnormal circulating cells. This case of malignant angioendotheliomatosis is unusual for its prolonged clinical course and presence of purpuric patches.

Immunoperoxidase evaluation of lichen planus biopsies for hepatitis C virus.

BACKGROUND: Lichen planus is a papulosquamous dermatosis which has recently been linked to infection with hepatitis C virus. It is unclear whether or not viral antigens may be present in the cutaneous lesions of lichen planus. MATERIALS AND METHODS: Twenty-five paraffin-embedded samples of glabrous lichen planus were evaluated using immunoperoxidase staining for the presence of hepatitis C virions. Control tissues consisted of hepatitis C-infected hepatic tissue (n = 2), normal hepatic tissue (n = 2), normal human skin (n = 1), and two cutaneous biopsies of lichen planus from persons known to be infected with hepatitis C. RESULTS: The sections of hepatitis C-infected liver tissue stained positive for hepatitis C virions. The 25 biopsies of glabrous lichen planus, the two biopsies of lichen planus from hepatitis C patients, the two sections of normal liver, and the one normal skin sample all failed to take up the stain. CONCLUSIONS: Cutaneous lesions of lichen planus are more probably reactive to the underlying infection than a manifestation of skin involvement by this disease. This theory is supported by the histologic findings in a lichenoid drug eruption, which are virtually identical to those of idiopathic lichen planus. Insufficient sensitivity by the immunoperoxidase procedure used is a possible explanation for our results; however, it appears more probable that no virus exists at the sites of cutaneous involvement.

Heterocyclic nucleoside analogues by cycloaddition reactions of 1-vinylthymine with 1,3-dipoles.

1,3-Dipolar cycloaddition of 1-vinylthymine to azides, nitrile oxides, nitrones and nitronates has been investigated as a route to heterocyclic nucleoside analogues in which the nucleoside ribose moiety has been replaced by an alternative heterocycle. Reaction of 1-vinylthymine with highly reactive nitrile oxides affords 1-(isoxazolin-5-yl)thymine products in excellent yield at room temperature. The less reactive nitrone dipoles undergo cycloaddition to 1-vinylthymine at elevated temperature to afford 1-(isoxazolidin-5-yl)thymine cycloadducts in good-to-moderate yields, but show a tendency to eliminate thymine from the cycloaddition products over long reaction times. Azide cycloadditions to 1-vinylthymine proceed only under forcing conditions to which the fragile triazoline products are unstable.

Lichenoid dermatitis after consumption of gold-containing liquor.

Medicinal gold has a well-known side effect profile that includes mucocutaneous eruptions. We describe three patients with a pruritic dermatitis that began after consumption of a gold-containing alcoholic beverage. Blood and urine gold levels, chemistry panels, hepatitis screens, skin biopsies, and patch tests were performed. The gold-containing liquor was analyzed for the presence and quantity of gold. The liquor consumed by all of the patients was a cinnamon schnapps with free-floating gold-colored flakes. Gold is present in the liquid portion of this liquor and in the solid flakes. Elevated levels of gold in the urine and blood were present in one patient 3 months after last drinking this beverage. Another patient had a positive patch test to gold sodium thiosulfate. All patients experienced improvement of their dermatitis after they stopped drinking the gold-containing liquor.

Rickettsialpox.

Rickettsialpox is a member of the spotted-fever group of the rickettsioses and results from an infection with Rickettsia akari. This microbe is transmitted by the bite of the house-mouse mite Liponyssoides sanguineus. Patents experience fevers, sweats, headaches, and a vesicular eruption over the trunk and extremities. The palms and soles are spared. An eschar results at the spot of the mite bite. Tetracycline is the treatment of choice.

False-positive polymerase chain reaction results for human papillomavirus in lichen planus. Potential laboratory pitfalls of this procedure.

BACKGROUND: The polymerase chain reaction (PCR) is a common tool in laboratory evaluation of tissue samples. It has been particularly useful in testing for nucleic acids of infectious organisms implicated in the pathogenesis of cutaneous diseases. OBJECTIVE: Our purpose was to evaluate archival biopsy material (paraffin-embedded) of lichen planus for human papillomavirus (HPV) DNA with PCR and in situ hybridization (ISH). METHODS: Tissue sections were obtained from 13 specimens of lichen planus and 10 control tissues. Standard methods for PCR and ISH were used to evaluate the presence of HPV DNA. RESULTS. Initial PCR results demonstrated HPV DNA in 11 of 13 specimens and in none of the control tissue. However, testing for specific HPV types revealed nucleic acid contamination. ISH was negative in all specimens. CONCLUSION: Accurate evaluation of tissue with PCR is difficult because of the procedure's profound sensitivity. Positive results reported in the literature should be viewed with caution. Potential causes for false-positive and false-negative results should be considered.

An evaluation of the accuracy of residency applicants' curricula vitae: are the claims of publications erroneous?

Competition for residency positions in dermatology is intense. Training programs in other disciplines have described significant misrepresentation among their applicants in claims of research and publication. An evaluation was made of the curricula vitae for the 138 applicants with completed applications for our 1994 residency match. Cited articles were placed in four categories: "published," "in print/in press," "submitted for publication," and "in preparation/in progress." We found significantly less misrepresentation than has been described in other specialty training programs. Simplification of the way in which applicants verify their bibliography may reduce erroneous citations and remove implications of willful misrepresentation of academic achievements.
