A 17 year old male patient presented with short stature and delayed puberty. Investigations showed normal thyroid function and intact TSH response to TSH-releasing hormone (TRH). Although basal levels of LH were low, both LH and FSH rose following the administration of LH-releasing hormone (LHRH). ACTH secretion assessed indirectly by the cortisol response to insulin hypoglycemia was normal. Growth hormone levels increased following the onset of sleep, as well as after the administration of insulin, L-dopa and L-arginine. Basal levels of prolactin were low (2-5 ng/ml) compared with 5-12 ng/ml in controls. There was a markedly impaired prolactin response to TRH (maximum rise above basal values of 3 ng/ml compared to a rise of 12-29 ng/ml in controls). Prolactin levels did not rise after the administration of chlorpromazine or L-arginine. There was some suppression of prolactin levels after L-dopa. Similar patterns of prolactin are seen in panhypopituitarism, where they are usually associated with other hypophyseal hormonal deficiencies. The diminished prolactin reserve demonstrated in this subject in the presence of intact function of the remainder of the anterior pituitary is compatible with the diagnosis of diminished prolactin reserve.

The incidence of maternal bleeding complicating the pregnancies of 326 children examined at the Jerusalem Community Center for Child and Family Development is surveyed. A history of bleeding during pregnancy was more frequent in children with congenital anomalies of the central nervous system (and/or psychomotor retardation) (33 per cent) and in children with congenital anomalies of other systems (29 per cent) than in control children (12 per cent). The implications of these findings are discussed in relation to placental pathophysiology and the pathogenesis of threatened abortion.

Spasticity and severe psychomotor retardation in a 2-year-old girl, apparently in expression of chronic progressive encephalopathy, was found in association with prolinuria, hydroxyprolinuria and hyperglycinuria. The pattern and levels of these amino acids in plasma proved to be normal, however, as was their intestinal absorption as well as their concentration in cerebrospinal fluid. The proband appears to be homozygous for iminoglycinuria with an apparent inborn defect of the renal tubular transport system specific to these amino acids. An isolated hyperglycinuria, however, was traced in several obligate heterozygotes in this family. The possibility of a more direct causative link between this anomaly and her clinical manifestations is discussed. Where instances of iminoglycinuria occur in association with cerebral pathology of this type, an analogous impairment of a correspondingly specific cerebral amino acid transport system is postulated.

The effect of a dexamphetamine-amylobarbitone mixture (1;6.5 by weight) on a sensory-contingent bar pressing (SCBP) for different sensory changes was investigated. The aim was to examine the role of (a) reward value of contingent change per se (non drug) and (b) stimulus properties in enhanced SCBP under the drug mixture. The sensory reinforcers used were light Onset, light Offset, and sound offset combined with the light changes contemporaneously. In the first two phases the sensory stimuli were given as continuous reinforecement and in the third phase on an FR2 schedule. The results from the first two phases were consistent with the notion that the drug mixture responding under the drug was enhanced was not significantly related to the kind of sensory change. Instead, responding under the mixture was enhanced in proportion to the level of non drug responding, especially in Phase 1. However, in Phase 3, SCBP was enhanced proportionately more, to a significant extent, in those treatments receiving the combined light and sound change. It was concluded that properties of the sensory stimuli other than their reward value per se (non drug) may also be related to the extent of SCBP enhancement under the drug mixture. In particular, properties of the sound offset reinforcer appear to make it more amenable to enhancement under the drug than the two light reinforcers.
The bilirubin-binding affinity of infant serum, as determined by Sephadex gel filtration, began to increase during the first week of life and continued to increase gradually until the age of 5 months, when it reached that of adult serum. This increase might be due to a transition from a fetal to an adult type of serum albumin and/or to a decrease in the concentration in serum of the substances thought to compete with bilirubin for binding to albumin.
