Referral follow-up by clinicians in a community mental health center.

In 1973 a study was conducted at the Western Psychiatric Institute in Pittsburgh to assess the extent of discrimination by clinicians against chronically ill clients in establishing initial treatment arrangements. Forty referrals (all simulated) were assigned to the center's five treatment teams; 20 of the referrals suggested a mild disorder, and 20 suggested a severe and chronic disorder. Premature termination of the study reduced the sample size from 40 to 29, and the findings regarding discrimination were inconclusive. However, the study revealed the unexpected finding that 34 per cent of the referrals received no follow-up. The author discusses the implications of that finding in relation to the basic principles of community mental health, and also to the large amounts of resources allocated to increasing service utilization in the center under study.

Urinary tract diseases in adolescence: a radiological and clinical view.

A total of 2,003 adolescent in-patients aged 14-19 years were studied. Of these 801 (40 per cent) were found to have urinary tract disease. In 224 patients (28 per cent) a previously undetected urinary tract anomaly was diagnosed: this number including 59 per cent of girls and 41 per cent of boys. This is the first analysis showing such a high frequency of urinary tract anomalies in clinical and radiological investigations during the period of adolescence. Of particular importance are minor anomalies, in so far as these are responsible for disorders to the flow of urine. It is thought that urinary tract anomalies manifest themselves in adolescence because of increased stress and a different pattern of life, as compared with that in the preceding era of childhood. The present paper shows the cardinal importance of radiological examination and of the radiologist's role in evaluating the dynamics of the process and the prognosis of the disease and, in helping to assess the future potential for physical stress in either work or sport.

Turner's syndrome with XO/XXp- karyotype: a case report.

An unusual case of Turner's syndrome is presented. The case highlights the importance of chromosome analysis in patients presenting with primary amenorrhoea. This patient demonstrates the effects of mosaicism and the importance of the long and short arms of the X chromosome. Deletion of the short arm has only been reported in 2 previous cases of Turner's syndrome. The absence of the short arm is believed to be responsible for the shortness in stature in these patients.
