Suicide among psychiatric hospital in-patients.

We examined for risk factors for suicide among psychiatric in-patients by comparing 37 in-patients from an Ontario Provincial Psychiatric Hospital who had committed suicide with 37 age and sex matched in-patient controls. Significantly more of the suicide victims had made a previous suicide attempt (62.2 v. 35.1%), suffered from schizophrenia (75.7 v. 35.1%), were involuntary at their last admission (70.3 v. 43.2%) and lived alone (70.3 v. 43.2%). Only six patients committed suicide on the ward. Almost a third of the patients, the majority schizophrenic, committed suicide after having been in the hospital for more than a year. These results suggest that in the psychiatric hospital setting the in-patient at risk for suicide has previously exhibited suicidal behaviour, suffers from schizophrenia, was admitted involuntarily, lives alone and that the risk of suicide may remain high among long-stay schizophrenics.

Methodological aspects of life events research in people with learning disabilities: a review and initial findings.

A review of the life events studies relevant to people with learning disabilities is discussed. Issues surrounding the collection of information by using a structured interview method as set out in the Life Events and Difficulties Schedule Manual and the adjustment of threat rating to accommodate the perspective of people with learning disabilities are presented as a result of conducting a pilot study. The importance of using a probing interview style in contrast to a checklist method and eliciting the parents/carers' perception of events is emphasized.

Blood loss in termination of early pregnancy with mifepristone and gemeprost.

The combination of an antiprogestogen with prostaglandin has been shown to be an effective alternative to surgical procedures for pregnancy termination in the first 49 days of amenorrhoea and has been available in France since late 1988. Mifepristone when used alone caused poor efficacy with unacceptable vaginal bleeding. The addition of a prostaglandin analogue (sulprostone, gemeprost) produced acceptable efficacy with less bleeding. The present study provides a quantitative assessment of blood loss in a sample of 20 women undergoing medical termination of early pregnancy (amenorrhoea < or = 49 days). Vaginal bleeding occurred between days 3 and 15 of commencing therapy. The median number of days of bleeding was 4 with a median blood loss of 91.5 mL. The mean blood loss was 136.8 mL (+/- SD 159.2) but this did not adversely affect the hemoglobin level in the volunteers. Most patients describe the experience as that similar to a heavy period and viewed the procedure positively especially since they are spared the anaesthetic risks and hospitalization attendant with surgical abortion. Caution is still however advised as there may be the occasional subject with excessive haemorrhage (as in this study where one woman bled a total of 761.4 mL) and close supervision by a physician is essential.

The relocalization of v-Rel from the nucleus to the cytoplasm coincides with induction of expression of Ikba and nfkb1 and stabilization of I kappa B-alpha.

The v-Rel oncogene induces the expression of major histocompatibility complex class I and II proteins and the interleukin-2 receptor more efficiently than does c-Rel (R. Hrdlicková, J. Nehyba, and E. H. Humphries, J. Virol. 68:308-319, 1994). The kinetics with which these immunoregulatory receptors are induced in B- and T-lymphoid cell lines and chicken embryo fibroblast cultures expressing c-Rel or v-Rel have been examined. v-Rel induced the expression of major histocompatibility complex classes I and II and interleukin-2 receptor more efficiently than did c-Rel at later times after infection. In all three cell types, this increased efficiency was accompanied by a shift in the majority of v-Rel from the nucleus of the cytoplasm. The concomitant relocalization of v-Rel was also demonstrated during the in vitro transformation of spleen cells. The translocation coincided with increased steady-state levels of I kappa B-alpha. Coninfection by retroviral vectors expressing v-Rel, I kappa B-alpha, or NF-kappa B1 demonstrated that either I kappa B-alpha can contribute to the shift of v-Rel to the cytoplasmic compartment. The induction of nfkb1 and Ikba mRNA and the stabilization of I kappa B-alpha by v-Rel were shown to be responsible for these effects. In comparison with c-Rel, the expression of v-Rel was associated with lower levels of transcription of these genes. However, the ability of v-Rel to stabilize I kappa B-alpha remained unchanged. The ability of v-Rel to stabilize I kappa B-alpha but poorly induce Ikba mRNA expression relative to c-Rel may play a role in regulating gene expression, thereby leading to transformation.

Attempted suicide among living co-twins of twin suicide victims.

OBJECTIVE: The authors hypothesized that significantly more living monozygotic than dizygotic co-twins of twin suicide victims would themselves have attempted suicide. METHOD: They determined the absence or presence of an attempt at suicide at any time among 26 living monozygotic co-twins and nine living dizygotic co-twins of twins who had committed suicide. RESULTS: They found that 10 of the 26 surviving monozygotic co-twins but none of the nine surviving dizygotic co-twins had themselves attempted suicide. CONCLUSIONS: Although monozygotic and dizygotic twins may have some differing developmental experiences, these results contribute to findings of previous studies of suicide in twins in suggesting that genetic factors may play a role in suicidal behavior.

Survivability and leucocyte migration inhibition in mice immunized with cryotreated ascites fibrosarcoma cells using different freeze-thaw cycles.

Attempts have been made to assess as to what extent in vitro assay of cellular immunity, e.g. leucocyte migration inhibition (LMI) in mice immunized with different freeze-thaw cycles could reflect host resistance in vivo. While survivability of animals improved significantly by immunization with single cycle (P < 0.05) to three cycle (P < 0.001) and programmed three cycle (P < 0.001) cryo-treated tumor cells compared to controls, the percentage LMI in the same groups of animals decreased progressively. The KCl(3M) extracted tumor cell protein (antigen) of both viable and cryo-treated cells showed a progressively increased protein concentrations per 1 x 10(6) tumor cells with viable cells being least and programmed three cycle cryo-treated cells highest. The apparent discrepancy observed between percentage migration inhibition and survivability may be due to the fact that (1) survivability is a function of body's total immune response while LMI represents the response of one effector limb only; (2) immuno-regulatory mechanisms depend on a balance between activation and suppression and suppressor cells being more sensitive and of shorter life span, affect migration inhibition but not the survivability; (3) cryo-treatment alters tumor cell surface antigen affecting immunological balance; and (4) suppressor and antitumor activities against antigenic stimulation develop simultaneously in different organs and LMI performed with sensitized splenic cells, where, perhaps, suppressor cell activity dominates.

[Role of extracorporeal shock wave lithotripsy in the treatment of common bile duct and intrahepatic calculi].

The management of intrahepatic and common bile duct stones has been modified by the advent of endoscopic sphincterotomy and percutaneous extraction through a T-tube tract or transhepatic access. Occasionally, nonoperative extraction is incomplete. The use of extracorporeal lithotripsy is reviewed in this setting. From May 1990 to February 1994, 18 patients (age 68.4 +/- 4.6 years) were treated by extracorporeal shockwave lithotripsy combined with endoscopic sphincterotomy and retrograde extraction or percutaneous approach. 72% of patients had previously undergone a cholecystectomy and 44% exploration of the common duct. Patients were submitted to 1.56 +/- 0.17 session of lithotripsy (5.546 +/- 701 shockwaves). Hospital stay was 19.5 +/- 3.3 days. After the lithotripsy, 1.17 +/- 0.19 endoscopic or percutaneous procedures per patient were necessary to clear the biliary tract. Seventy-eight percent of patients became stone-free. The five failures were treated by endobiliary prosthesis (n = 4) or cholecystectomy and bile duct exploration (n = 1). Lithotripsy in association with the usual therapeutic modalities contributes to clearing the bile duct from stones and avoids surgery in the majority of patients. A multidisciplinary approach is necessary in order to obtain those results.

Fine needle aspiration cytology of bone and joint lesions: an assessment.

Fine needle aspiration cytology and subsequent correlation with histology and therapeutic follow-up was done in 198 cases of bone and joint lesions. Overall accuracy was found to be 87.34%; false diagnosis was recorded in 4.5% of cases; 8.08% cases were excluded from correlation as they did not turn up for further treatment or follow-up after the diagnosis. False diagnosis was encountered mostly with inflammatory lesions and primary neoplasms, diagnosing osteosarcoma and Ewing's sarcoma as chronic inflammatory lesion, chondroma as chondrosarcoma and vice versa. The possible cause of default has been discussed. The benign cystic lesions could be diagnosed with almost 100% accuracy by fine needle aspiration cytology where 10% cases were excluded as they could not be followed up.

Humoral immune responses to the Plasmodium falciparum antigen Pf155 RESA in adults with differential clinical conditions from an Indian zone where malaria is endemic.

Ring infected erythrocyte surface antigen of Plasmodium falciparum (Pf155/RESA) has been considered as a vaccine candidate. However, the relative immunogenicity of this antigen has not been studied in Indian populations. Pf155/RESA was investigated for its immunogenicity by studying humoral immune responses against Pf155/RESA and Pf155/RESA derived peptides (P1, P2 representing immunodominant epitopes from the 3' and P3 from the 5' repeat regions) by erythrocyte membrane immunofluorescence (EMIF) assay and by enzyme linked immunosorbent assay (ELISA) in P. falciparum primed donors living in hyperendemic malarious areas (Orissa State, India) where P. falciparum infections are highly prevalent. Subjects of different clinical status namely acute (A), clinically immune (CI) and acute with history of repeated P. falciparum infections (R) were included in this study. All the donors were seropositive against the crude antigen. There was considerable variation in the responses among the donors. While humoral responses in the plasmas against the P2 peptide (EENV)4 were significantly higher in magnitude and in frequency in the CI donors than in the A donors, no positive response was seen in the R donors. The responses to the peptides P1 (EENVEHDA)2 and P3 (DDEHVEEPTVA)2 were poor both in the A and in the CI groups. Whereas, most of the R donors were seropositive against the P3. The present results indicate that Pf155/RESA contains B cell epitopes which were recognized differently by the immune system of individuals living in malaria-hyperendemic areas of India who have been primed by natural infection. Our studies also suggest that in order to investigate the possible functional role of a given antigen, study of immune responses against the antigen in donors of different clinical status may be useful.

Tuberculous cervicitis: a clinicopathological and bacteriological study.

Seven cases of tuberculous cervicitis were detected out of 91 culturally and histologically established cases of uterine tuberculosis. There was simultaneous infection of cervix and endometrium in 4 cases and only cervical lesion in 3 cases. Two women gave a definite past history of primary extragenital tuberculosis-peritonitis in one and pulmonary lesion in the other. Clinically, cervix was predominantly hypertrophied in 4 cases and predominantly ulcerative in 3 cases. Acid-fast bacilli (Myco tuberculosis) were recovered from 5 cases, in abundance from 2 hypertrophied lesions and in sparse number from the 3 predominantly ulcerative lesions. Histological examination revealed pseudo-epitheliomatous hyperplasia with poor cellular response in the hypertrophied cervix and non-caseating tuberculous granuloma in predominantly ulcerative cervix. All the patients were from Darjeeling hills where endemic tuberculosis is high. An awareness of this entity is necessary while dealing with the cervical lesion of these patients.

Application of simple peptide ELISA for stratification of malaria endemicity.

A serological investigation was conducted in north India to determine malaria endemicity based on the antibody levels against a nonapeptide RI (EENVEHDA-Cys) from the P. falciparum antigen Pf 155/RESA. P. falciparum sonicated crude antigen was also used in the study. Subjects of all age groups from various strata of malaria endemicity were included in this study. A total of 4273 finger prick blood samples from 49 villages of five districts were collected during January to March 1991 which is a non-transmission season and the antibody levels were estimated by ELISA. Although a good correlation was found between the antibody titre to the RI peptide and that to the crude antigen, the most consistent results were obtained with the RI peptide. When compared with the annual parasite index (API) values, an established method for defining malaria endemicity, mean anti-RI antibody titres obtained from several villages within a single zone correlated (r = 0.94, p = 0.023) with mean API value of the area. Thus, our results suggest that by using the RI peptide as antigen in seroepidemiology, it is possible to stratify malaria endemicity. We didn't distinguish between endemicity of Pv and Pf since each area experiences cycle of transmission of P. vivax followed by P. falciparum and our sera were from individuals having no fever.

Detection of type specific human papillomavirus (HPV) DNA in cervical cancers of Indian women.

Commercial Vira Pap and Vira Type kits of Life Technologies Inc., U.S.A., were used to determine prevalence and type specific distribution of human papilloma viruses (HPV) in 18 biopsy samples of cervical carcinomas and 26 specimens of exfoliated cervical cells (swabs). The women were either attending out-patient's department of a suburban hospital or a cancer hospital in Calcutta, India. HPV DNA was detected in 9 biopsy specimens but not in any of the cervical swabs. Five of the 9 HPV positive biopsies had HPV DNA type 16/18. Neither HPV 6/11 nor HPV 31/33/35 DNAs were detected in any of these 9 specimens. Results indicated possible presence of HPV DNAs of types other than the above in at least 4 specimens.

Mutational spectrum induced in Saccharomyces cerevisiae by the carcinogen N-2-acetylaminofluorene.

The spectrum of mutations induced by the carcinogen N-2-acetylaminofluorene (AAF) was analysed in Saccharomyces cerevisiae using a forward mutation assay, namely the inactivation of the URA3 gene. The URA3 gene, carried on a yeast/bacterial shuttle vector, was randomly modified in vitro using N-acetoxy-N-2-acetylaminofluorene (N-AcO-AAF) as a model reactive metabolite of the carcinogen AAF. The binding spectrum of AAF to the URA3 gene was determined and found to be essentially random, as all guanine residues reacted about equally well with N-AcO-AAF. Independent Ura- mutants were selected in vivo after transformation of the modified plasmid into a ura3 delta yeast strain. Plasmid survival decreased as a function of AAF modification, leading to one lethal hit (37% relative survival) for an average of approximately 50 AAF adducts per plasmid molecule. At this level of modification the mutation frequency was equal to approximately 70 x 10(-4), i.e. approximately 50-fold above the background mutation frequency. UV irradiation of the yeast cells did not further stimulate the mutagenic response, indicating the lack of an SOS-like mutagenic response in yeast. Sequence analysis of the URA3 mutants revealed approximately 48% frameshifts, approximately 44% base substitutions and approximately 8% complex events. While most base substitutions (74%) were found to be targeted at G residues where AAF is known to form covalent C8 adducts, frameshift mutations were observed at GC base pairs in only approximately 24% of cases.(ABSTRACT TRUNCATED AT 250 WORDS)

The use of peptide ELISA in determining malaria endemicity.

A study was undertaken with the objective to find out whether a reliable transmission index could be established by a simple peptide ELISA. This would be important to evaluate the success of the bioenvironmental malaria control programmes implemented in endemic areas like the Haldwani and Shankargarh districts of Uttar Pradesh, India. We have estimated antimalarial antibodies against three synthetic peptides derived from blood stage antigens of Plasmodium falciparum and against P. falciparum crude antigen by enzyme-linked immunosorbent assay (ELISA), in children and adults. We have found that while all the peptides react with serum antibodies, as does the whole crude blood stage antigen, a nonapeptide from RESA antigen gave the most uniform results. Our results suggest that synthetic peptides, which are easy to obtain and can be fully characterized, may offer a simple way to measure malaria endemicity.

Complete androgen insensitivity due to a splice-site mutation in the androgen receptor gene and genetic screening with single-stranded conformation polymorphism.

OBJECTIVE: To characterize the genetic defect in a family with complete androgen insensitivity syndrome and to determine whether single-stranded conformation polymorphism (SSCP) can be used to detect subtle mutations in the androgen receptor (AR) gene. DESIGN: Amplification, subcloning where appropriate, and sequencing of the AR gene in members of the affected family and to use SSCP to differentiate rapidly mutant from normal alleles. SETTING: Reproductive endocrinology clinic and laboratory in a university hospital. PATIENTS: A family of which two sisters (46XY) have complete androgen insensitivity syndrome. RESULTS: A novel single base (G --> A) mutation in the exon G-intron 7 junction of the AR gene caused an abnormal donor splice site leading to complete androgen insensitivity in both affected siblings. Their mother was demonstrated to be the heterozygous carrier of this mutation while the other two males in the family carried the normal allele. Single-stranded conformation polymorphism proved useful for defining the normal, mutant, and heterozygous carrier status of each member of this family. CONCLUSIONS: This new mutation of the human AR gene illustrates the importance of exon G in receptor function. Single-stranded conformation polymorphism is a simple and rapid screening technique that can be used to detect unknown subtle mutations in the AR gene.

Iatrogenic injury to the bile duct: a working classification for radiologists.

PURPOSE: To assess the usefulness of the Bismuth classification method in the preoperative localization of iatrogenic bile duct lesions with cholangiography and to correlate these cholangiographic findings with surgical findings. MATERIALS AND METHODS: The records of 33 patients who underwent open or laparoscopic cholecystectomy and who sustained injuries to the biliary tract during the course of these procedures were reviewed retrospectively. The accuracy of the cholangiographic localization of bile duct injury was assessed with the Bismuth classification method, which is based on the localization of the traumatic lesion according to the distance from the biliary confluence. RESULTS: An exact correspondence between cholangiographic and surgical findings was found in 85% of the subjects. A minimal discrepancy was found in the remainder. There was no interobserver variation. CONCLUSION: The use of the Bismuth classification method appears to be an accurate and practical method for the grading of postoperative bile duct lesions with cholangiography.

Are cerebrospinal fluid or urinary monoamine metabolite measures stronger correlates of suicidal behavior in depression?

A series of depressed patients had monoamine metabolites measured in cerebrospinal fluid (CSF) and urine. We wished to see if CSF or urinary measures were more strongly associated with suicidal behavior in depression. Using a variety of graphical and analytic statistical approaches we found that the urinary measures of dopamine metabolism had the strongest association with suicide attempts in depression. These surprising results suggest that selected peripheral correlates of monoaminergic neurotransmission deserve further study in suicidal behavior.
