The validity of individual radiological signs for the differentiation of osteoplastic bone metastases from osteitis deformans has been investigated quantitatively (103 skeletal metastases from carcinoma of the prostate, 45 cases of Paget's disease, two osteoplastic bone metastases from a carcinoma of rectum and bronchus). The similarities were demonstrated by three cases observed by us. Problems in the quantitative evaluation of the radiographs are discussed.
In surgical treatment of Varicocele compete the ligation of the V. testicularis (Ivanissevich/Bernardi) with the ligation of the A. and V. testicularis (Palomo). Because there are doubts against throttling the arterial blood supply of a previous damaged testis, the effect of ligation of A. and V. testicularis on the testis was studied and compared with the ligation of the vein alone. On 18 adult rabbits either the Palomo operation was performed on one side, leaving the contralateral side as control, or on one side only the vein was ligated and on the other side artery and vein were ligated, permitting, so a direct comparison on the same animal. All the testes were examined histometrically. In the control group the diameter of the tubuli increased from 218 mu after 3 months to 253 mu after 10 months. In the Palomo-group the diameter decreased from 153 mu after 3 months to 82 mu after 6 months, and increased again to 153 mu after 10 months. In the Bernardi-group no differences to the normal testes were found.
Persorption is the excretion in urine of particles that have been absorbed (persorbed) from the gastrointestinal tract. In experiments with rats the effect of Herbst was combined with the production of renal calcium oxalate sediments by the oral administration of ethylene glycol and acidification with ammonium chloride. We used this method to investigate whether persorbed corpuscula may represent nuclei of crystallization in the formation of renal calculi. Lead sulfide, zirconium silicate and 45Ca-labeled calcium oxalate crystals were used as persorbable model bodies. It became apparent that orally supplied lead sulfide and zirconium silicate crystals could be found in the urine and the kidneys but they did not function as crystallization centers in the calcium oxalate sediments. However, radioactive labeled calcium could be demonstrated histoautoradiographically within the renal concrements. Our experiments showed that persorbed calcium oxalate crystals can work as nuclei for stone formation.

Both in a control group and in 154 patients with malignant tumors of the urogenital tract in the serum- and urinary levels of the carcinoembryonic antigen (CEA) have been ascertained by a radioimmunoassay. Serum-CEA-values lay above normal range in 71%, and urinary CEA-levels exceeded it in 46%. Only in the case of the carcinoma of the bladder a correlation was found between the CEA-curves and the clinical findings following tumor therapy. Up to the present, the CEA-test cannot be recommended as a universal screening method, but may be useful means for diagnostics and case controls, above all, with regard to the carcinoma of the bladder.
Since 1967 we have been using preoperative radiation therapy for hypernephroma as proposed by RICHES. Radiation therapy, to include the para-aortic lymph nodes, is given in 250 rad increments 4 times weekly to a total dose of 3000 rads. After an interval of 3 weeks following the radiation therapy, we are performing the radical nephrectomy. 100 patients were treated by this method in the years 1967-1975: 32 patients were in stage I, 7 in stage II, 50 in stage III and 11 patients in stage IV. In 26 patients more than 5 years have passed since the beginning of the treatment: 46% od these survived. The survival for 3 years is 63, for 1 year after the surgery 80%. The surgical mortality rate is 2%. The object od the preoperative treatment is: 1. Devitalization of growing cells in the periphery of the tumor, thus preventing metastases and local recurrence. 2. Decreasing the size of the tumor and thereby facilitating surgery. In one-third of the cases there is radiologically demonstrable decrease in the size of the tumor mass, probably secondary to obliteration of the dilated veins in the capsule. The delay of six weeks has had no adverse effect on the outcome of the disease.

A partial urinary bladder resection was performed in 25 female cats. The defect was replaced with an X-ray treated autologous full thickness skin graft. This graft was shown to be an appropriate foundation for the rapid regeneration of transitional cell epithelium which was proven to be present after 4 weeks. Smooth muscle fibres were present in all cases after 12 weeks. Postoperatively, bladder capacity was adequate without evidence of incrustation or ossification. No changes were seen in the upper urinary tract. Compared to other types of autologous bladder wall replacement this method is simple and warrants clinical investigation.

51 patients with proven prostatic carcinoma stage B-D underwent control aspiration biopsy after hormonal therapy using the Franzén needle. The malignancies were classified into three grades and the cytological findings were compared with the malignancy grade of the tumor and the clinical course. At the same time the caryopycnotic index, as determined by swabs from the urethral fossa navicularis was measured in order to determine the serum estrogen level. As the result of the anti-androgenic therapy one finds degenerative changes in the tumor cells. The results showed that the poorer the histological differentiation of the tumor, the lesser was the cytological change. Only 6 of 34 patients with a good hormonal effect showed a progression of the tumor, whereas 16 of 17 patients with a poor hormonal effect showed clinical progression. 70% of the cases demonstrated agreement between the clinical course and the caryopycnotic index. No correlation was found between the caryopycnotic index and the histological grade of the tumor.

In this study the pathophysiological connections in varicocele genesis are discussed, as well as its effect upon spermiogenesis and the causal factors. In 73 patients the high retroperitoneal ligature of the testicular vein was performed. Indication was given either on the basis of subjective complaints or on the proof of subfertility. In 79% of the cases a good anatomic result was obtained after operation. Of the spermiograms established prior to operation 56% showed postoperatively a distinct improvement in quality. In 52% of the sterile marriages conception occurred following operative intervention. In our clinic the operative method according to Bernardi is preferred, since it meets best the criteria of an ideal varicocelectomy. Operative intervention should take place early for possible preventing of spermiogenetic lesion.
