Comparative analysis of the results of suspension-urethroplasty according to Marshall-Marchetti-Krantz and of urethrovesicopexy with adhesive.

A comparative analysis is presented of the results of adhesive urethrovesicopexy and the conventional suspension method according to Marshall, Marchetti and Krantz. It appears that the results are less favorable when compared to those mentioned in the literature, and no doubt attributable to the difficult differential diagnosis of stress and urge incontinence. The adhesive method supplies significantly less favorable results than the conventional method according to Marshall, Marchetti and Krantz.

[Determination of auto-antibodies against spermatozoa after vasectomy and epididymectomy by indirect immunofluorescence (author's transl)].

As a result of successful recanalization of the vas deferens, a low pregnancy rate has been observed. The formation of auto-antibodies is considered as cause. In vasectomized and epididymectomized rats, the presence of auto-antibodies against spermatozoa was proven by indirect immunofluorescence. After uni- and/or bilateral obstruction of the vasa, the formation of antibodies against spermatozoa takes place which - after 2 months - proves to be permanent for 9 months, then showing a decreased titre. An animal model is presented suitable for the study of immunological processes in unilateral gonadal diseases.

[Evaluation of hydroxyproline excretion in the urine (UHP) in prostate carcinoma].

82% of the male patients with healthy urogenital tract showed an isoelectric line in the stimulataneous EMG-derivation of the sphincter of the bladder under conditions of rest; in 18% of the males a moderate activity was present. In voluntary innervation at once an intensive electric activity of a spinle-shaped discontinuous pattern of the action potential developed in 76% of the patients. In the pressing trial in 83% of the cases a slight to moderately strong activity appeared. In contrast to the examined patients with healthy outlet of the vesicle the patients in whom an adenoma of the prostate gland or a carcinoma of the prostate gland, respectively, was present exhibited deviations from the normal pattern of the action potential, in which cases carriers of adenoma and carcinoma differed. Whether and to what extent the electromyography on the m. sphincter vesicae allows the possibility of the differentiation between adenoma of the prostate gland and carcinomaof the prostate gland in the early clinical stage shall be reserved for further examination.

[Comparative statistic of complications in openprostatectomy and transurethral resection (author's transl)].

On the basis of the case material of 1049 patients, the advantages and disadvantages of enucleation and the transurethral resection of the prostatic adenoma are compared. There are no significant differences in the lethality of the operation. Although at present preference is generally given to the transurethral resection, this procedure appears less favorable to us with regard to the intraoperative complications as well as late complications of urethral strictures, during long-term follow-up.

[Autologous human skin flaps in partial replacement of bladder wall (author's transl)].

Untreated autologous compound skin (Rehn's skin flap) was used in three patients for partial replacement of the urinary bladder wall. The early postoperative results regarding the general tendency of the healing process, primary density, capacity, and functional voiding of the bladder correspond with an ideal replacement of the bladder wall. Observations of long-term controls will be reported later.

[The effect of tramadol and other analgesics on the pain threshold in human dental pulp (author's transl)].

In 60 young, healthy volunteers the analgesic efficacy of 1-(m-methoxyphenyl)-2-(dimethylaminomethyl)-cyclohexan-1-ol (tramadol; Tramal), dextropropoxyphene and a commercial drug mixture (acetylsalicylic acid 200 mg; phenacetin 200 mg; codeine phosphor. 10 mg; caffeine anhydr. 50 mg; phenobarbital 25 mg) was investigated by determining the pain threshold in dental pulp. All three drugs increased the pain threshold considerably but there was no difference in the analgesic effect of the three drugs.

Preoperative irradiation of renal cell carcinoma.

Since 1967 we have been using preoperative radiation therapy for renal cell carcinoma as proposed by Riches. Radiation therapy, to include the para-aortic lymph nodes, is given in 250-rad increments four times weekly to a total dose of 3,000 rads. After an interval of three weeks following radiation therapy, we performed radical nephrectomy. The object of this preoperative treatment is devitalization of growing cells in the periphery of the tumor, thus preventing metastases and local recurrence, and and decreasing the size of the tumor and thereby facilitating surgery. In one third of the cases there is radiologically demonstrable decrease in the size of the tumor, probably secondary to obliteration of the dilated veins in the capsule. The delay of six weeks has had no adverse effect on the outcome of metastases.

[Irradiation for prevention of gynecomastia prior to estrogen therapy in cases of carcinoma of the prostate (author's transl)].

The most frequent and most unpleasant side-effect of therapy with estrogen hormones in patients with carcinoma of the prostate is the painful gynecomastia. Since 1969, we have been performing the prophylactic irradiation of the mammary glands on 284 patients in order to prevent a hormone-induced gynecomastia. The majority (262 patients) was irradiated with 600 rad surface dose in fractions of 150 rad prior to the hormone therapy. One hundred and two patients, having been treated endocrinologically for 2-75 months, had a follow-up examination. After irradiation, only 19% of the patients did not develop a gynecomastia, and 60% had no mamillary hyperesthesias. Compared to other authors, the therapy was less efficient, the symptoms, however, were discrete. As a consequence, an increase of the radiation dose as well as an alteration of the fractionation must be used.

[Epididymal tumors (author's transl)].

Six cases of benign primary epididymal tumor have been investigated in our patients. The tumor size fluctuated between 3 and 35 mm in diameter. Frequency, histology and medical development are described. The majority of epididymal tumors (53%) is comprised of so-called adenomatoid tumors of benign nature and their histogenesis is being discussed. The difficulties of differential diagnosis are pointed out. Since malignity exists in 26% of the cases, the surgical exposure and diagnosis by frozen section is being postulated, when suspicion of epididymal tumor becomes evident.

[Results of radical perineal prostatectomy: 14 years' experience (author's transl)].

One hundred radical perineal prostatectomies are reported, which were performed during the past 14 years. In this series were 36 patients with stage C carcinomas, which could not be operated radically by total prostatectomy. This shows the difficulties in preoperative staging of these tumors. In spite of doing routinely a vesiculography and a lymphography these tumors were frequently undervalued. The 5-year survival rates, being 66.6% in stage A/B and C carcinomas correspond nearly to the results of Belt. The postoperative incontinence rate was 10%. Only two patients retained their potency. There were no postoperative mortalities.

[The diagnostic value of radiological, nuclear medicine and biochemical methods for detection of bone metastases in carcinoma of the prostate (author's transl)].

785 combined diagnostic procedures are reported which were carried out on 353 patients with microscopically proven carcinoma of the prostate in order to detect metastases. X-ray films of the thorax, spine and pelvis were taken. Also bone-scintigraphy was done with 87MSr or 99MTc-polyphosphate. Additionally the alcaline, acid and prostate phosphatases were determined. A diagnostic coincidence between radiological results and bone-scan was found in 95.1% of cases. The bone-scan was false negative in 3.7%. Only in 1.2% metastases were detected earlier by the bone-scan than by x-ray examinations. Referring to the number of studies identical results were found. In roentgenologically detected metastases an increased serum level of the alcaline phosphatase was found in 61%, of the acid phosphatase in 31% and of the prostate-phosphatase in 25%.

[Echinococcal infection of the kidney (author's transl)].

800 echinococcal infections of the kidney have been described in the world literature, nevertheless, here this disease is very rare. As a result of increasing tourism and employment of workers coming from endemic areas, the morbidity rises and the clinical picture gains in importance. A review has been made of epidemiology, life cycle and symptomatology of the cystic echinococcus and the diagnosis, differential diagnosis and therapy of this parasitosis has been discussed with reference to two misdiagnosed cases from our own patients.

Denudation of the urinary bladder mucosa in the cat by formaldehyde.

Chemical Stripping of the urinary bladder mucosa was studied in 38 cats using 5 to 25% formaldehyde solutions. The contact time varied from 1 to 20 min. With a 20% solution and contact time of 1 min, total denudation was possible without necrosis of subepithelial layers. In such cases, complete reepithelialisation and normal bladder dynamics were seen within 3-4 weeks after formaldehyde instillation. Signs of formaldehyde intoxication due to vesical resorption were not observed.
