Lysozyme tear level in patients with herpes simplex virus eye infection.

The lysozyme level in tears of patients with HSV eye infection was examined and correlated with the clinical findings and presence of virus. The concentration of the enzyme in tears of patients during acute attack was 2.83 mg./ml. This value was significantly lower than that in tears from healthy controls (6.1 mg./ml.) and tears from the patient's healthy eye (4.46 mg./ml.). After termination of treatment with either IUDR or poly I:C, the lysozyme level rose to an average of 4.34 mg./ml. During the latent period of the disease the level increased (5.34 mg./ml.), but it remained lower than in healthy subjects who had never suffered from HSV eye infection. This may be an indicator of possible future recurrences.

A case of late pseudomonas ocular infection following scleral buckling.

Two and half years after a circling buckle operation in which two Supra-mid sutures and a solid silicone explant were employed, a Pseudomonas infection developed around the explant and progressed to the inner eye. Removal of all implanted foreign material and treatment with Rifamicin saved the eye. The relevant literature on infection following sclero-plastic procedures is reviewed, and the pathogenesis of the late infections is discussed.

Histoacryl tissue adhesive in some types of retinal detachment surgery.

Preserved human scleral graft and histoacryl-blue tissue adhesive were used in four cases of retinal detachment surgery to obtain scleral buckling effect and to protect staphylomatous or necrotic scleral areas. The use of histoacryl produced a strong and resistant adhesion between the host and the preserved scleral patch. The postoperative inflammatory reaction was mild and disappeared within one week.

Family study of inherited syndrome with multiple congenital deformities: symphalangism, carpal and tarsal fusion, brachydactyly, craniosynostosis, strabismus, hip osteochondritis.

A syndrome of brachydactyly (absence of some middle or distal phalanges), aplastic or hypoplastic nails, symphalangism (ankylois of proximal interphalangeal joints), synostosis of some carpal and tarsal bones, craniosynostosis, and dysplastic hip joints is reported in five members of an Italian family. It may represent a previously undescribed autosomal dominant trait.

Treatment of external ocular Candida infections with 5-fluorocytosine.

15 patients who suffered from severe and prolonged external Candida ocular infections and who had not responded to Mycostatin and/or Amphotericin B treatment, were successfully treated with 5-Fluorocytosine administered topically or combined topical-orally. In all patients, a cure was achieved after a treatment period of approximately 6 months. No side effects were observed and no resistance to the drug developed.

[Revascularization of the aorto-femoral axis by means of a by-pass. Remote results].

The Authors report their experience in 73 patients wit aorto-iliaco-femoral chronic obliteration, subjected to revascularisation. In 49 cases a bilateral by-pass was performed, in 22 a monolateral by-pass, and in two a thromboendarterioectomy. The results obtained were classified as successes or failures according to the perviousness or otherwise of the substituted or disobliterated arterial segments, and as immediate or longterm according to whether they were evaluated at the time of the patient's discharge or during a period lying betwee six months and seven years from the operation. Immediate successes were obtained in 95.9% of cases with bilateral by-pass, and in 90.9% of the monolateral by-passes. Aggregate mortality was 2.7%. In the long-term chick-ups, the previousness of the bilateral by-passes was 82.9%, that of the monolateral by-passes 72.2%. On the basis of the results obtained the Authors therfore reiterate their preference for by-passes, and for bilateral by-passes in particular (70% of cases). Monolateral bridging were in fact performed only in those cases where the contralateral vascular district was unharmed, in patients with serious ganrenous lesions and those with a high operative risk.

[Long-term results of prolonged dicoumarol anticoagulant therapy in 327 peipheral arteriopathic subjects].

327 patients with peripheral arteriopathy were treated solely with protracted dicoumarol anticoagulant therapy. The indication for such therapy was applied not only in patients in whom the high risk and poor run-off contraindicated revascularisation operations, but also in those in good general conditions at the 2nd or 3rd stage with iliac and femoral thromboses. Two-thirds of the arteriopathics observed were given the anticoagulant treatment for more than two years, with a maximum of eight years. Clinical evolution of the disease is discussed in relation to the treated patients' age and stage, while emphasis is given to the very low incidence of complications and contraindications, despite the long period of treatment.

Human ocular dirofilariasis in Israel.

This is the first report of human ocular dirofilariasis in Israel. A single female worm, Dirofilaria (Nochtiella) conjunctivae, was removed from an eye of each of two male patients. In the first case the worm was dead upon removal. It had penetrated into the sclera and was lodged in a subconjunctival abscess. In the second case, the worm was alive and located superficially in a slight swelling near the inner canthus. Surgical removal of the worms under local anesthesia was followed by complete recovery in both patients, with no impairment of vision.

Experimental and clinical studies of 5-fluorocytosine activity in candida ocular infections, II. The effect of 5-fluorocytosine in treating experimental candida infection in rabbits' eyes.

Mild and severe experimental ocular infections in rabbits' eyes were induced by inoculation of Candida albicans with or without addition of dexamycin drops. These rabbits were treated with 5-Fluorocytosine, topically, orally and in a combined topical and oral manner. The topical treatment was effective only in mild infections. Severe infections responded to oral and especially to combined oral-tropical treatment.

Experimental and clinical studies of 5-fluorocytosine activity in Candida ocular infections. I. In vitro activity of 5-fluorocytosine on Candida species isolated from ocular infections.

In vitro sensitivity to 5-fluorocytosine (5-FC) of 35 Candida strains (14 C. albicans, 10 C. tropicalis, 5 C. parapsilosis, 2 C. guilliermondii, 2 C. krusei, 2 C. pseudotropicalis), isolated from the eyes of patients suffering from various external ocular inflammations who had not been treated with 5-FC, was tested and compared to that of standard Candida species. The minimal inhibitory concentration (MIC) and minimal fungicidal concentration (MFC) of 5-FC for the strains isolated from ocular infections ranged from 0.06 to 1.95 mug/ml and from 0.24 to 7.81 mug/ml, respectively, indicating that no primary resistant strains to 5-FC were found in this series.

Yeasts in banal external ocular inflammations.

In 53 out of 313 patients suffering from longstanding inflammations of the outer eye or dacryocanaliculitis, not yielding to the obligate treatment with antibiotics and steroids, yeasts were isolated and regarded as the causative agents. Among the yeasts, different Candida species were prevalent, with C. albicans and C. guillermondii at the top. An obstinate, superificial epithelial and subepithelial punctate keratitis appeared to represent an entity pathognomonic of a yeast infection. Eradication of the yeasts and cure of the inflammation was achieved only by intensive prolonged treatment with Mycostatin or amphotericin B.

A spectrophotometric method for quantitative determination of lysozyme in human tears: description and evaluation of the method and screening of 60 healthy subjects.

A spectrophotometric micromethod for lysozyme determination in tears is described, which enables the estimation of very low concentrations of lysozyme in individual tear samples, and which can be performed rapidly. The method is based on collecting the tears by a microcapillary tube and diluting them in a special manner which increases the volume of the tear sample, thus making possible analysis of other tear constituents in addition to lysozyme. Lysozyme activity in tears is determined by reduction of optical density (OD) of Micrococcus lysodeikticus suspension. The sensitivity of the method was determined on the basis of repetitive readings. The level of lysozyme in tears of 60 healthy people was determined by this method and found to be 6.1 mg. per milliliter hen egg lysozyme (HEL) with standard deviation of 1.57 mg. per milliliter HEL or 1.5 mg. per milliliter human tear lysozyme (HTL) with standard deviation of 0.39 HTL. Lysozyme level in tears of both eyes of each individual is equal, and any difference observed between the two eyes is due to the variability of the method.
