Ischaemia related ventricular arrhythmias in patients with variant angina pectoris.

UNLABELLED: Twenty-three patients with variant angina were studied by Holter monitoring both to assess the incidence of serious ventricular arrhythmias (a risk factor of sudden death in variant angina), during ischaemic episodes and to examine the time-relation of the arrhythmias to ST changes. Serious ventricular arrhythmias were observed in 12/23 patients (52%). In the 23 patients, a total of 491 episodes of ST segment elevation were recorded during 954 h of Holter monitoring; serious ventricular arrhythmias were found in only 46 ischaemic episodes (9.4%). Six out of 12 patients showed serious ventricular arrhythmias at the onset of ischaemic episodes or during maximal ST elevation (phase 1), one patient during return or immediately after return of ST to baseline (phase 2) and five patients during both phases. Thirty-three out of 46 ischaemic episodes (76%) showed serious ventricular arrhythmias during phase 1, eight (17%) during phase 2, and five (11%) during both phases. Serious ventricular arrhythmias were neither related to previous myocardial infarction nor to the presence of serious ventricular arrhythmias during inter-crisis periods, whereas a good relationship with severity of ischaemic episodes, as assessed by the magnitude and duration of ST elevation, was found. A modest relationship with anterior ST elevation was also found. IN CONCLUSION: (1) serious ventricular arrhythmias occur in a high percentage of variant angina patients, but in only a small proportion of ischaemic episodes; (2) serious ventricular arrhythmias are related to the severity of ischaemia and occur predominantly at the onset of ischaemic episodes and/or during maximal ST elevation; in only a few cases do they occur during resolution of ischaemic episodes.

Malignant peripheral nerve sheath tumors arising from ganglioneuromas.

Two typical malignant peripheral nerve sheath tumors (PNST) arising in preexisting ganglioneuromas are described. To the best of our knowledge, this association of tumors has not been reported in detail previously. Neither patient had the stigmata nor family history of Von Recklinghausen's neurofibromatosis. In both cases, the ganglioneuromas evolved from more primitive neuroectodermal tumors (one neuroblastoma, one ganglioneuroblastoma) and both patients developed their malignant PNST at previously irradiated sites. Both patients died within 2 years of the diagnosis of their malignant PNST. The origin of these malignant PNSTs from Schwann cells is supported by an ultrastructural analysis of 27 neuroblastomas, ganglioneuroblastomas, and ganglioneuromas.

Automated determination of anti-streptolysin O antibodies by a kinetic hemolytic method.

An automated method which uses oxidized Streptolysin O and determines the anti-Streptolysin O titer in whole blood without dilution of the sample is described. The addition of a reducing agent starts the hemolysis at an initial rate which is inversely proportional to the anti-Streptolysin O titer of the sample. An instrument (Taso-matic), designed to automatically execute all of the procedures, was used. Comparison between the described method and the classical hemolytic method gave the following linear regression slope y = 1.06x-13 (r = 0.974). Precision of the method at two different anti-Streptolysin O titers (200 and 500 IU), expressed as relative standard deviation, was 12.1 and 4.7% in the between-run procedure and 10.1 and 4.9% in the within-run procedure, respectively.

[New heterocyclic derivatives of benzimidazole with germicidal activity. II. Mechanism of action of fluoro-5-(nitro-2'-furyl)-2-benzimidazole in B. subtilis and C. albicans].

Study of the microbiological activity of 5-fluoro-(2'-nitrofuryl)-2-benzimidazole F-O-NO2 (I) was continued. The substance was tested on cultures of B. subtilis and C. albicans. From the microbial strains studied, the mortality rate, the minimum inhibitory and bacterial concentrations and the cytotoxicity were determined. The action mechanism was studied using tritiated leucin, uracil and thymidine. Acute toxicity in the mouse (DL50 = 275 mg/kg) was also determined. The germicidal activity of (II), homologous N-ethyl derivative of (I), was also studied. On the basis of the results obtained, a possible mechanism of action of compound (I) is discussed.

Anatomical localization schemes for use in positron computed tomography using a specially designed headholder.

A headholder system is described for use in the correlation of images obtained with positron computed tomography (PCT) and other neuroradiological imaging modalities. Methods are described for defining brain anatomy from PCT images by the use of projection schemes from tomographic data. Cross-correlation of images between positron CT and standard lateral skull films, X-ray computed tomography (XCT), and two-dimensional rectilinear positron images are discussed. Such methods allow for improved reproducibility of head positioning and more precise cortical localization necessary in studies of normal brain function as well as in neuropathological conditions.

Unusual inclusions in plasmacytoid cells. Their occurrence in a patient with Waldenström's macroglobulinemia.

The bone marrow from a patient with Waldenström's macroglobulinemia was studied, and histologic, immunopathologic, and ultrastructural features correlated. The infiltrating plasmacytoid lymphocytes contained PAS-negative cytoplasmic inclusions, which were shown to contain a specific immunoglobulin that matched the patients's circulating paraprotein. Electron microscopic studies revealed abundant dense fibrillar and globular material within dilated cisternae of rough endoplasmic reticulum. This material was morphologically similar to, but quantitatively different from, an endoplasmic reticulum-associated structure previously described in several patients with chronic lymphocytic leukemia. The differences between these fibrillar inclusions and Russell bodies are stressed.

[N1-substituted 1H-indazole-3-ethyl carboxylates and 1H-indazole-3-hydroxamic acids].

Sixty five new derivatives of ethyl-1H-indazole-3-carboxylate are described; they contain in N1 various aliphatic or aromatic acyl radicals. Moreover halogens or methyl groups are present as substituents at the 5 position or methyl groups at 5,6. The synthesis of seven 1H-indazole-3-hydroxamic acids, substituted at 6 and/or 5 as above, is also described. Some of the synthesized derivatives have preliminarily been tested on rats to investigate acute toxicity, possible antiarthritic effects on primary or secondary arthritis, and their action on weight gain. Some of these indazole derivatives had an antiarthritic effect at doses much lower than the toxic ones; among the compounds tested up to now, the ethyl-5-methyl-N1-p-chlorobenzoyl-1H-indazole-3-carboxylate gave the best results. Weight gain as not affected by any of the examined compounds.

[Diabetic cardiomyopathy in preclinical phase. Polycardiographic and echocardiographic study (author's transl)].

Eighteen selected diabetic patients, without symptoms or signs of cardiac diseases and a control group of 15 normal subjects, age and sex matched, underwent polycardiographic and echocardiographic study, with the aim to evaluate their systolic and diastolic function of the left ventricle. PEP, LVET and PEP/LVET ratio were determined on the polycardiographic tracings. In eleven diabetic patients these data were also determined after amyl nitrite inhalation. Left ventricular diastolic dimension (DD), LV systolic dimension (DS), ejection fraction (EF), VCF, fractional shortening (FS), mitral opening delay (MOD) and the relaxation speed of the left ventricular posterior wall were determined on the echocardiographic tracings. All these data underwent statistical analysis by Student test, and some significant differences were found between diabetic patients and normal subjects. The mean value of PEP was augmented (diabetic patients: 100 +/- 18 msec; normal subjects: 88 +/- 9 msec; p 0,05), while the mean value of LVET was diminished (diabetic patients: 275 +/- 22 msec; normal subjects: 300 +/- 23 msec; p less than 0,005); the PEP/LVET ratio was, therefore, increased in diabetic patients (diabetic patients: 0,37 +/- 0,08; normal subjects: 0,29 +/- 0,04; p less than 0,005). Of all echocardiographic data only MOD (diabetic patients: 54 +/- 31 msec; normal subjects: 14 +/- 21 msec; p less than 0,001) was significantly increased in diabetic patients. The feasibility and reliability of polycardiographic and echocardiographic techniques in detecting early myocardial disfunction is discussed and the accurate selection of patients in order to exclude other myocardiopathies in pointed out. It is then concluded that diabetic patients, in comparison with normal subjects, show an impairment of both systolic and diastolic function of the left ventricle, exclusively depending on diabetes.

New hemolytic method for determination of antistreptolysin O in whole blood.

A new method is proposed for the determination of antistreptolysin O, based on the properties of streptolysin O. The reduced form of the toxin is hemolytic, whereas the oxidized form is not; this activity can be restored, however, by a reducing agent. Both forms maintain the capacity to bind specific antibodies. Unlike the classical method, the assay is performed on whole blood, using the patient's own erythrocytes as revealing agents. A procedure is proposed which limits the operation to a single dilution of a microvolume of blood (0.05 ml brought to 2 ml) and to the distribution of constant quantities in test tubes prefilled with different amounts of oxidized streptolysin O. After a few minutes, a reducing agent is added, and the reading is performed as in the classical technique after sedimentation of the erythrocytes. Correlation with the traditional method (r = 0.987) is evaluated on the basis of the results of samples from hospitalized patients.

[Primary pericardial mesothelioma: report of a case (author's transl)].

A case of primary pericardial mesothelioma in a 36-year-old man is reported. The disease, with an atypical clinical onset and rapidly fatal course, was diagnosed before death, by means of clinical examination, chest X-rays, cineangiography and microscope examination of pericardial and pleural exudate, revealing the presence of malignant mesothelial cells. Postmortem examination confirmed the diagnosis and revealed thoracic and extrathoracic metastasis. The clinical, anatomic, pathologic aspects, as well as treatment of primary pericardial mesothelioma are discussed.

[Goldmann-Favre hyaloido-tapetoretinal degeneration].

In a sister and brother we found a case of vitreo-tapeto-retinal degeneration (macular retinoschisis, night-blindness, vitreous micro-fibrillose degeneration, abolished E.R.G., very pathological E.O.G.) and a "fruste" form of tapeto-retinal degeneration (De Lange's curve and flicker-E.R.G. both modified, E.R.G. subnormal and slight pathological changes in adaptometry). The father of these two patients suffered from tapeto-retinal degeneration which had caused complete blindness. Basing their suppositions on their personal experience and medical literature, the authors believe that the type of vitreo-retinal degeneration is best determined after examination of the mode of hereditary transmission. In the family which is presented, they believe that it is a form of Goldmann-Favre's disease with pseudo-dominance.
