Induction of human T colony formation by phorbol myristate acetate.

Phorbol myristate acetate (PMA) is a potent inducer of T colony formation by peripheral blood lymphocytes. A mean cloning efficiency of 0.3% (0.05-0.5%) is obtained with PMA concentrations of 100-1000 ng/ml. PMA-induced T colony formation does not require the presence of monocytes and therefore differs from other mitogens in this respect. Purified T-colony-promoting activity (TCPA) (devoid of phytohaemagglutinin (PHA)) increases PMA-induced T colony numbers and induces T colony formation at low PMA doses (0.01 to 1 ng), concentrations at which no T colonies are detected in the absence of added TCPA. PMA-induced colonies are mainly composed of cells bearing Fc receptors for IgM (54%), which is not the case for colonies obtained with PHA (11%). PMA-induced colony cells do not bind OKT3 and OKT4 monoclonal antibodies, whereas 23% are able to bind OKT8 antibody. These results demonstrate that PMA is a potent inducer of T colony formation and may therefore serve as a useful tool for the study of T-cell differentiation.

Modulation of PHA-induced T colony formation by phorbol myristic acetate.

PMA (10 ng/ml), induced a four-fold increase in PHA-induced T colony formation by peripheral blood lymphocytes (PBL). At lower (0.1-1 ng/ml) and higher concentrations (100-1,000 ng/ml), PMA had an inhibitory effect. The potent co-mitogen effect observed at 10 ng/ml PMA was associated with a strong increase in T colony promoting activity (TCPA) released by PHA stimulated PBL cultured in the presence of PMA (10 ng/ml). PMA at all concentrations exerted an inhibitory effect on T colony formation when PBL were cultured in the presence of PHA and an optimal concentration of exogenous TCPA. PMA was also capable of assuming the essential role played by monocytes in T colony formation. Thus, PMA can fulfil both co-mitogen and monocyte like roles in T colony formation. These activities closely resemble those previously described for interleukin 2 (IL2) production.

[Analysis of infectious sequelae of 1000 neurosurgical operations. Effects of prophylactic antibiotherapy].

Postoperative infections: cellulitis at the site of skin incision and/or meningitis, were reported in 5.1 p. cent of 1 000 cases treated by neurosurgery in Pr R. Houdart's department between december 1980 and march 1982. Statistically significant factors predisposing to infection were: emergency surgery, opening of the sinus, presence of a foreign body, and operation lasting more than 5 hours. The age of the patient, diabetes, or previous corticoid therapy did not significantly alter the risk of infection. Prophylactic antibiotic therapy had been administered to 37 p. cent of patients, but this had not affected the incidence of general infection, a statistically significant effect being observed only after operations lasting for more than 5 hours. The risk of infection was high after craniotomies and major after external ventricular shunts (valves). For the latter type of operation it was not possible to determine factors favorable for infection: neither duration of surgery, nor age of patient, nor absence of antibiotic therapy. The risk of postoperative infection was low (less than 1 p. cent) in the absence of factors favorable for its development, but its frequency increased considerably in patients presenting one or more other intercurrent infections. It is therefore possible to recognize surgical and general factors influencing infection, but prophylactic antibiotic therapy has only a weak effect on morbidity modification.

[Use of real time ultrasonography in neurosurgery. Peroperative value in adults].

The authors present their beginning experience in the use of a new technique applied to neurosurgery: that is real-time echography. A preliminary non operative experience has first been carried on. It concerned five adults who have been studied through surgical defects. Then, eight patients, were explored intra-operatively: concerning three cerebral abscesses, one intra-cerebral haematoma, two subcortical tumors and two biopsies. In these cases, the ultrasonographic study was performed through a 3 centimeters diameter craniotomy--in case of abscess or haematoma aspiration or of biopsy--or through a classic bone flap--in case of tumor extirpation. Echography allowed an easy study of intracerebral structures and lesions. Its permits to visualize subcortical neoplasms and thus to aim them with accuracy. In the treatment of brain abscess, real time echography gives the possibility of guiding and following the progression of the probe. It, also, allows to appreciate the reduction of the suppurated cavity and thus assures a more reliable efficacity. Such a procedure indeed facilitates brain tumor biopsy; but we think that such a technique should be used only if a classic stereotactic procedure is not considered as necessary and if the mass lesion is large enough (over 1.5 cm) and not too profondly seated.

[Segmental dilatation of the ileum in a neonate. Study of the myenteric plexus with a silver staining preparation (author's transl)].

A case of segmental dilatation of the ileum in a neonate is reported. Conventional staining methods failed to show any abnormality, but a study of the myenteric plexus with a silver preparation demonstrated reduction in the number of agyrophil neurons and irregular and altered neuron cell bodies. Occurring in a neonate, therefore without previous clinical history, this case, together with various published data suggests that in some cases segmental dilatation of the ileum is due to congenital damage to the myenteric plexus.

Folate and iron deficiencies in mothers and their newborn children.

Iron and folate status was assessed in 79 multiparous French women and 21 immigrant parturient women, previously defined as belonging to a high risk population for nutritional deficiencies, and their newborn infants. Maternal hemoglobin was correlated with ferritinemia and with serum folate values; weight gain during pregnancy appeared to be correlated with mother's iron status at start of labour and gestation duration with folate status. The need of iron and folic supplementation is emphasized for such groups. While a correlation appeared between maternal and newborn values for serum and red blood cell folates and for the RBC indices, no statistical difference was found between the infants born to iron or folate deficient mothers when compared with others.

A new radionuclide method for the diagnosis of posttraumatic cerebrospinal fistulas. A study of 308 cases.

The authors describe their technique for identifying and localizing posttraumatic cerebrospinal fluid fistulas. The method entails injection of radionuclide tracer into the subarachnoid frontal space with the patient in a sitting position followed by gamma camera scintigraphic recordings. The results in 308 patients are presented; these are compared with those of suboccipital radionuclide cisternography in 40 cases and metrizamide computerized tomography cisternography in nine cases. Transient complications occurred in only three patients.

[Intraspinal dracunculosis. Apropos of a case of epidural medullary compression].

The authors report on a case of spinal cord compression due to Dracunculiasis. They study the nine others cases of the literature. In all these cases, the worm was epidural. The extradural guinea worm granuloma was localised at the level of the cervical region or in the upper part of the thoracic region of the spinal canal. Before operation the diagnosis is almost always impossible. It could be evocated if the guinea worm is calcified and, then, visible on the spinal tomograms.

[Treatment of tumors in relation to the transverse portion of the vertebral artery].

Among 28 lesions, 22 tumors and 6 anteriovenous malformations, operated with control of the vertebral artery, 15 are located close to the transverse canal. Analysis of arteriographic datas from 20 cases of tumors related to the vertebral artery, points out the interest of the surgical control of this vessel, in the treatment of these tumors. This control is mainly indicated in case of hypervascularized tumors, or compression of the vertebral artery, especially if it is the dominant vessel or if it gives a radiculo-medullary branch. For this control, the lateral retro-jugular approach is used, and sometimes associated with other techniques: embolization, posterior approach, or anastomosis between carotid and vertebral artery about the lesion.

Decrease of tumor growth in mice after intravenous thymosin-treated bone marrow cell injection.

The effect of iv injection in C57BL/6 mice of 3X10(7) bone marrow cells preincubated in either thymosin fraction V or thymosin alpha-1 was evaluated on the growth of a 3-methylcholanthrene-induced transplantable tumor in a syngeneic system. The effect was then compared to that elicited by theophylline or levamisole, which both demonstrated thymosin-like action in vitro. The results showed significantly retarded tumor growth (P less than 0.001) and prolonged survival time (P less than 0.001) when thymosin fraction V was used. The same results were obtained with thymosin alpha-1 with use of the same protocol but only one-twentieth of the concentration of fraction V. Theophylline and levamisole demonstrated no antitumor effect.

Prognostic evaluation of human tumor cell in vitro cloning assay.

In vitro cloning of human tumor cells were carried out for 19 patients: 13 breast cancers (4 metastatic effusions and 9 primary tumors), 3 ovarian cancers, 2 glioblastomas and 1 meningioma. Cloning efficiency varied from 1 X 10(-5) to 2.8 X 10(-3). Tumor cells with the highest cloning rates were provided by patients with rapidly evolving tumors. 7 drug assays were performed: in vitro-in vivo correlation was observed in all cases (drug resistance in 5 cases: drug sensitivity in 2).

[Hypoglossofacial anastomois. Analysis and results on 12 cases and comparison with results of facio facial anastomosis].

Regular facial clinical testing (G. Freyss) and surface integrated electromyography was employed to quantitatively assess the course of 12 hypoglossofacial anastomoses. Recovery was complete after an average period of 4 months, the time before conducting anastomosis (2 to 14 months after onset of facial paralysis) not affecting the quality of results obtained. Age significantly influenced recuperation of remaining tone, but this was always superior to 70 p. cent in relation to the other half of the face. Motor recovery was not affected by age, and was a mean of 51 p. cent after one year (50 to 70 p. cent when compared with the normal half of the face). Lingual sequelae were minimal, early physical therapy compensating for the inevitable resulting synkinesis. Results after hypoglossofacial anastomosis are compared with those obtained by faciofacial anastomoses (published in 1980). The former is a simpler and more rapid technique which produces more constant results and leads to earlier recovery (4 instead of 9 months after faciofacial anastomosis. Tone is also improved and motility satisfactory.
