GTP depletion and other erythrocyte abnormalities in inherited PNP deficiency.

GTP levels were low and NAD+ levels high in purine nucleoside phosphorylase (PNP) deficient erythrocytes, in addition to the raised deoxy-GTP (dGTP) levels previously noted by others. dGTP was also identified in the PNP deficient child's lymphocytes. A further novel finding was the conversion of hypoxanthine to inosine by the PNP deficient red cells, as compared to inosine monophosphate (IMP) in controls. This has been attributed to IMP formation with subsequent breakdown, and raises interesting questions regarding the controls which normally maintain erythrocyte nucleotide pools. These findings may also explain the gross purine overproduction seen in this defect; they may likewise be related to the associated immunodeficiency, anaemia, and other clinical manifestations. The results may also have important implications for the development and clinical use of PNP inhibitors.

Evaluation of an objective structured clinical examination.

An objective structured clinical examination (OSCE) was introduced at the end of the fourth year undergraduate paediatric training. We have compared the results obtained with those of our traditional method of assessment. We feel that the OSCE is a valid and reliable assessment; it provided good feedback and was well received by the students.

Family studies on nucleoside phosphorylase and the short arm of chromosome 14.

A family with two nucleoside phosphorylase-deficient patients has been scored for the segregation of NP0 and the variable region 14p. The mose likely 14p:NP recombination fraction is 0.15 in males and 0.30 in females. There is no family data to assign the Pi:Gm linkage group to chromosome 14, but as immunoglobulin heavy chain has been assigned to this chromosome by somatic cell methods the most likely gene order is 14p:NP:Pi:Gm with Pi in 14q2 and Gm in 14(q23 leads to q32), but the order 14p:NP:Gm:Pi with Pi in 14(q24 leads to qter) and Gm in 14(q22 leads to q24) is not excluded. The available linkage data between biochemical markers on acrocentric chromosomes and their short arm markers suggest that there may be more recombination towards the ends of human chromosomes whether or not those ends carry centromeres.

The clinical syndrome of Impila (Callilepis laureola) poisoning in children.

The administration of herbal medicines made from Callilepis laureola is common among the Black population in Natal. This practice can and does cause poisoning, which has only been diagnosed with any confidence at postmortem examination, where the characteristic hepatic and renal tubular necrosis is obvious. Necropsy records of 50 children in whom these typical histological changes were noted were analysed retrospectively in order to determine the clinical picture. Results show that the disease can be recognized in young Black children when there is hypoglycaemia and an alteration in the level of consciousness, together with evidence of hepatic and renal dysfunction. This syndrome can easily be distinguished from viral hepatitis and fulminant hepatic failure, but less readily from Reye's syndrome.

Immunization against measles in children at risk for severe disease.

Live measles vaccine induced protective levels of antibody in 70% of children with protein-calorie-malnutrition (PCM) within 21 days and possibly in 90% by 42 days. The development of specific antibody was delayed and symptoms due to the vaccine more frequent (64%) in these children than in healthy children. Administration of measles vaccine may have predisposed to an associated fatal pneumonia in one malnourished child. Human measles hyperimmune globulin can maintain adequate antibody levels in most children with PCM (75%) for at least three to four weeks. Vaccination induced protective levels of measles antibody in 83% of healthy children under 10 months of age, which compared well with 86% of successful immunizations done at 10 months or later. There was wastage of vaccine in 40 to 70% of children who, despite a negative history of measles, had protective levels of antibody when admitted to the study. It is suggested that malnourished children in the community or the very young can be safely and effectively vaccinated against measles. But passive immunization is preferred in children with PCM severe enough to be admitted to hospital and thereby at increased risk of exposure to measles and other infections.

The practical training of New Zealand veterinary undergraduates in meat hygiene.

Development of the veterinary undergraduate course in meat hygiene over a ten year period is discussed. Arrangements and supervision of students during a two week practical course at six specified centres of meat production, and the value of such training in an actual industrial environment are discussed. The timing of this course in relation to the veterinary curriculum and the seasonal nature of the meat industry in New Zealand creates some problems. Training centres must slaughter a variety of animals and have sufficient staff to effectively teach students. Adequate preparation of students and close co-operation with both the Ministry of Agriculture and the meat industry is essential for success.
