PGD in female carriers of balanced Robertsonian and reciprocal translocations by first polar body analysis.

Preimplantation genetic diagnosis (PGD) using the first polar body (1PB) is a modality of PGD that can be used when the woman is the carrier of a genetic disease or of a balanced chromosomal reorganization. PGD using 1PB biopsy in carriers of balanced chromosome reorganizations has not become generalized. Here, we describe our experience based on the analysis of unfertilized or fresh, non-inseminated control oocytes, by fixing separately the 1PB and the corresponding oocyte, and on the study of six clinical cases of PGD using 1PB biopsy (four Robertsonian translocations and two reciprocal translocations). In fresh oocytes, the chromosome morphology of the 1PB was well preserved, and the results were always concordant for each oocyte-1PB pair. This indicates that the 1PB can be reliably used for the diagnosis of chromosome reorganizations. In these studies the technical problems encountered when performing PGD using 1PB biopsies for chromosome studies are also addressed. Three different strategies of 1PB biopsy (laser beam, partial zona dissection and acid Tyrode's) and two different protocols (intracytoplasmic sperm injection before or after 1PB biopsy) and their effect on the percentage of oocytes diagnosed and the fertilization rate, are discussed. In reciprocal translocation cases, published in the literature or studied by us, in which at least nine oocytes had been diagnosed, a correlation has been found between the frequency of nondisjunction observed and the theoretical recombination rate. To date, PGD by 1PB analysis alone or combined with blastomere biopsies in female carriers of chromosomal rearrangements has been used in 18 cases, with a further six cases reported here. A total of 325 cumulus-oocyte complexes have been obtained, of which 294 were biopsied and 224 were diagnosed. A total of 52 embryos was transferred, 19 of which implanted and 17 produced full-term pregnancies.

[Clinical complications in a series of patients with anticardiolipin antibodies].

An analysis was made of clinical complications, determined with objective methods, in 68 patients at the University Clinic of Navarra with the clinical suspicion of antiphospholipid syndrome (APS) and presence of anticardiolipin antibodies (ACA). Patients with IgG higher than 23 GPL were considered for the study. The most prevalent pathology was thrombosis: venous thrombosis (42.6%) and arterial thrombosis (22%). Other complications were abortions (23.8% of the 42 women) and thrombocytopenia (12.1%). With respect to the localisation of venous thrombosis, predominance corresponded to lower extremities (51.7%), followed by superficial thrombophlebitis (27.5%) and pulmonary thromboembolism (20.6%). Among the arterial complications, the most frequent were cerebrovascular disease (86.6 %) and coronary disease (13.4%). There was no correlation between the presence of high values of ACA and the prevalence or severity of clinical manifestations. In the series of patients with ACA IgG>23 GPL, we appreciated a high percentage of venous and arterial thrombosis detected with objective methods. A study on the development of nociceptive responses in pre- and postweanling rats: the tail electric stimulation test as a suitable methodology.

The development of nociceptive responses to the tail electric stimulation test was investigated. Male Wistar albino rats were used for studies on adult and pre- and postweanling rats of 20 and 25 days of age, respectively. The thresholds for the motor response (tail withdrawal), vocalization during stimulus and vocalization afterdischarge were assessed. These responses are considered to be integrated at spinal, medulla oblongata and diencephalon-rhinencephalon levels, respectively. Adult animals showed significantly higher thresholds for the three responses when compared to the neonates, while 25-day old rats showed significantly higher thresholds for the vocalization afterdischarge than the 20-day old pups. Although the effect of age cannot be ruled out, these latter results might indicate that the weaning process is critical in the development of the mechanisms related to the emotional/affective component of pain. The results also indicate the suitability of the nociceptive test employed for these kinds of studies.
