Tuberous sclerosis and associated pleuropulmonary lesions.

Tuberous sclerosis (TS) is a rare disease of the nervous system, being characterized by seizures, mental retardation and adenoma sebaceum. Concomitant pleuropulmonary lesions and spontaneous pneumothorax are extremely rare during the evolution of this disease. To date, only 19 cases of TS and spontaneous pneumothorax have been described in the literature. Here we present a case of TS and associated pleuropulmonary lesions with spontaneous pneumothorax in a 29-year-old female patient. Clinical, roentgenographic and histological aspects of this disease are commented upon.

The hormonal pattern in varicocele and its relationship with the findings of testicular biopsy: preliminary results.

In 26 patients with varicocele, measurements of plasma follicle stimulating hormone (FSH), luteinising hormone (LH), testosterone (T) and oestradiol2 (E2) were made; pre-operative and post-operative sperm counts, dynamic stimulation tests with HCG and clomiphene, and testicular biopsies including light microscopy, morphometry, and ultrastructural study in 4 cases, were also performed. A correlation was found between the hormonal pattern and testicular morphology.

[Radioimmunoassay for prostatic acid phosphatase in human serum. Methodologic aspects (author's transl)].

We propose a double antibody radioimmunoassay for human prostatic acid phosphatase (PAP) in serum for diagnosis and management of prostatic adenocarcinoma under treatment. The antigen is purified from human prostatic fluid by a gel-filtration on Sephadex G 100 followed by affinity chromatography on Con A Sepharose. A specific antibody is raised in rabbits and purified by immunoadsorption with a female serum. The described technique offers both radioisotopic sensibility and immunologic specificity. Physiological values determined in the serum of 125 healthy males are below 2 ng/ml. No significative differences are observed with age. The proposed technique also shows significant differences between values evaluated for benign prostatic hyperplasia and prostatic adenocarcinoma.

Relation between the test for gonadal stimulation with HCG and testicular histology in cryptorchism.

A histological study, by means of bilateral biopsy, on 44 cryptorchid patients (26 of whom are monolateral and 18 bilateral), comparing them with the response obtained for the HCG gonadal stimulation test, is presented. A direct, although not always exact, relation is established between the testicular histology and the testosterone levels after the administration of 4,500 U of HCG.

Androgen receptors in the rat epididymis and their hormonal control.

The concentrations of cytoplasmic receptor sites for androgens in the caput, corpus and cauda epididymidis, and the effect of ligation of the efferent ducts and testosterone treatment after bilateral castration on the concentration of receptors in the caput have been measured. Androgen receptors in the ventral prostate have been measured in the same animals for comparison. The caput has the highest concentration of receptor sites, the corpus the lowest. The ligation of the efferent ducts has no effect on this concentration which is dependent on testicular androgens. The present data do not yet allow explanation of the differential response of the different regions of the epididymis and of the other accessory glands to the administration of androgens.

5alpha-Reductase and 3alpha-hydroxysteroid oxidoreductase enzyme activities in epididymis and their control by androgen and the rete testis fluid.

The 5alpha-reductase and 3alpha-hydroxysteroid oxidoreductase enzyme activities have been measured in epididymal tissues and the control of these activities by androgens and the rete testis fluid appreciated. The highest 5alpha-reductase enzyme activity was found in the caput, the lowest in the corpus epididymidis. Androgens have a positive control on the 5alpha-reductase but no effect on the 3alpha-hydroxysteroid oxidoreductase activity. Ligation of the efferent ducts decreased significantly both enzyme activities in the caput but not in the corpus or in the cauda epididymidis.

The value of bilateral biopsy in unilateral cryptorchidism.

Here are the results of bilateral biopsies on 52 cases of unilateral cryptorchidia. In 12 patients the scrotal testicle showed equal lesions or more important lesions in the undescended testicle. It points out the importance of systematizing the histological testicular study, keeping in mind: (1) spermatogenic index; (2) variation in spermatogonias; (3) tubular diameter, and (4) interstitial tissue. Stress is made on the innocuousness of the bilateral testicular biopsy and its indispensability since it is the only way to recognize a dysgenetic cryptorchidia and evaluate the future fertility of the patient.

Antibody-coated bacteria and pregnancy.

A study is made of 158 pregnant patients, between 3 and 9 months pregnant, in order to determine, by means of urine culture and ACB, the index of subclinical infections during pregnancy involving the renal parenchyma. The conclusion is reached that these infections occur in 4.42% of cases and out of those cases 57.14% were later shown to have an organic pathology of the excretory tracts.

A new method for detection and localization of urinary infection. The fluorescence antibody test.

The urinary sediment of 56 patients with active urinary infection were studied through a direct immunofluorescence technique in order to detect the presence of antibody-coated bacteria. In 35-36 pyelonephritis cases the results were positive. However, in 11 cases with lower urinary infection there was no sign of antibody coasted bacteria. A pathological intravenous pyelogram (IVP) was obtained from 33 pyelonephritic cases. In the flurorescent antibody (FA) test was positive, one was negative. In the remaining three pyelonephritic cases the IVP was normal the the FA test positive. In these three cases, cystography revealed the presence of ureterovesical backflow. A good statistical correlation was obtained by comparing the positive results of the test with the titer of serum antibody against the bacteria causing the infection. We have introduced some variation to the original method which permits simpler administration of this test to ambulatory patients and increases the diagnostic possibility of those cases of bacteriuria less than 100,000 bacteria/ml.

Testosterone and dihydrotestosterone concentrations in plasma, epididymal tissues, and seminal fluid of adult rats.

The concentration of testosterone (T) and 5alpha-dihydrotestosterone (DHT) in plasma, seminal fluid, and epididymis of male Wistar rats have been determined by radioimmunoassay. The DHT/T ratio rose from 0.1 in blood to 1 in testicular fluid and 4 in epididymal fluid. This ratio is 2 in tissues of "caput" epididymis and 1 in "cauda" epididymis. These data indicate a transformation of T to DHT in the epididymal tissue, particularly at the "caput" level.
