Aphasic status epilepticus.

We studied a case of focal status epilepticus with aphasia as the sole manifestation, lasting 21 days. A 77-year-old woman developed aphasic seizures followed by aphasic status epilepticus 10 months after a left temporo-occipital hemorrhage. The diagnosis was made only after EEG, and seizures were stopped by anticonvulsant medication. Aphasic status epilepticus may be only a reflection of cerebral irritability, suggesting the possibility of a functional etiology for prolonged language disorders also in adults.

[Clinical aspects of spontaneous cerebral hematomas].

CT has revolutionized management of brain hemorrhage by depicting these lesions with clarity and detail. Emerging clinical and CT criteria for case selection may clarify the role of surgery in this field.

Long-term prognosis of patients with lacunar syndromes.

A follow-up study of 107 patients with lacunar syndromes has been performed using the analysis of the survival curves and the recurrence curves for new focal cerebrovascular acute episodes. At the end of the 7th year of follow-up, the survival rate of lacunar patients was 479 per 1000, lower than the survival rate of the normal population matched for sex and age (755 per 1000). A more severe prognosis was observed in subjects over 65 years of age, in patients with pseudobulbar syndrome, hypertension and higher degree of disability. The average recurrence rate for new cerebrovascular episodes was 4.74 per 100 patient-years, much lower than that in survivors from cerebral infarction.

Evaluation of asymmetries of blood flow rate and of circulation time by intravenous radionuclide cerebral angiography in patients with ischemic completed stroke.

155 patients with ischemic completed stroke of varying severity and outcome have been evaluated by radionuclide cerebral angiography with analysis of regional time-activity curves. Two parameters have been evaluated: area under the upslope of the curve (Aup) reflecting regional blood flow rate and moment of the whole curve reflecting tracer circulation time (rABCT) Combination of these two methods ensured increased detection of perfusion asymmetries.

Periodic lateralized discharges in Creutzfeldt-Jakob disease: serial electroencephalographic studies.

Serial EEG recordings in 12 cases of Creutzfeldt-Jakob disease were analyzed. Focal abnormalities, consisting of unspecific slowing of the rhythm or periodic lateralized discharges (PLD), were observed in 50% of cases in the prodromal stage. As the disease progressed, conversion of unspecific focal alterations into PLD and of these into generalized periodic patterns was seen. This evolution and the correlation between PLD and focal epileptic phenomena suggest that the discharges are cortical in nature.

Extravasation of pertechnetate in cranial extracerebral tissues during first pass across the cerebral vasculature.

By evaluating regional cerebral first pass curves following Pertechnetate administration in common, internal and external carotid artery, it has been shown that about 20% of the injected dose of Pertechnetate extravasates in cranial extracerebral tissues during the first pass. This extravasation may lead to an overestimation of about 1 sec in the determination of regional cerebral mean transit time by intravenous radionuclide Angiography.

Relationships between moment of monitored time-activity curves and regional cerebral mean transit time following common carotid administration of a non diffusible tracer.

By intravenous Radionuclide Angiography it is possible to estimate a whole path regional Arm-Brain mean transit Time as the first moment of monitored activity about injection time. This time is composed by an input mean transit time or transit time in the vein-inflow cerebral path and a circulation time within the monitored cerebral space. By evaluating this last by means of common carotid administration it was concluded that, although calculated by residue detection and not from outflow curves, this time is not far from regional cerebral mean transit time.

[Parenteral penicillin in rats: an experimental model of periodic EEG activity].

Parenteral G Penicillin has been administered to 10 rats and EEG pattern has been recorded. High voltage spikes appeared on one hemisphere, 12 to 25 minutes after injections. Gradually spike frequency and voltage increased till periodical EEG was observed on both hemispheres. Such activity was synchronous, symmetrical, stereotyped and often accompanied by myoclonias. This pattern lasted from 45 to 100 minutes. The authors underline the analogies with the Ouabain model of epilepsy and with periodical EEG patterns in man.

Tendency to periodic recurrence of EEG changes in Lafora's disease. Case report.

A young male patient affected with Lafora's disease and concomitant mental deterioration, myoclonic jerks and epileptic seizures is reported. A cerebral biopsy showed round PAS-positive myoclonus bodies in nerve cells and neuropile. A tendency to periodic recurrence of paroxysmal activity in the EEG tracings, an unusual finding in Lafora's disease, is briefly discussed.

Assessment of 99mTc-HM-PAO parameters with SPECT for improved evaluation of blood flow in cerebral lesions.

A method for investigating the linearity between blood flow and distribution of 99mTc-HM-PAO uptake in patients undergoing single photon emission computed tomography (SPECT) is presented. This method, based on particular elaboration techniques of sequential multiple acquisitions, provides a local index: cumulative transit time (CTT) related to all factors, other than blood flow, affecting 99mTc-HM-PAO uptake. Because a correct qualitative blood flow estimation is based on the uniformity of the above factors, the occurrence of a change of this index indicates a nonlinear relation between blood flow and 99mTc-HM-PAO uptake distributions. It was shown that although linearity between blood flow and 99mTc-HM-PAO uptake may be questionable, a qualitative assessment of blood flow was correct in 100% of ischemic patients. In nonischemic patients this percentage fell to 60%.

Functional circulation time images in the diagnosis of cerebral lesions by angio CT.

An application of angio CT as an adjunct to contrast enhancement evaluation of cerebral lesions is presented. We performed rapid sequential scanning following intravenous bolus injection of an iodinated contrast medium, analyzed the time contrast curves and generated a functional circulation time image. This was accomplished by calculating on a pixel-by-pixel basis the first mathematical moment of the monitored time-density curves about injection time which corresponds to regional arm brain circulation time (rABCT). This method increased the rate of detection of cerebral lesions by contrast enhancement from 30 to 90%.

Functional circulation and blood-brain permeability images by Angio CT in the assessment of cerebral ischemia.

We performed rapid sequential computerized tomography (CT) scanning following i.v. injection of a bolus of contrast medium, and generated three functional images related, respectively, to intravascular circulation time (rABCT), vascular volume density (Vv) and blood-brain barrier (BBB) unidirectional constant uptake rate (Ki). This was accomplished by calculating the first mathematical moment of the monitored time-density curves about the injection time, and by the multiple time graph analysis described by Patlack and coworkers. A satisfactory resolution was achieved, allowing separate appreciation of changes in rABCT both at large vessels and at tissue small vessels. Combined evaluation of rABCT and Vv images allowed us to draw qualitative conclusions about blood flow and perfusion reserve.

Functional circulation images by angio-CT in the assessment of small deep cerebral infarctions.

We analyzed circulation time (rABCT) and vascular volume density images obtained by angio-computerized tomography (angio-CT) in 63 patients with small deep cerebral infarctions. Abnormalities in the rABCT image were found in 88% of the patients and in the vascular volume image in 48%. Two groups with different clinical pictures were picked out by rABCT changes: one with major-vessel involvement, the other with small-vessel involvement. The perfusional changes found were mainly due to altered vascular canalization rather than to altered vasomotility. The hemodynamic theory could explain the spatial relations between perfusion changes and CT hypodense areas without needing assumptions linking blood flow and metabolism.

Intravenous 99mTc HM-PAO radionuclide angiography as an adjunct to SPECT assessment of cerebral disease.

An application of intravenous radionuclide angiography as an adjunct to SPECT with 99mTc HM-PAO is presented. From the angiographic sequence a two-dimensional parametric image is generated computing, pixel by pixel, the center of gravity (COG) of the local time curves. This index is independent of blood flow and reflects the relation between transmitted and extravasated activity. COG value, compared in two symmetrical regions analyzed for differences in flow by SPECT, may help to state whether a detected SPECT changed uptake is linearly related or not to the corresponding change of blood flow. All 23 ischemic patients taken into consideration had a similar backflux in symmetrical regions with conspicuous changed uptake. In 3 meningiomas and 2 arteriovenous angiomas it was possible to detect the occurrence of a changed backflux in the lesion suggesting a nonlinearity between blood flow and 99mTc HM-PAO uptake.
