[A comparative study of intravenous immunoglobulin and plasmapheresis preoperatively in myasthenia].

INTRODUCTION: Thymectomy is a well established procedure in myasthenia gravis (MG). To reduce its morbidity, treatments which are effective in the short term, such as plasmapheresis and intravenous immunoglobulin (IGI) have been used. OBJECTIVE: To evaluate the efficacy and tolerability of the IGI, compared with plasmapheresis, in the preparation of myasthenic patients before thymectomy. PATIENTS AND METHODS: We compared a group of 33 prospective myasthenic patients treated with IGI with 38 clinical histories taken as controls treated by plasmapheresis during the peri operative period of thymectomy. RESULTS: The patients treated with IGI were in the intensive care unit and neurology ward for less time. The endotracheal tube was also removed sooner. However, these differences were not significant. The commonest complications of IGI were fever, shivering and phlebitis. The most frequent adverse reaction to plasmapheresis were cutaneous eruptions. One patient developed Hepatitis C after plasmapheresis. CONCLUSION: IGI is comparable in efficacy to plasmapheresis in the peri operative period of MG, but has a better profile of adverse reactions.

[Development of a questionnaire for the assessment and quantification of overweight and obesity related lifestyles].

INTRODUCTION: Lifestyle intervention is mandatory for obesity treatment. The aim of this study is to design a questionnaire to describe and quantify those behaviours more closely related to obesity in the Spanish obese population. METHODS AND PROCEDURES: An expert panel designed a preliminary 57 Liker-type item questionnaire, which was self-administered to 335 overweight patients (110 male, 225 female; age, 42 +/- 14 years; BMI, 32.6 +/- 3.7 kg/m2). After a subjacent dimensionality searching and item reducing first phase, a shrunk questionnaire of 24 items was then self-administered to 156 overweight patients (52 male, 104 female; age 42 +/- 12 years; BMI, 33.1 +/- 3.5 kg/m2); 56 of those patients were re-administered the questionnaire in order to provide test-retest information. RESULTS: Final questionnaire includes 22 items clustered in five dimensions: diet caloric intake, searching for psychological well-being eating, physical activity, healthy eating and alcohol intake. Proposed factorial structure is mostly reproduced in different samples and using different extraction methods: all dimensions but alcohol intake score alpha values > 0.75 for liability; test-retest stability is greater than 0.90 in all dimensions but alcohol intake; results for all validity tests performed (of construct, of content and discriminative) are highly satisfactory. CONCLUSION: Metrics study results (liability and validity) demonstrate that the proposed questionnaire provides an excellent tool to assess those lifestyles related to obesity control.

[The use of computed tomography in assessing parotid masses].

Thirty-one patients with suspected parotid masses have been studied by computed tomography (CT) without and with contrast to demonstrate the existence of this and to value possible relation between degrees of malignancy and the morphological characteristics of the masses.

[On parosteal lipomas of the limbs].

Among the benign neoplasias of mesenchymal origin, parosteal lipomas represent a relatively rare occurrence. They are deep seated (subfascial) lipomas which during the course of their slow growth become adherent to the periosteum. The parosteal lipoma is not dissimilar to the so-called common or superficial lipoma both in terms of its macroscopic appearance, since this neoplasia has no capsule which separates it from surrounding tissue and likewise enables an easy enucleation owing to the presence of a cleavage plane, and in terms of its microscopic appearance since it is made up of mature adipose cells. Moreover, comparative cytogenic tests have revealed chromosomic alterations common to both the superficial and parosteal lipoma. The peculiarity of the parosteal lipoma lies in its clinical evolution which depends on the original site; it is in fact most often described in relation to bone segments of the limbs where it may lead to the compression of the vasculonervous axes. In particular, parosteal lipoma of the forearm, especially if localised in the upper third, represents a separate clinical entity given that, during the course of its growth, it may cause the compression of the posterior interosseous nerve and the surface (sensitive) branch of the radial nerve. This occurrence, which is observed in approximately half of all cases, must be taken into consideration and should lead to the decision to operate in order to prevent compressive sequelae involving nervous axes. After reviewing the literature on the subject, the authors report their experience of four recent cases and evaluate the anatomical, clinical, diagnostic and therapeutic aspects of parosteal lipoma.
