Release of gastrin by vagal stimulation in the acid secretory response to food.

Gastric pouches were constructed in 8 dogs; in 4 they were of denervated type and in 4 the innervation was intact. An oesophageal fistula was then prepared in each dog. The acid secretory response to oral, tube and sham feeding was determined before and after denervation of the pyloric antrum. The results support the view that vagal release of gastrin makes a relatively small contribution to the total acid secretory response to food.

The inhibitory influence of the pyloric antrum on gastric secretion in dogs.

The effect of the pyloric antrum and of subsequent vagotomy on the secretory responses of an innervated or a denervated gastric pouch in the dog to histamine and pentagastrin was determined. The increased sensitivity of the gastric pouch to stimulation after these procedures is in keeping with the concept that an inhibitory reflex is concerned with the control of acid gastric secretion.

Needle aspiration of breast cysts.

A total of 149 breast cysts have been treated with primary aspiration. Findings were abnormal in 36 patients, 30 of whom underwent excision of the breast cyst with cancer being found in four. Careful examination of the breast after aspiration and follow-up are necessary, but, provided these precautions are observed, this type of treatment is simple, safe, and cheap besides often giving the patient relief at the first consultation.

Lymph-node biopsy during simple mastectomy.

The distribution of pectoral (external mammary) nodes identified during the operation and removed with the axillary tail of the breast was studied in 45 patients treated by simple (total) mastectomy. Up to 13 nodes may lie within the axillary tail, and these are continuous with the pectoral nodes. Lymph-nodes were identified in 90 percent of patients treated by simple (total) mastectomy without dissection of the axilla.

Urinary aetiocholanolone in patients with early breast cancer from South East Scotland and South Wales.

Urinary aetiocholanolone levels have been measured in 417 women aged between 20 and 70 years. The women were drawn from South East Scotland and South Wales and consisted of patients with either benign or malignant disease of the breast and control patients suffering from no detectable breast disorder. The pattern of aetiocholanolone excretion with respect to age and menopausal status has been defined in each group of patients. No significant differences in urinary levels have been detected between patients with breast disease, whether benign or malignant, and control patients. More detailed examination of the 201 women with early cancer of the breast has also shown that there is no consistent correlation between pre-operative aetiocholanolone levels and factors of prognostic significance detectable at the time of primary treatment-tumour size, grade, round cell infiltration, histological involvement of nodes by tumour and the clinical palpability of lymph nodes. It would seem, therefore, that the prognostic value of pre-operative aetiocholanolone measurements is somewhat limited in patients with early breast cancer. It is noted, however, that low levels of aetiocholanolone are associated with post-menopausal patients, a group in which the prognosis is generally poorer than that in pre-menopausal women.

Effect of denervation of pyloric antrum on the gastric secretory response to food.

The effect of selective denervation of the pyloric antrum on secretory response to three types of food was determined in 3 dogs with innervated and 4 with denervated stomach pouches. With the antrum in situ this procedure has surprisingly little effect on the total meal responses in innervated pouches, on the variation in response induced by altered buffering capacity of the meals, or on the effect of subsequent vagotomy. However, the antrum appears less sensitive to local stimuli after denervation, as evidenced by the reduced meal response from the denervated indicator pouch after denervation of the antrum.

A light and electron microscopic study of the changes in the duodenal mucosa induced by sustained pentagastrin stimulation of gastic acid secretion in young rats.

The histological and electron microscopic changes in the duodenal mucosa have been studied over an 8-week experimental period in rats whose gastric acid secretion was stimulated by pentagastrin in a medium of beeswax and mineral oil, and in controls that received injections of beeswax and mineral oil only. The changes leading of final disintegration of the epithelium are described in detail. The main aim of the experiment--to induce mucous change in the duodenal mucosa--was not realized.
